A 69-year-old man presented to the emergency department with a 48-hour history of intestinal obstruction and tumour in the right iliac fossa. CT scan revealed a small bowel obstruction secondary to a Spiegel's hernia, together with an appendix incarcerated within. A laparoscopic approach was decided, performing a hernia reduction together with appendectomy, transabdomino-preperitoneal repair and subsequent small bowel resection through a Pfannenstiel incision.
