Soft tissue sarcoma consists of neoplastic proliferation of extraskeletal connective tissue. These tumors represent 9.8% of all adult tumors and provoke 2% of total mortality. We present a case of sarcoma localized in the oblique muscles of the abdominal wall in a 68-year-old woman who presented to the emergency department because of pain in the right iliac fossa. Radiologically-guided aspiration needle biopsy revealed a high-grade pleomorphic sarcoma. Surgical intervention with substitution of the resulting abdominal wall defect with a double prosthesis consisting of dura mater and polypropylene produced satisfactory results.
