Resumen

El tumor de células de Sertoli-Leydig de ovario es poco frecuente. Es predominantemente sólido pero puede haber áreas quísticas; cuando tiene abundante componente heterólogo de tipo intestinal puede ser predominantemente quístico. El contenido de células de Sertoli y de Leydig es variable,y puede ser muy escaso. Este conjunto de hechos puede llevar a confundir esta lesión con un tumor mucinoso quístico. Enalgunas ocasiones se ha descrito elevación de AFP,lo que obliga a plantear el diagnóstico diferencial con un tumor de células germinales. El tumor de células deSertoli-Leydig se presenta el 80% de las veces en unestadio IA,y dado que en general se presenta en mujeres jóvenes en edad fértil,el tratamiento es conservador,y una anexectomía simple es suficiente. El pronóstico,generalmente bueno,está en función del estadio y del grado de diferenciación.

Se presenta el caso de una paciente de 25 años de edad con polimenorreas,aumento del volumen abdominal y elevación de AFP sérica.

Abstract

ovarian sertoli-leydig cell tumour is an infrequent neoplasm. it is predominantly solid but cystic areas may be present; however when an extensive heterologous component is present it may be predominantly cystic. the number of sertoli and leydig cells is variable, sometimes being very scarce. these findings may lead to an erroneous diagnosis of mucinous cystic tumour. an increase in the serum alphafetoprotein level has occasionally been reported in these tumours, usually suggests a germinal cell tumour, which must be ruled out. at the time of diagnosis 80% of sertoli-leydig cell tumours are at stage ia. this, along with the fact that most patients are young and fertile, leads to a conservative approach and a simple adnexectomy is considered sufficient. prognosis is usually good, and correlates with the stage and grade of differentiation of the tumour. we report the case of a 25 year old lady with menorrhagia, increased abdominal girth, and serum elevation of alpha-fetoprotein.
