Introduction

Isosulfan blue is the most frequently used vital dye in selective sentinel lymph node biopsy in breast cancer. However, because of its limited availability in Spain, other vital dyes are increasingly being used, especially methylene blue. The aim of this study was to compare the utility of methylene blue vs isosulfan blue in localizing sentinel lymph nodes in patients with breast cancer.

Method

We performed a prospective, randomized study in 89 patients in whom 91 traces and 94 sentinel lymph node biopsies were performed. In all procedures, a peritumoral isotopic tracer injection and subareolar vital dye injection were performed: methylene blue was used in 45 patients and isosulfan blue in 46 patients. The parameters recorded were: sentinel node detection or not, agreement between isotopic and vital dye results, adverse effects, the number of sentinel nodes and their location (axillary basin/internal mammary basin). These parameters were compared between the two groups.

Results

Sentinel lymph nodes were not detected in only one patient (1.06%). No significant difference was found in the mean number of sentinel nodes per drainage or in extra-axillary location. The agreement between isotope and vital dye was 100% for both groups. No adverse effects attributable to vital dye were recorded.

Conclusions

In patients with breast cancer, methylene blue is an alternative to isosulfan blue in sentinel lymph node biopsy.
