Resumen

La artritis séptica esternoclavicular es una entidad poco frecuente en el adulto. El microorganismo aislado más frecuentemente como causa de la misma es Staphylococcus aureus. La afectación por Streptococcus betahemolítico del grupo b (S. agalactiae) es excepcional y debe sospecharse en pacientes diabéticos o con cirrosis hepática.

El retraso en la instauración del tratamiento adecuado condiciona la progresión del cuadro pudiendo aparecer extensión mediastínica y hacia pared torácica.

El tratamiento quirúrgico agresivo es fundamental para la resolución de estos cuadros. Los defectos creados deben ser cubiertos por colgajos musculares que mejoran la vascularización de la zona y protegen las estructuras profundas.

Septic arthritis of the sternoclavicular joint is a rare entity in adults. The most frequently isolated microorganism causing this disease is Staphylococcus aureus. Involvement of group B beta-hemolytic Streptococcus (S. agalactiae) is exceptional and should be suspected in patients with diabetes or liver cirrhosis. Delay in the initiation of appropriate therapy affects disease progression, with mediastinum and thoracic wall involvement.

Aggressive surgical treatment is essential to resolve these cases. The defects created should be covered by muscular flaps that improve vascularization of the area and protect the deep structures.
