Afecta a la población joven, con una incidencia creciente y suele cursar en brotes que habitualmente responden a tratamiento médico, aunque el 50% necesitará cirugía a lo largo de su enfermedad. La zona más frecuentemente afectada a nivel abdominal es el íleon terminal. La fisiopatología de la EC no es bien conocida, y ni el tratamiento médico ni la cirugía curan la enfermedad. De hecho, tradicionalmente se ha indicado la cirugía sólo en casos de complicaciones como estenosis, fistulización o malignización. Esto es interesante si pensamos que –en los albores de la descripción de la EC– el papel de la cirugía parecía mucho más prominente<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">1,2</span></a>. La recurrencia de la enfermedad tras la cirugía es la norma, con un 90% de recurrencia endoscópica al año, recurrencia clínica del 50% a los cinco años y necesidad de nueva cirugía del 25% a los cinco años<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">2</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">En tiempos recientes ha resurgido el interés en la posibilidad de que la cirugía pueda influenciar en la evolución natural de la enfermedad. Se han publicado varios estudios importantes al respecto que nos han hecho pensar en un posible cambio de paradigma en el tratamiento de la EC. El estudio LIR!<span class="elsevierStyleSmallCaps">C</span> mostró que la cirugía temprana en EC de íleon terminal de fenotipo inflamatorio presentaba una calidad de vida a los 12 meses comparable con la observada con tratamiento biológico en pacientes con afectación de menos de 40<span class="elsevierStyleHsp" style=""></span>cm de íleon y sin abscesos. Además, aproximadamente un cuarto de pacientes tratados con cirugía necesitó tratamiento con biológicos, mientras que casi uno de tres del grupo tratado con biológicos necesitó cirugía en un seguimiento mediano de cuatro años. Un análisis sucesivo del estudio LIR!C ha demostrado que, con un seguimiento mediano de 63,5 meses, ninguno de los pacientes intervenidos necesitó otra cirugía, y 26% requirieron biológicos, siendo el tratamiento quirúrgico más «coste-efectivo» que el tratamiento con fármacos<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">3</span></a>. Datos de vida real de un estudio global que ha analizado a más de 200 pacientes operados por EC abdominal han demostrado que los intervenidos de forma «temprana» (antes de desarrollar un daño estructural; es decir, con enfermedad principalmente inflamatoria) tuvieron mejores resultados postoperatorios, menor tasa de complicaciones y tasas de recidivas razonables, con un riesgo de recurrencia dos veces más alto en los pacientes tratados con biológicos antes de la cirugía<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">4</span></a>. Resultados similares han sido confirmados en un análisis de más de 2.000 pacientes, además demostrando que los pacientes intervenidos por EC inflamatoria necesitan resecciones menos extensas, menos procedimientos asociados y se operaron más frecuentemente con cirugía mínimamente invasiva<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">5</span></a>. Estos datos demostraron que el <span class="elsevierStyleItalic">timing</span> de la cirugía juega un papel fundamental al momento de optimizar los resultados y disminuir las recidivas de enfermedad.</p><p id="par0015" class="elsevierStylePara elsevierViewall">El estudio de Coffey et al., publicado en 2018, introdujo otro concepto en la disminución de las tasas de recidiva quirúrgica: la necesidad de eliminar el mesenterio afectado por EC en estos pacientes<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">6</span></a>. El estudio mostró que la resección más amplia del mesenterio (incluyendo el <span class="elsevierStyleItalic">creeping fat</span> y la zona mesentérica más engrosada) reducía la recurrencia postquirúrgica. Estos resultados han sido criticados por unos sesgos intrínsecos del estudio, principalmente, la cohorte de control histórico y el hecho de haber sido efectuado en un solo centro. Es importante tener en cuenta también que una escisión del mesenterio puede comportar un aumento de las complicaciones perioperatorias.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Al mismo tiempo, el profesor Kono publicaba sus resultados con una nueva configuración anastomótica, la anastomosis Kono-S, que incluye una columna de soporte que «aísla» la anastomosis del mesenterio (lo que justifica la importancia de la «S» en el nombre del procedimiento, que significa <span class="elsevierStyleItalic">supporting column</span> –concepto que está en línea con la teoría presentada por Coffey). Un estudio randomizado realizado en un centro de Italia, el SuPREMe-CD <span class="elsevierStyleItalic">study</span>, encontró una reducción de la recurrencia endoscópica a los 18 meses en el grupo de anastomosis Kono-S, frente a la anastomosis latero-lateral mecánica<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">7</span></a>. En una revisión sistemática publicada en 2021, los autores sugieren que estos cambios en la técnica quirúrgica podrían tener una influencia importante en la reducción de la recurrencia postquirúrgica, y quizás deberían ser adoptados por todos los cirujanos dedicados a la enfermedad inflamatoria intestinal<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">8</span></a>. Se lanzaron varios estudios multicéntricos randomizados para comprobar estos resultados, y crecieron las esperanzas de que los cirujanos podamos ser más «protagonistas» en el tratamiento de la EC.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Sin embargo, estos estudios no han dado los resultados positivos esperados. El estudio REMEDY, efectuado en dos centros de referencia de EC en Italia, comparó la recidiva endoscópica, ecográfica y quirúrgica en pacientes con EC ileocólica sometidos a cirugía con (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>204) vs. sin (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>122) escisión del mesenterio, demostrando que no hubo diferencia entre los grupos<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">9</span></a>. Dichos resultados sugieren que la resección del mesenterio no se tendría que efectuar de forma rutinaria.</p><p id="par0030" class="elsevierStylePara elsevierViewall">El estudio internacional randomizado SPICY tampoco encontró diferencias significativas entre la resección más amplia del mesenterio y la resección estándar<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">10</span></a>. El estudio multicéntrico francés KoCoRICCO, publicado en 2023, no encontró diferencias en la recurrencia endoscópica a los seis meses entre la anastomosis Kono-S y la latero-lateral<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">11</span></a>; y en el congreso de la <span class="elsevierStyleItalic">European Crohn's and Colitis Organization 2024</span> (ECCO 2024) se presentaron resultados de otro estudio randomizado internacional comparando la anastomosis Kono-S y la estándar. En dicho estudio se randomizaron 288 pacientes con EC a tratamiento con Kono-S (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>154) versus anastomosis latero-lateral (n<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>134): la recidiva endoscópica y quirúrgica a los 3-6 meses fueron iguales entre los grupos, así como el tiempo libre de enfermedad<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">12</span></a>.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Es importante tener en cuenta que existen varios aspectos de heterogeneidad y fuentes de posibles sesgos que se tienen que valorar cuando se interpretan los estudios sobre la utilidad de nuevas técnicas quirúrgicas que intenten disminuir la tasa de recidiva en EC. Mientras que la técnica de anastomosis latero-lateral mecánica ha sido bastante estandardizada, se están publicando numerosas variaciones de la anastomosis Kono-S, aportando variabilidad al procedimiento y complicando la homogeneidad de los resultados. Así mismo, la extensión de la escisión del mesenterio queda bajo el juicio del cirujano que efectúa el procedimiento.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Los autores del octavo <span class="elsevierStyleItalic">ECCO Scientific Workshop</span> sobre la recidiva después de tratamiento de le EC ileocecal, sugieren esperar a los resultados de todos los estudios en marcha sobre Kono-S anastomosis, y concluyen que en el momento actual los beneficios de dicha anastomosis quedan por ser demostrados<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a>. Esta posición parece prudente. Por lo tanto, de momento, no tenemos una evidencia clara de que los cirujanos podamos modificar las tasas de recurrencia sólo con un cambio de técnica quirúrgica. Es cierto que el diablo está en los detalles, y conocer los problemas que existen hace que tengamos más claro cómo interpretar los datos y hablar con nuestros pacientes.</p><p id="par0045" class="elsevierStylePara elsevierViewall">El tiempo y «timing» de la cirugía, los tratamientos médicos y la optimización perioperatoria probablemente son más importantes que el efecto de la técnica en sí<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">14</span></a>. Es probable que la individualización de cada caso, con tratamiento personalizado, constituya la dirección a seguir. Será clave que en los próximos años se efectúen estudios de largo seguimiento, utilizando definiciones y «<span class="elsevierStyleItalic">endpoints</span>» de tratamiento homogéneos. Más que nunca, todas estas decisiones deben ser tomadas en equipo multidisciplinar, en estrecha colaboración con los gastroenterólogos, y, por supuesto, debemos utilizar la técnica que nos dé más seguridad, sin poner en riesgo al paciente, intentando evitar las complicaciones.
