Colombia" ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">Extreme maternal morbidity is a state in which a woman is at risk of dying, but survives a complication that occurred during pregnancy, birth or within the 42 days after the termination of pregnancy.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Some of the deaths which occur are preventable, such as those caused by severe haemorrhages, infections, hypertensive disorders, complications during birth and abortions.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Maternal mortality may also occur as the result of problems associated with the quality of medical care. High quality obstetric care, as is the case for evaluation of the care process, is a key factor to reduce not only maternal mortality, but also maternal morbidity. Evidence-based decision-making for correct interventions is the foundation for the prevention of new cases.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,4</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Applying the Early Warning Scale gives rise to a major advantage, as this tool makes it possible to improve clinical interventions and activate the health team in case of alterations. This scale evaluates vital parameters according to the Confidential Enquiry into Maternal and Child Health (CEMACH), thereby facilitating the detection of adverse results.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">An observational descriptive study analysed 204 pregnant women who were admitted to the Gynaecological–Obstetric Department for more than one day in a high complexity institution in Colombia. Alterations in their vital parameters were recorded (temperature, systolic and diastolic pressure, heart and respiratory rates, oxygen saturation and state of awareness) together with their clinical characteristics.</p><p id="par0025" class="elsevierStylePara elsevierViewall">At the time of the study the pregnant women were aged from 14 to 45 years old, with an average age of 27.5 years and a standard deviation (SD) of 6.6 years. The duration of their hospitalisation varied from 2 to 29 days, with an average of 6 days and a SD of 3.6 days. 48% of the participants were pregnant for the first time, with a confidence interval of 95% (CI 95%) of 41.0%–55.1%. 64% of the pregnant women were in the last three months of pregnancy (CI 95%: 57.7%–71.3%). The shortest gestational age was 3.4 weeks and the longest was 40.5 weeks, with an average of 28.1 weeks and a SD of 9.2 weeks.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Of the pregnant women with a history of obstetric problems, 82% of these were associated with abortion (CI 95%: 68.6%–91.4%), foetal death in 8% (CI 95%: 2.25%–19.2%) and 6% were associated with hypertensive disorders (CI 95%: 1.3%–16.5%). Of those who had given birth previously, 35.3% (CI 95%: 28.7%–42.3%) had finalised gestation with vaginal birth and 16.2% by Caesarean section (CI 95%: 11.4%–22.0%).</p><p id="par0035" class="elsevierStylePara elsevierViewall">Morbidity associated with pregnancy in hospitalised women was present in 70.6% of the women (CI 95%: 63.8%–76.7%). The most frequent diagnoses according to the International Classification of Diseases (ICD 10) were preeclampsia in pregnancy in 25.1% (CI 95%: 18.90%–32.2%), urinary tract infection in 13.7% (CI 95%: 9%–19.7%) and cervical incompetence, diabetes mellitus that commenced during pregnancy, the threat of abortion and preterm premature rupture of membranes, at 2.8% (CI 95%: 0.9%–6.5%) for each one of them.</p><p id="par0040" class="elsevierStylePara elsevierViewall">During hospitalisation the highest proportions of vital parameters which altered were: systolic pressure, in 70.6% of the pregnant women (CI 95%: 63.8%–75.7%), followed by diastolic pressure in 50.5% (CI 95%: 43.4%–57.5%) and to a lesser degree, alteration in oxygen saturation in 1% (CI 95%: 0.1%–3.5%).</p><p id="par0045" class="elsevierStylePara elsevierViewall">As may be seen in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>, significant differences were found in the rates at which these alterations occurred between the pregnant women who were younger than 29 years old and the older ones (<span class="elsevierStyleItalic">P</span> = .004), as well as between the group of women with a history of pathology and those with no such history (<span class="elsevierStyleItalic">P</span> = .004), and between the group hospitalised for less than 3 days and those with a longer average stay (<span class="elsevierStyleItalic">P</span> = .002). On the other hand, alterations in the parameters were found in 87.9% of the women in the final three months of pregnancy, although no significant differences were found with the women in other months of pregnancy (<span class="elsevierStyleItalic">P</span> = .460).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0050" class="elsevierStylePara elsevierViewall">Nurses play an important role in promoting health and preventing problems, and therefore in the identification of risk factors before conception. The purpose of this is to reduce the percentage of women who commence a pregnancy with comorbidities that may complicate gestation, birth and the puerperium.</p><p id="par0055" class="elsevierStylePara elsevierViewall">On the other hand, in the clinical field it is the nurses who control and evaluate clinical aspects.
