Manejo perioperatorio de un paciente con alergia al látex

Las reacciones de hipersensibilidad inmediata desencadenadas por productos derivados del látex se están incrementando debido al contacto cada vez más frecuente de la población con este tipo de compuestos. Se presenta el caso de una paciente con alergia demostrada al látex que presentó disnea y prurito ocular en el postoperatorio de una artroscopia. Además, se propone un protocolo de actuación para el manejo de este tipo de pacientes en el medio hospitalario, donde generalmente se presentan los casos más graves. Se incluye el modo de establecer el diagnóstico en caso de sospecha preoperatoria, el material de quirófano que hay que revisar para ofrecer un ambiente sin látex, las medidas eficaces de tratamiento en caso de que se desencadene una reacción anafiláctica, así como el modo de establecer el diagnóstico inmunoalérgico posterior

There are an increasing number of immediate hypersensitivity reactions triggered by latex-based products as a result of frequent previous exposure to rubber materials. It has been recognized not only in high-risk patients (health care providers, patients with spina bifida or a history of atopy), but also in the general population. Severe, life-threatening anaphylaxis to latex generally occurs in the hospital environment. We present the case of a patient with demonstrated latex allergy scheduled for orthopedic surgery. The procedure was uneventful but during the postoperative period, the patient developed dyspnea that disappeared after the administration of steroids and antihistamines. We suggest a protocol to manage these patients while hospitalized, indicating how to identify the high-risk population and how to confirm the diagnosis. We also include a detailed list of the material to be replaced during the postoperative period, and discuss the management of an anaphylactic episode and how to establish the diagnosis of an immune-mediated allergic reaction
