¿Ha disminuido la incidencia de la cirugía por estenosis pilórica péptica en el último cuarto de siglo?

Introducción

Aunque existe un descenso muy importante de la cirugía por úlcera duodenal no complicada, no es bien conocido el efecto que la generalización de su tratamiento médico ha ocasionado sobre las complicaciones (hemorragia, perforación y estenosis) de la úlcera. Eneste trabajo, estudiamos la evolución de la incidencia de cirugía por estenosis pilórica péptica (EPP) por úlcera duodenal durante un período de 25 años.

Pacientes y métodos

Un total de 158 pacientes intervenidos con el diagnóstico de EPP durante 25 años, incluidos en una misma área geográfico-sanitaria. Se cuantificaron los casos intervenidos anualmente y se agruparon en períodos de 5 años para su comparación estadística basándonos, aproximadamente, en la introducción de los antihistamínicos H2 e inhibidores de la bomba de protones en nuestro medio.

Resultados

El número de casos intervenidos por año muestra un descenso progresivo durante los 25 años considerados, encontrando un descenso significativo desde 1991.

Conclusiones

Existe una diferencia significativa entre la indicación del tratamiento quirúrgico por EPP en los años previos y posteriores al uso de los inhibidores de la bomba de protones y el tratamiento erradicador de Helicobacter pylori.

Introduction

Although there has been a considerable decrease in surgery for uncomplicated duodenal ulcer, the effect of the widespread use of medical treatment on complications (hemorrhage, perforation and stenosis) is not well known. We studied the evolution of the incidence of surgery for peptic pyloric stenosis due to duodenal ulcer over a 25-year period.

Patients and methods

The study included 158 patients with a diagnosis of peptic pyloric stenosis who underwent surgery over a 25-year period within the same health district. The yearly intervention rate was quantified and the patients were divided into 5-year periods for statistical analysis. This was approximately based on the introduction of histamine H2- receptor antagonists and proton pump inhibitors in our environment.

Results

The yearly intervention rate progressively decreased over the 25 years studied. From 1991 this decrease was significant.

Conclusions

A significant difference was found between the incidence of surgical treatment of peptic pyloric stenosis before and after the introduction of proton pump inhibitors and Helicobacter pylori eradication therapy.
