The patient is a 74-year-old male who came to our consultation due to bowel obstruction. He presented a distended abdomen with pain in the hypogastrium and signs of peritoneal irritation. Physical examination ruled out the existence of inguinal hernia. CT revealed distension of the small bowel, and the appearance of the terminal ileum in the hypogastrium was suggestive of band obstruction (Fig. 1). Laparotomy was performed. An internal supravesical hernia was observed that contained the ileal loop with signs of ischemia, which improved after its release (Fig. 2). The defect was closed with continuous polydioxanone suture. Internal supravesical hernias should be considered in cases of intestinal obstruction without previous surgery or signs of complicated external hernias.
