Presentamos una serie de 40 casos de hidrómetraen mujeres posmenopáusicas con una media de edadde 66 años (intervalo, 38-86 años), con diagnóstico ecográfico. Clínicamente, consultaron por revisiónanual. Metrorragia posmenopáusica o senil hubo sóloen 8 casos (20%).Citológicamente, el aspirado del líquido, en 8 casos, era poco celular, y había placas decélulas endometriales con cambios degenerativos propios de la atrofia. Histeroscópicamente, de 13 casosencontramos pólipos endometriales en 6 casos (15%)e hiperplasia endometrial en 3 casos (7,5%). No huboningún cáncer endometrial.
Concluimos que el hallazgo casual de hidrómetraen la mujer posmenopáusica asintomática no es demal pronóstico.En el tiempo persiste el hallazgo sóloen 7,5%. La citología del líquido y la histeroscopiaencuentran atrofia. Pero en caso de metrorragia posmenopáusica o senil, para buscar el cáncer endometrial, debe hacerse un estudio completo con citologíadel líquido, histeroscopia y biopsia endometrial o legrado. Lo importante no es el líquido, sino el espesorendometrial de alrededor.
We present a series of 40 cases of hydrometra in postmenopausal women with a mean age of 64 years (range 38-86 years), with ultrasound diagnosis. Their consultation was for a yearly check-up. Postmenopausal or senile metrorrhagia was present in only 8 cases (20%). In 8 cases cytology of the aspirate did not show many cells and there were plaques of endometrial cells with degenerative changes due to atrophy. Hysteroscopically, in 13 cases we found 6 cases of endometrial polyp (15%) and 3 cases of endometrial hyperplasia (7.5%). No cases of endometrial cancer were found.
We conclude that hydrometra as a casual finding in asymptotic postmenopausal women does not mean a bad prognosis. Over time it persists in only 7.5% of cases, cytology of the liquid and hysteroscopy show only atrophy. But in cases of postmenopausal or senile metrorrhagia, a complete cytological study of the liquid, hysteroscopy and endometrial biopsy or curettage must be carried out, to rule out endometrial cancer. The liquid itself is not important, but the thickness of the endometrium around it is.
