Sistema intrauterino de liberación de levonorgestrel en el tratamiento de la hemorragia uterina disfuncional

Summary

Introduction:Dysfunctional uterine bleeding is a common gynaecological condition for which medical treatment is often ineffective, and a large number of women undergo surgery.

Objective:To assess the effect of a levonorgestrel releasing intrauterine device in the management of menorrhagia.

Material and methods:The study included 47 premenopausal women with menorhagia. Menstrual loss was estimated with a pictographic chart and the measurement of serum haemoglobin and ferritin levels preinsertion and at six and twelve months postinsertion. We also evaluated the degree of satisfaction with the treatment.

Results:Menstrual blood loss was significantly reduced after six months (70%) and after twelve months the reduction was 78%. There was a significant increase in serum ferritin during the first year. The degree of satisfaction was high.

The degree of satisfaction was high. Conclusion:The levonorgestrel releasing intrauterine device is an effective nonsurgical treatment for the management of dysfunctional uterine bleeding and seems to be an important alternative to oral medication.

Resumen

La hemorragia uterina disfuncional es un trastornoginecológico frecuente en el que los tratamientos médicos son a menudo inefectivos y un gran número demujeres se somete a cirugía. Intentamos valorar elefecto de un sistema intrauterino de liberación de levonorgestrel en el tratamiento de la menorragia.

Se incluyen 47 mujeres premenopáusicas con menorragia. La pérdida menstrual fue estimada medianteuna tabla pictográfica y los niveles séricos de hemoglobina y ferritina antes de la inserción y a los 6 y 12meses postinserción. Valoramos el grado de satisfacción con el tratamiento.

La pérdida sanguínea menstrual se redujo de formasignificativa después de 6 meses (70%), y a los 12meses la reducción fue del 78%. Apreciamos un incremento significativo de la ferritina sérica durante elprimer año. El grado de satisfacción fue elevado.

El sistema intrauterino de liberación de levonorgestrel es un efectivo tratamiento no quirúrgico enel manejo de la hemorragia uterina disfuncional yse convierte en una alternativa importante a la medicación oral.
