Coagulopatía de consumo y embarazo. Un caso raro de afibrinogenemia del segundo trimestre

Resumen

Presentamos un caso de desprendimiento placentario a las 20 semanas y 4 días de gestación, asociado a una coagulopatía de consumo con pruebas de coagulación que hacían pensar en una fibrinólisis primaria más que en la coagulación intravascular diseminada clásicamente descrita. Una revisión de la bibliografía nos aporta 6 casos de coagulopatía durante el segundo trimestre, de los cuales solamente tres fueron debidos a una fibrinólisis primaria. Apuntamos las posibilidades terapéuticas de esta coagulopatía.

Summary

We present a case of placental abruption at 20 weeks and 4 days of pregnancy associated to a consumptive coagulopathy with coagulation tests which suggested primary fibrynolysis rather than a classically described disseminated intravascular coagulation. A literature review revealed 6 cases of coagulopathy in the second trimester of which only 3 were due to a primary fibrinolysis. Management of this coagulopathy is discussed.
