Life expectancy has increased in recent years; thus, the number of older adults has increased, and the population pyramid has become inverted.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> This has given rise to new needs in the older population associated with chronic diseases, sensory impairments, polypharmacy, dependency, and sedentary behaviours.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Inactivity in older adults may promote adverse effects such as chronic diseases and sarcopenia. However, physical activity has been proposed as an effective intervention to improve the health of the older adult population, as it maintains their independence and improves their quality of life.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">Advances in information and communication technologies (ICT) to improve quality of life, health and wellbeing is another great change we are experiencing with the increased use of technology.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> There is a plethora of ICT tools, including fixed devices such as computers; wearable devices such as smartphones, pedometers, belts and smart bands; and ambient technology, including physiological and environmental sensors, such as medication sensors, videos, sensors in the kitchen or bathroom. These devices report biometric data (pulse, blood pressure and weight), user symptoms, level of physical activity and medication compliance.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">Increased use of technology increases the digital divide, i.e., the social divide between generations. Using innovative technologies in the care of older adults can encourage their use of technology and help them benefit from these technological advances and age more healthily.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> Through the use of ICT tools applied to health, areas can be worked on such as communication, cognitive stimulation, self-efficacy, interaction, intergenerational relations, active ageing, autonomy, memory, wellbeing, nutrition, and physical activity. We can help reduce the prevalence of diseases such as depression and anxiety.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> Information technology also provides useful tools for elderly patients, as they facilitate the rapid transmission of clinical information to relatives and doctors, and can thus avoid early admission to care homes.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">Vedel et al.,<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> identified five main types of health information technologies in the field of geriatrics: telecare, electronic health records, decision support systems, web-based packages for patients and/or family caregivers, and assistive information technologies. Telecare is useful for health monitoring, rehabilitation, and remote case management and diagnosis. It also optimises the work time of healthcare staff, improves conditions and makes medical expertise more available.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Telecare has emerged because conventional telephone calls are sometimes not sufficient, and therefore education and training of older adults can be important to ensure it is used well.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> Electronic health records ensure patient care is well monitored and information passed easily between different institutions, ensuring good teamwork between professionals.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> These records replace handwritten and printed prescriptions and, if properly implemented, could ensure safe pharmacotherapy in the older adult population, improving communication between patients and practitioners.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> Finally, decision support systems are computer-based with reminder intervention, and help patients remember to take their psychoactive drugs and ensure correct drug administration in the older adult population.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">Web-based packages for patients and/or family caregivers are effective in providing information and improving patients' participation and independence in decision-making. If used before the visit, they encourage cooperation with the medical team during the visit; if used after the visit, this is because patients need more information than has been provided. These tools help to reduce the anxiety that patients can feel due to lack of knowledge, however substituting a medical visit for a search on these sites would be an example of misuse of these tools.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> Finally, assistive information technologies were developed to monitor patient self-care in normal and health-critical scenarios, treatment and health literacy.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> Because the older adult population is generally sedentary, interactive wearable devices have been developed that are beneficial for them and for public health. Several companies are marketing these devices; however, the drawback is that the consumers of these technologies are not older adults.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">The use of ICT in health has been reported as effective in developing more efficient and individualised interventions for older adults.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> It has also shown a positive effect on cardiovascular parameters, as in the case of patients with metabolic syndrome, and in controlling body weight, blood glucose and blood pressure.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> Physical activity using ICT tools seems to improve cardiovascular fitness and reduce sedentary lifestyles in older adults.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> Several studies have also confirmed the effectiveness of ICT in encouraging and increasing adherence to physical activity in older adults.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">Despite the evidence that the available technologies improve physical activity levels in older adults, no systematic review has analysed the joint effect of these interventions. Furthermore, there is no evidence whether these interventions improve cardiovascular risk parameters in older patients who increase their levels of physical activity.</p><p id="par0060" class="elsevierStylePara elsevierViewall">This review, therefore, aims to assess the effectiveness of ICT interventions in increasing physical activity levels in older adults, and establish whether these same ICT interventions have a positive effect on their cardiovascular parameters.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Methodology</span><p id="par0065" class="elsevierStylePara elsevierViewall">This review followed the recommendations of the Cochrane handbook<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> and was drawn up according to PRISMA guidelines.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a></p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Search strategy</span><p id="par0070" class="elsevierStylePara elsevierViewall">The search strategy used in this study was first piloted to test its sensitivity and specificity. Through this piloting we observed that, after modifying the search terms so that fewer studies were identified, the search became less sensitive, making it impossible to locate all the studies relevant to this manuscript. In a second phase, the final search strategy was applied using the databases PudMed, Scopus, Cochrane, and Web of Science until 20 February 2020. The full search strategy for PubMed is shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. Thirdly, we reviewed the reference list of the articles included in this review to identify any additional relevant studies (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Inclusion and exclusion criteria (eligibility)</span><p id="par0075" class="elsevierStylePara elsevierViewall">We have included controlled clinical trials, quasi-experimental studies<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> and pilot studies in this review to study the effectiveness of ICT tools in increasing physical activity levels among older adults, and to assess the effect of this increase on cardiovascular parameters. The ICT tools considered for inclusion in this review are those that serve to monitor, communicate, and track health status (e.g., pedometer, accelerometer, or tablet). The studies had to meet the following inclusion criteria: with respect to participants, general population or population with chronic illness aged 55 years or older; with respect to the characteristics of the interventions, ICT tools aimed at increasing physical activity and improving cardiovascular parameters. All articles had to be written in English or Spanish; no publication date was established for the selection of studies.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Studies were excluded if they included a population with dementia, neurological manifestations, or diseases that prevented patients from using ICT tools correctly, those that included historical cases in the control group and those whose objective was to examine the effectiveness of ICT tools to prevent falls.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Data search and extraction</span><p id="par0085" class="elsevierStylePara elsevierViewall">We collected full information from the selected studies and summarised the following data for each included study in a table: authors of the article, type of study, year of publication, country of study, number of participants (for control and intervention group), age of participants, condition of participants (for control and intervention group), characteristics of the physical activity interventions, and baseline data on outcome variables, including daily steps of physical activity (PA), minutes per week of PA, minutes per day of PA, days per week of PA, glycosylated haemoglobin, systolic blood pressure, diastolic blood pressure, and body mass index (BMI) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Evaluation of methodological quality</span><p id="par0090" class="elsevierStylePara elsevierViewall">The risk of bias of randomised controlled clinical trials (RCTs) was assessed using the Cochrane Collaboration's tool for assessing risk of bias (RoB2).<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> This tool has six domains: 1) randomisation process; 2) deviations from predetermined interventions; 3) missing data on variables of interest; 4) measurement of variables of interest; 5) selection of reported outcomes; and 6) overall risk. The overall risk of bias for each article was categorised according to the following: “low risk of bias” if all domains are classified as “low risk”, “some concerns” if there is at least one domain classified as “some concern”, and “high risk of bias” if there is at least one domain classified as “high risk”, or several domains classified as “some concerns”.</p><p id="par0095" class="elsevierStylePara elsevierViewall">For non-randomised controlled clinical trials (non-RCTs) and quasi-experimental studies, we used the tool for assessing risk of bias in non-randomised studies of interventions (ROBINS-I).<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> This tool has seven domains: confounding, selection of study participants, classification of interventions, deviations from intended interventions, missing data, measurement of outcomes, and selection of reported result. The overall risk of bias is rated as: “low risk” if all domains are rated as “low risk”, “moderate risk” if all domains are rated as “low risk” or “moderate risk”, “serious risk” if there is at least one domain rated as “serious risk”, “critical risk” if there is at least one domain rated as “critical risk”, and “no information” if there is no clear indication that the study is at “serious risk” or “critical risk” of bias and if there is a lack of information in one or more key domains of bias.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Two independent investigators (MILLT and JVO) conducted the search, data extraction and assessed the methodological quality of the studies included in this review.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Results</span><p id="par0105" class="elsevierStylePara elsevierViewall">The search yielded 1140 articles, of which 11,<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17,18,25–33</span></a> whose objective was to assess the effectiveness of interventions, were included in this review.</p><p id="par0110" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRefs" href="#tbl0010">Tables 2 and 3</a> provide a summary of the characteristics of the studies included. The studies were conducted in 3 countries: United States (8), Canada (2), and Australia (1). The total sample included 744 older adults aged 55 years and older, of whom 325 participated in the control group. All were inactive geriatric patients, although two articles included women exclusively.<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">28,32</span></a> The studies included participants with type 2 diabetes,<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> knee osteoarthritis<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">33</span></a> and heart disease.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">32</span></a> Seven of the articles included data from two or more cohorts,<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17,18,27,28</span></a><span class="elsevierStyleSup">,</span><a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">31,33</span></a> one article included three intervention groups,<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> and two of the studies had no control group.<a class="elsevierStyleCrossRefs" href="#bib0125"><span class="elsevierStyleSup">25,32</span></a></p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0115" class="elsevierStylePara elsevierViewall">The interventions conducted aimed to increase physical activity and assess improvement in cardiovascular parameters using ICT tools. Fifty percent of the studies used the pedometer,<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,29–33</span></a> supported by other tools such as telephone assistance,<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,32</span></a> self-regulation interventions,<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">31</span></a> goal setting,<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,33</span></a> assessment,<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">33</span></a> accelerometer,<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">33</span></a> tablet,<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">29</span></a> or intervention with a virtual exercise trainer.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">29</span></a> Other tools were telephone consultations with<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> or without<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> technological assistance, tablet,<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> activity monitor,<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">27,28</span></a> and mHealth tool.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> The interventions were often accompanied by physical activity education, rehabilitation, workshops, activity prescription and social support, among others. The duration of the interventions ranged from,<a class="elsevierStyleCrossRefs" href="#bib0125"><span class="elsevierStyleSup">25,26</span></a> eight,<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">29</span></a> twelve,<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17,28,30,31</span></a><span class="elsevierStyleSup">,</span><a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">33</span></a> and twenty-four weeks.<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,27,32</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">Physical activity was measured in steps,<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17,26–33</span></a> minutes of activity,<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,25,27,28</span></a> days per week of physical activity,<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">32</span></a> and sedentary behaviour.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a> In general, the interventions were effective regardless of how physical activity was measured. Eight of the studies reported a statistically significant difference between the intervention and control groups. The remaining three studies<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,26,31</span></a> found no significant differences, although the participants in the intervention group found the ICT tool useful.</p><p id="par0125" class="elsevierStylePara elsevierViewall">The cardiovascular parameters on which the effectiveness of increased physical activity was studied were blood pressure,<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17,18,27</span></a> glycosylated haemoglobin,<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> blood glucose,<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> mean kilograms lost,<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> BMI,<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,32</span></a> and mean body fat.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">The studies showed statistically significant changes in blood pressure,<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17,18,27</span></a> cardiovascular training,<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> and blood glucose.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> However, the effects on glycosylated haemoglobin<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> were not statistically significant. However, the studies reported statistically significant changes in mean body fat<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> and mean kilograms.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> Regarding changes in BMI, one study showed statistically significant differences,<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> whereas another did not.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">32</span></a></p><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Evaluation of risk of bias</span><p id="par0135" class="elsevierStylePara elsevierViewall">The overall risk of bias of the RCTs showed some concerns in most of the studies included. By specific domain, in the domain derived from the randomisation process 75% of the studies were rated as low bias; in the domains of measurement of the variables of interest and missing data from the variables of interest, 89% of the studies were rated as low bias. Finally, in the domains of deviations from predetermined interventions and selection of reported outcomes, 75% of the studies were rated as “some concerns” (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0140" class="elsevierStylePara elsevierViewall">The overall risk of bias of non-RCTs and quasi-experimental studies showed moderate risk and serious risk of bias in all the studies included. By specific domain, in the domains of confounding, interventions and deviations from intended interventions, 100% of the studies were rated as low risk. In the domain of study participant selection and outcome measurement, 100% of the studies were rated as moderate risk. In the domain of missing data and selection of reported outcomes, 50% of the studies were classified as moderate risk (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>).</p><elsevierMultimedia ident="fig0015"></elsevierMultimedia></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Discussion</span><p id="par0145" class="elsevierStylePara elsevierViewall">To our knowledge this is the first review to summarise the effectiveness of ICT tools in increasing physical activity and the effect of this increase on cardiovascular parameters in older adults. The results demonstrate the effectiveness of ICT in increasing physical activity in older adults and the consequent effectiveness in improving cardiovascular parameters in this population.</p><p id="par0150" class="elsevierStylePara elsevierViewall">This review shows that the use of ICT is effective in increasing physical activity levels in older adults, and how this increase is reflected in improved cardiovascular parameters. Only two of the articles included in this review<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,31</span></a> showed no significant differences, although both groups increased physical activity levels and reported significant weight loss and waist circumference reduction. These results may be affected by socio-economic differences between groups,<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> limitations with sample size,<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">31</span></a> and evaluation of risk of bias of the articles included, which may influence the study results. Previous research shows that ICT tools are a viable and optimal option in the sedentary older adult population.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> These tools provide support for physical activity decision-making and can create individualised programmes tailored to each participant's active routine.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a> Avoiding sedentary lifestyles is an effective approach to prevent chronic and cardiovascular diseases.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a></p><p id="par0155" class="elsevierStylePara elsevierViewall">In general, older adults tend to be more inactive than the rest of the population, and a sedentary lifestyle has been shown to increase the risk of mortality, and not only associated with an increased risk of metabolic syndrome.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> Apart from the known benefits of physical activity, such as metabolic and cardiovascular benefits, physical activity in older adults has been shown to be effective in preventing the risk of falls, improving cognitive health and functional health levels.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">34</span></a> Interventions to improve physical activity have so far been delivered face-to-face, and this review confirms that the use of non-face-to-face interventions, such as ICT tools, is effective. Moreover, the use of ICT tools is common throughout the world, and therefore they may have the potential to improve health globally.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a></p><p id="par0160" class="elsevierStylePara elsevierViewall">There is a wide range of ICT tools that have been included in this study and that have proven effective in increasing physical activity in older adults. These include the pedometer, which objectively measures total daily activity and has been shown to correlate well with health parameters.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">34</span></a> Electronic activity monitors, such as the accelerometer, can monitor physical activity or provide additional information, such as sleep monitoring.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> Smartwatches are other electronic activity monitors, which, together with smartphones and apps, are designed for physical activity prescription and data collection, and can help develop personalised interventions with interactive messages and reminders.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">35,36</span></a> The use of these devices together is known as mHealth technology. They have been designed as more effective and individualised interventions for patients’ lifestyles and health.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> Embodied conversational agents can be effective in patients with low computer literacy.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">29</span></a></p><p id="par0165" class="elsevierStylePara elsevierViewall">Older adults' lack of connection to innovative technologies may be a barrier to harnessing the potential of these tools, as internet use is observed to decline after the age of 75.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">37</span></a> However, even among older adults there are generational differences, with younger older adults reporting greater optimism in the use of these tools. Studies have described better acceptance of ICT among recently retired older adults, as many may have been familiar with ICT in their jobs.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> Therefore, the number of older adults using these technologies has been growing in recent years, as younger older adults use these tools to solve their health problems and those of their parents, who are older adults.</p><p id="par0170" class="elsevierStylePara elsevierViewall">It has also been shown that older adults who have more health problems, and therefore more diagnoses, are more likely to use technology to search for information,<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> and are therefore more willing to learn how to use innovative technologies. ICT education has been shown to be an effective approach to increase computer literacy, reduce rejection, increase interest, and exploit the potential of the technology.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">38</span></a> In this regard, older adults with higher incomes and better education are able to use the internet more frequently, even outperforming the general population.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">37</span></a></p><p id="par0175" class="elsevierStylePara elsevierViewall">Due to advances in technology, this study sought information from a nursing perspective on the use of ICT and nursing taxonomies (NNN). The information found shows that innovative technologies are not being included in taxonomy interventions, which implies that they are not being adapted to nursing practice; nursing interventions using ICT tools for diabetics are the only interventions that contemplate the use of ICT. As with diabetic patients, and in view of the health benefits, interventions could be designed for older adults.</p><p id="par0180" class="elsevierStylePara elsevierViewall">Digital aids have also been developed for online taxonomy consultation. The development of ICT has facilitated communication between health staff and patients and between health professionals, and has helped create information points to resolve patients' queries.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">39</span></a> However, ICT tools are currently used in the health sector for communication and information purposes, not for care purposes. As a novelty, during the COVID-19 pandemic we have seen how nurses have used their electronic devices to help patients communicate with their families. We should consider including the use of ICT in the taxonomy to improve patients' mood and well-being. For older adults, ICT could be implemented to increase physical activity in healthy, homebound older adults, either with gym sessions or family sessions, tailored to individual needs.</p><p id="par0185" class="elsevierStylePara elsevierViewall">The limitations of this study are those common to systematic reviews. We should also highlight the following aspects that could limit the robustness of the results: (a)there may be publication bias due to the tendency to publish articles with positive results more frequently; (b) there may be selection bias from having consulted a limited number of databases and having conducted the search only in Spanish and English; c) there is a possible study selection bias due to the inclusion and exclusion criteria, and relevant information included in studies that did not meet these criteria may have been lost; d) it is possible that the inclusion of pilot studies may limit the conclusions by involving smaller sample sizes and, therefore, imply lower power; quasi-experimental studies were also included, with no control group, therefore lacking a comparison group for the effect of the intervention; e) we should consider that the elimination of articles rated with a higher risk of bias<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">31,32</span></a> could make the results of this review more homogeneous, indicating a greater effect of ICT in increasing physical activity and improving cardiovascular parameters, as one<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">31</span></a> did not find significant differences and the other<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">32</span></a> lacked a control group; f) finally, we must consider the heterogeneity of age, interventions, socio-economic status, sample size, country of origin and year of publication of the studies.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Conclusions</span><p id="par0190" class="elsevierStylePara elsevierViewall">To conclude, this review supports previous evidence that ICT tools are effective in increasing physical activity in older adults. Furthermore, this study shows that these interventions could improve the cardiovascular parameters of patients who use ICT tools to increase their level of physical activity. In recent years, there has been an increase in the use of technology, with health technology tools (ICT) appearing as an intervention to increase physical activity and improve associated health problems.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Objective</span><p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">In this review, we evaluated the effectiveness of health technology to increase physical activity and to improve cardiovascular parameters in older adults.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Methodology</span><p id="spar0050" class="elsevierStyleSimplePara elsevierViewall">Studies with a great variety of health technology tools to increase physical activity levels, and that evaluated the effect of that increase on cardiovascular parameters were included by searching the main databases.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Results</span><p id="spar0055" class="elsevierStyleSimplePara elsevierViewall">Eleven studies reporting the use of a variety of ICT tools were included in this review.
