We present four cases of penetrating injuries to the common carotid and subclavian arteries. The associated lesions were slight and included a contused right brachial plexus that healed without sequelae. One patient showed widening of the mediastinum, which indicated the diagnosis. The carotid artery injury was approached through vertical neck incisions, anterior to the sternocleidomastoid. The right subclavian artery was approached through a transverse supra-and intraclavicular incision and the left subclavian artery through a left posterolateral thoracotomy and left infraclavicular incision. All arterial repairs remained patent and without complications during the 5-year follow-up
