Duplication of the inferior vena cava is a rare anatomic anomaly (0.3%–3%) that is caused by the anomalous regression of the left supracardinal vein in the first trimester of pregnancy. Preoperative diagnosis in retroperitoneal approaches minimizes the risk for hemorrhage (Fig. 1).
Our patient is a 70-year-old male, who is a smoker with diabetes mellitus. He underwent scheduled surgery for abdominal aortic aneurism measuring 5.2cm, using a xipho-pubic incision with evisceration of the intestines to access the retroperitoneal space. We observed that the left inferior vena cava crossed over the infrarenal aorta anteriorly. The bridge between the two venae cavae was cut and ligated. Dissection was performed with aorto-aortic graft insertion.
