It is frequently associated with inadequate breastfeeding technique<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">1–3</span></a> and has a higher incidence in the first week of postpartum, especially on the second and third days.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">3–5</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">Nipple trauma affects more than half of breastfeeding women<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">5–7</span></a> and can be defined as any modification of the normal anatomy of the nipple tissue such as erythema, edema, scaling, fissure, blister, scabbing,<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">8</span></a> abrasion, crack and bruise.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">3,9</span></a></p><p id="par0030" class="elsevierStylePara elsevierViewall">Health education on breastfeeding, focusing on the correct technique, aims to prevent breast complications and maintain exclusive breastfeeding.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">10,11</span></a> Despite its recognized importance there is a lack of consistent clinical practice guidelines to address this education.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">10</span></a> It is recommended that health education starts in prenatal care and extends to postpartum<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">7,12</span></a> combining different strategies, for example, clinical demonstration and the use of illustrated albums.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">11</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">In addition to health education, experimental studies have investigated products and strategies to prevent nipple trauma,<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">13</span></a> such as: olive oil, hydrogel, peppermint water, peppermint gel, <span class="elsevierStyleItalic">Guaiazulene</span>-based ointment, hot wet compress and tea compress. Despite positive results in the prevention of nipple trauma, the clinical use of these products requires more careful examination, especially if they need to be removed prior to breastfeeding or may add to the risk of nipple maceration due to excess moisture.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">13</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">The use of lanolin for nipple treatment is widespread in clinical practice and is supported by breastfeeding consultants and <span class="elsevierStyleItalic">La Leche League International</span> and <span class="elsevierStyleItalic">International Board Certified Lactation Consultants</span> (IBCLC).<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">14</span></a> Similarly, scientific studies of lanolin use demonstrate favorable results for the treatment of nipple pain and trauma.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">5,14</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">Lanolin properties help to control skin moisture, favor cell proliferation and consequently wound healing. It is not necessary to remove it before breastfeeding, which reduces friction of the nipple region and the risk of injury.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">4</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">Despite the advances and consolidated use for treatment, a gap remains in clinical studies investigating the efficacy of lanolin from a prenatal prevention perspective. A study for the prevention of nipple pain using lanolin was identified,<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">15</span></a> however, it recruited participants in the postpartum period, with no prolonged use of the ointment, or preparation for breastfeeding in prenatal care, factors that may have contributed to a negative outcome. The research question for this study was: Is the use of anhydrous lanolin from the prenatal period with continuity in the postpartum period effective in preventing trauma and nipple pain during breastfeeding?</p><p id="par0055" class="elsevierStylePara elsevierViewall">Thus, this study aimed to investigate the effect of lanolin on nipple pain and trauma in breastfeeding after application of a health education initiated in prenatal care.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Method</span><p id="par0060" class="elsevierStylePara elsevierViewall">This randomized controlled clinical trial, with two arms, open, included 66 women receiving national public health system prenatal care at Family Health Centers in a brazilian metropolitan city between May 2018 and February 2019.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Inclusion criteria were: gestational age ≥35 weeks, verbal expression of desire to breastfeed, low-risk pregnancy, live in the city of the study, have a contact telephone number and a minimum age of 18 years. Exclusion criteria were: women who had breastfeeding abnormalities or unfavorable nipple characteristics (flat, inverted nipples) and twin pregnancy. Withdrawal criteria were: obstetric or neonatal complications that could interfere with breastfeeding, discontinuity of the intervention or impossibility of contact after three attempts at different times.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The sample size was calculated considering the estimation of nipple trauma proportion in both groups. We consider an estimate that 47% of women in a group with health education would have no nipple trauma.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">10</span></a> With statistical power of 90% and significance level of 95% (<span class="elsevierStyleItalic">α</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05), the calculation suggested the recruitment of 66 eligible participants, which was corrected for 82 participants (41 in each group), considering an attrition rate of 20%. Open Epi Version 3.01 software was used for this calculation.</p><p id="par0075" class="elsevierStylePara elsevierViewall">The study<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">10</span></a> used for the sample calculation was chosen because it is the only one found in the literature whose sample is similar to that of the present study, having applied the health education intervention in prenatal care. There are no similar studies on the use of lanolin to prevent nipple pain and trauma in prenatal care.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Participants were randomized (1:1) using computer generated numbers and equal allocation of 41 participants in both the experimental group (EG) and control group (CG).</p><p id="par0085" class="elsevierStylePara elsevierViewall">Health education sessions provided all participants with orientation to and guidance about breastfeeding using an illustrated serial album and clinical demonstration with a breast model. Both groups received consistent education in trauma prevention using the same approach. The first session took place during prenatal care, individually in private consultation room and in person, and the second by telephone.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The album approaches breast physiology, anatomy and skin conditions; mechanisms of breast milk production; proper breastfeeding technique; prevention and treatment of breast engorgement and nipple trauma; and advised against the use of soaps, oils, lotions and creams on the breasts.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">16</span></a> In addition, it has images of different types of nipple trauma so the women could recognize any lesions that may appear.</p><p id="par0095" class="elsevierStylePara elsevierViewall">In addition to the general educational session, EG participants received anhydrous lanolin ointment (10<span class="elsevierStyleHsp" style=""></span>g) for immediate use twice a day for six weeks. The recommendations for use were carefully explained, including to apply the product on the nipples after bathing, and during postpartum after each breastfeeding, without removal for the next breastfeed. Telephone follow-up was systematically conducted to ensure knowledge of the use of lanolin for EG participants and to emphasize important educational topics for all women of both groups seven days after the first contact.</p><p id="par0100" class="elsevierStylePara elsevierViewall">At the eighth postpartum day, a home visit was made to the women to evaluate the breastfeeding process,<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">17</span></a> to verify the occurrence of pain using the Numerical Pain Scale and nipple trauma through clinical examination. This evaluation was carried out by a team of trained researchers different from the one that applied the intervention to the experimental group.</p><p id="par0105" class="elsevierStylePara elsevierViewall">The follow-up period of the participants was defined considering the intention to prevent nipple trauma, it is emphasized that this event has a higher incidence in the first postpartum week.<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">3–5</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">The breastfeeding parameters were classified as favorable or unfavorable according to the instrument used by Vieira,<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">18</span></a> which contains 25 items that determine: positioning of woman and infant, condition of the breast, latch, sucking, maternal-infant bond and general aspects.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Each parameter has behaviors/signs favorable to breastfeeding, and behaviors/signs of possible difficulties. During the assessment, the identified behaviors are highlighted. The score is obtained from the number of observed unfavorable behaviors noted in each item.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Thus, in the parameters observation of the general aspects of the woman and the newborn, position of the woman and infant, condition of the breast and maternal-infant bond, it is necessary that all favorable behaviors are observed to classify the situation favorable to breastfeeding. And, in the parameters latch and sucking by infant, when two or more behaviors indicating difficulty are identified, it is evaluated as an unfavorable situation to breastfeeding.</p><p id="par0125" class="elsevierStylePara elsevierViewall">This instrument was adapted from the form proposed by UNICEF<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">19</span></a> available in the authors’ language and was refined by five specialists and researchers in the obstetric area, in addition to being applied in a previous randomized clinical trial.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">18</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">In addition to the intervention carried out by the research team, women received the usual guidelines from the health and maternity unit. The Ministry of Health of Brazil recommends that guidelines on breastfeeding in prenatal and postpartum be carried out according to available guideline,<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">16</span></a> however, there is no standardization of the guidelines offered regarding the information and specific moment of the educational approach.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Data collection was performed by a nurse, who is the main researcher, and by two nursing students. Research assistants were duly trained in data collection and accompanied the main researcher in three days of data collection.</p><p id="par0140" class="elsevierStylePara elsevierViewall">The data were collected in printed forms and later transferred to the database. The research used descriptive and inferential statistical analyses, presenting simple frequency, percentage, measures of central tendency and dispersion of quantitative data. The Kolmogorov–Smirnov test was used to verify normality. Qualitative variables were tested using the chi-square test or Fisher's exact test to verify the association between sociodemographic, obstetric, neonatal characteristics and those related to nipple pain and trauma between the experimental group (EG) and the group (GC). Quantitative variables were tested by Student's <span class="elsevierStyleItalic">t</span> test. The value of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05 was considered significant. The effectiveness of the intervention on the prevention of breastfeeding nipple trauma and pain was calculated using absolute risk reduction (ARR: difference between percentage of nipple trauma and pain in the EG and in the GC) with confidence interval (95% CI). The database was prepared using IBM SPSS® version 24.0 to perform statistical tests.</p><p id="par0145" class="elsevierStylePara elsevierViewall">The study was approved by the Committee of the Clinics Hospital of the Federal University of Goias, Brazil (Number 2,781,365). Written informed consent was obtained from all participants and followed the current recommendations on research ethics.</p><p id="par0150" class="elsevierStylePara elsevierViewall">The present study is part of the registered clinical trial. It was not possible for all other interventions to be developed at the same time, so we opted for the two most necessary for clinical practice: lanolin and health education. However, we affirm that there were no implications regarding the robustness of the study, as well as the originality and innovation was maintained. The other interventions will be performed later in randomized clinical trials in order to maintain the level of evidence and provide relevant results for clinical practice.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0155" class="elsevierStylePara elsevierViewall">The recruitment of 82 participants reached, 41 in both groups. Among the study participants, 16 were withdrawn, eight in each group. In the EG, four women were withdrawn due to the impossibility of contact, and four, due to the discontinuity of the intervention/non-use of lanolin. In the CG, five women were withdrawn due to the impossibility of contact, and three for not being breastfeeding in the postpartum period. Thus, the sample size was 66 participants, as the projected sample size for analysis (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0160" class="elsevierStylePara elsevierViewall">The groups were classified as homogeneous regarding sociodemographic characteristics (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>) and results are presented comparing EG and CG.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0165" class="elsevierStylePara elsevierViewall">Regarding the number of prenatal consultations, women in the EG had, on average, higher numbers compared to the CG, 9.4 and 8.1 consultations, respectively, showing a significant difference between the groups (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.03). However, the number of consultations was not significantly associated with nipple trauma (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.88). Similarly, the EG had significantly more participants with a history of nipple trauma or pain in the previous breastfeeding experience EG <span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>16 (88.9%) versus CG <span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>8 (40.0%); <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002), yet this had no impact on the incidence of pain or trauma during the analyzed period (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.38) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0170" class="elsevierStylePara elsevierViewall">In the hospital, some participants reported having received breastfeeding guidance from health professionals, including correct positioning and latch, stages of breast milk and breastfeeding on demand. There was no statistical difference between groups, EG<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>63.6% and CG<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>78.8% (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.17), with regard to this random health education offered in addition to the study procedures.</p><p id="par0175" class="elsevierStylePara elsevierViewall">Among the participants, 11 (48.5%) had nipple trauma in the EG and 16 (33.3%) in the CG, starting on the second postpartum day, on average, for both groups. In 76.9% of cases, trauma occurred in both nipples (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0180" class="elsevierStylePara elsevierViewall">The estimated value of the attributable risk (ARR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>15.2%) and the confidence interval [95% CI: 15,151 (−8288; 38,592)] does not allow us to conclude that the treatment result is statistically superior to the result obtained in the control group.</p><p id="par0185" class="elsevierStylePara elsevierViewall">When asked during postpartum, 60.6% of women reported some degree of nipple pain, and among them, 77.4% reported pain in both nipples with predominant onset on the second day. Pain intensity, calculated according to the mean scores varying from 1 to 10, resulted in pain of low intensity (3.5) in the experimental group and moderate in the control group (4.5) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.10) (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0190" class="elsevierStylePara elsevierViewall">Similarly, the estimated value of the attributable risk (ARR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>6.1%) and the confidence interval [95% CI: 6061 (−17,470; 29,592)] shows that there are no statistically significant differences in preventing nipple pain.</p><p id="par0195" class="elsevierStylePara elsevierViewall">In breastfeeding evaluation, the appropriate technique prevailed in both groups, in all parameters observed, except for women's breast condition (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia><p id="par0200" class="elsevierStylePara elsevierViewall">Most women followed the recommendations offered during the health education sessions, homogeneously among the groups. The pattern of breastfeeding was mostly exclusively (78.8%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.76) and on demand (95.5%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.23). Breast prevention behaviors were positive regarding breast hygiene with water alone (97.0%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>0.99) and use of the little finger to interrupt breastfeeding (56.1%; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.80). It was noted that 56.1% affirmed the use of pacifier/bottle (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.45), and half of the women reported performing massage and hand expression of breastmilk between feedings (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.80).</p><p id="par0205" class="elsevierStylePara elsevierViewall">One participant reported itching in the nipple after the first application of lanolin and because of this the use of the ointment was stopped, with no major complications after care, the intensity of the itching was not reported and it was withdrawn from the sample.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0210" class="elsevierStylePara elsevierViewall">The effectiveness of the use of anhydrous lanolin associated with health education during prenatal care could not be proven through inferential tests, thus, it does not allow us to state that anhydrous lanolin combined with health education helps in preventing pain and nipple trauma.</p><p id="par0215" class="elsevierStylePara elsevierViewall">This study is innovative for testing the use of lanolin particularly during prenatal care. Few researchers<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">20,21</span></a> have performed investigation on lanolin for the prevention of nipple trauma.</p><p id="par0220" class="elsevierStylePara elsevierViewall">Studies using lanolin did not find significant differences in prevention when comparing it with expressed breast milk<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">21</span></a> and peppermint gel.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">20</span></a> However, methodological limitations in the first led to a sample size ineligible for parametric test analysis and in the second, recommended removal of lanolin prior to breastfeeding, increasing the risk of friction and nipple damage.</p><p id="par0225" class="elsevierStylePara elsevierViewall">The association of health education in breastfeeding with other interventions aimed at preventing breast complications can be beneficial and should be explored in clinical practice.</p><p id="par0230" class="elsevierStylePara elsevierViewall">Health education on breastfeeding is advocated by national and international entities with proven benefits for breastfeeding,<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">16,22</span></a> and this strategy should cover many aspects, especially positioning and latch techniques. In addition, there is an intense exchange of information between breastfeeding women and their support network (family and friends), as well as spontaneous searches for information on the internet, but it's important always consulting with a health professional.</p><p id="par0235" class="elsevierStylePara elsevierViewall">One of the main causes of nipple trauma is inadequate breastfeeding technique, including positioning and latch.<a class="elsevierStyleCrossRefs" href="#bib0185"><span class="elsevierStyleSup">11,23</span></a> As nipple trauma has a strong impact on women's decision to continue breastfeeding or discontinuing early, it is paramount that health care professionals work to prevent the onset of nipple trauma, thereby contributing to the protection of breastfeeding.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a></p><p id="par0240" class="elsevierStylePara elsevierViewall">By identifying and supporting parents in favorable breastfeeding parameters, health care professionals can help to prevent breast complications from the first breastfeeding moment and encourage a positive experience for the mother and infant.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">23</span></a> Moreover, research in this area is valuable to increase the effectiveness of prevention and to find options for treatment to minimize breastfeeding pain and nipple trauma.</p><p id="par0245" class="elsevierStylePara elsevierViewall">The educational intervention performed in this study may have contributed positively to the adequate breastfeeding technique in both groups, considering that the frequency of breastfeeding women with nipple trauma or pain was lower than expected. In scientific literature, there is a prevalence of nipple trauma higher than 55%,<a class="elsevierStyleCrossRefs" href="#bib0145"><span class="elsevierStyleSup">3,7,11</span></a> and 80% regarding nipple pain.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">24</span></a> Thus, it is possible that women who had initial difficulties in breastfeeding technique could correct them before the evaluation on the eighth day after birth. Therefore, the importance of breastfeeding support and correction of the technique is emphasized as a routine established in the maternity care and postpartum.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">12</span></a></p><p id="par0250" class="elsevierStylePara elsevierViewall">It is important to highlight that recommendations on prevention and management of breast complications goes beyond just proper breastfeeding technique.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">16,25</span></a> Most participants followed the recommendations made in this study, which may have contributed to reducing the prevalence of trauma and nipple pain. The use of soap to clean the breasts is contraindicated and may impair the integrity and resistance of the skin.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">4,16</span></a> At the end of breastfeeding, it is recommended to interrupt the feed by placing the finger on the child's lip to avoid abrupt cessation that could damage the nipple.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">16</span></a> The use of pacifier and bottle is also advised against, as these are considered predictors of nipple trauma, negatively interfering with breastfeeding,<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">1</span></a> however, in this study, there was no significant relationship between these variables.</p><p id="par0255" class="elsevierStylePara elsevierViewall">Despite the high adherence to the above-mentioned preventive measures, nipple trauma occurred in 40.9% of women. Another study that found a low prevalence compared to general literature (32%) considered only one type of nipple trauma, cracks, with identification self-reported by telephone by the women themselves.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">1</span></a> In this study, all types of lesions were included from erythema (initial stage, possibly unreported by other researchers in the field) to cracks. In addition, nipple assessment was verified by qualified nurses and who performed in-person clinical evaluations of occurrence and classification of the trauma.</p><p id="par0260" class="elsevierStylePara elsevierViewall">We believe that the method of reporting lesion by participants may not offer a safe assessment because the woman does not have a good view of her own nipple, and has not been trained to recognize nipple trauma.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">9</span></a> In this context, we question the comparability of the data between that research study<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">1</span></a> and this one.</p><p id="par0265" class="elsevierStylePara elsevierViewall">Skin quality, as a protective factor against nipple trauma, should be considered in preventive actions during prenatal care. Physiologically, areola undergoes changes during pregnancy preparing breasts for breastfeeding<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">16</span></a> and postpartum hydration guidelines with milk,<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">4</span></a> other substances,<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">22,26</span></a> or even non-exposure to dryness<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">4,16</span></a> have been reported in the literature as possibilities to mitigate the damage of nipple injuries.</p><p id="par0270" class="elsevierStylePara elsevierViewall">This study brought contributions to the area by signaling the positive effect of anhydrous lanolin as a protective factor for nipples to trauma and nipple pain, although no statistical difference was found, and reinforced the importance of quality guidelines in the preparation of pregnant women for breastfeeding. We suggest as future studies the analysis of the degree of trauma, comparing the effects of anhydrous lanolin not only in the presence or absence of nipple trauma, but also the extent and classification of skin injuries.</p><p id="par0275" class="elsevierStylePara elsevierViewall">Limitations of this research include a lack of double blindness. Also, the researchers found it difficult to mimic lanolin in the placebo group (considering its characteristics – color, texture and odor). Another limitation was the reliance on participant report regarding the daily application of lanolin by the experimental group, confirmed verbally throughout the study.</p><p id="par0280" class="elsevierStylePara elsevierViewall">There is also a variation in the time of lanolin use according to the woman's gestational age when participating in the study, however, it is observed that the research team established as an inclusion criterion gestational age equal to or greater than 35 weeks in order to reduce the discrepancies in the time of use of the intervention.</p><p id="par0285" class="elsevierStylePara elsevierViewall">The sample size was not sufficient to find statistical differences between the groups analyzed. In conclusion, it is not clear whether the intervention, lanolin combined with health education, has a positive effect on the prevention of nipple pain and trauma. We suggest that health education must be carried out systematically in prenatal care and postpartum as well as follow-up of breastfeeding assessment after discharge. Although there is no compelling scientific evidence at this time of the positive association of lanolin use or its prophylactic efficacy, further research is warranted.</p><p id="par0290" class="elsevierStylePara elsevierViewall">Preventing and managing nipple trauma is still a major challenge. Participants were randomized (1:1) using computer generated numbers in both experimental group (EG) and control group (CG). The EG received lanolin and health education on breastfeeding at two different times with clinical demonstration using cloth didactic breast and illustrative album as the intervention, while the CG received standard health education. Health education was carried out by the same researchers in both groups. Measurement of pain, nipple trauma, and breastfeeding technique occurred on postpartum day eight. The analysis included descriptive statistics and inferential analysis by means chi-square or Fisher test, and Student's <span class="elsevierStyleItalic">t</span>-test, significance level set at 0.05.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">A majority of the participants experienced no nipple trauma (59.1%) in both groups, and 60.6% of women experienced pain. In both groups, women showed favorable breastfeeding behaviors, except in the condition of the breasts.
