class="elsevierStylePara elsevierViewall">The main nursing tasks in primary care include the prevention and early detection of health problems such as obesity, which is an abnormal or excessive accumulation of fatty tissue that increases the risk for various diseases such as diabetes, hypertension, cardiovascular disease and psychological disorders such as depression.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> While this problem affects all population groups, young university students are particularly vulnerable as the transition from secondary school to higher education presents challenges that can lead to changes in dietary habits and lifestyles such as prolonged fasting, eating few meals a day, low fruit and vegetable intake, a preference for fast food, sedentary lifestyles and alcohol consumption.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">In Sudan, a prevalence of 15% overweight and 1% obesity was found in university students, with a predominance in women<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>; in China, 9.5% of university students are overweight and 2.2% obese<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>; in the United States, 33.7% of university students self-report being overweight and 11.8% obese.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> In Mexico, the situation is more alarming: a study of university students in the west of the country showed a prevalence of 27.8% overweight and 14.7% obesity in men, while in women the prevalence was 17% overweight and 5% obesity.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> In the northeast of the country, the prevalence of overweight was 31.2% (45.8% in men and 22.4% in women), and obesity 15.1%.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> In Veracruz, the prevalence of overweight and obesity was found to be 21.3% and 8.3% respectively, with a predominance in men.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">Body mass index (BMI [kg/m<span class="elsevierStyleSup">2</span>]) is classically used to determine obesity. Overweight is considered a BMI greater than or equal to 25 kg/m<span class="elsevierStyleSup">2</span> and less than 30 kg/m<span class="elsevierStyleSup">2</span>; and obesity a BMI greater than or equal to 30 kg/m<span class="elsevierStyleSup">2</span>.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> However, there is controversy about the ability of BMI to differentiate between muscle mass and fat tissue,<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,9</span></a> since two individuals could have the same BMI score and have a different body distribution, one with greater muscle mass and the other with greater fat mass.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> Many indices have been developed, therefore, to best determine obesity in a practical way in primary care, including waist circumference, the A Body Shape Index (ABSI), the waist-to-height ratio, the Body Roundness Index (BRI), and the Conicity Index. Any of these indices can be used by nurses by taking body measurements and applying the corresponding formula. However, nurses continue to use the BMI in the main.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Waist circumference is used as an indicator of central obesity because variations in its measurement depend on the accumulation of fat in the abdominal area. This indicator has been linked to the risk of various diseases such as diabetes, hypertension, and metabolic syndrome.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> Its interpretation will also depend on the group to be assessed as it can be influenced by gender and ethnic characteristics.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">Krakauer and Krakauer propose the ABSI index based on waist circumference adjusted for weight and height.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> The main objective of this index is to better profile individuals at risk as it includes waist circumference which indicates abdominal obesity, and therefore related to a greater amount of central adipose tissue.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> An individual with an elevated ABSI value is at increased risk for diabetes mellitus, hypertension, and cardiovascular disease.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">The waist-to-height ratio has also been used as a tool to identify problems related to excess weight. It is obtained by dividing waist circumference by height. Its value provides an estimation of abdominal obesity; high results indicate the presence of greater accumulation of centrally distributed body fat.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> This index has been used as a screening tool for chronic non-communicable diseases such as diabetes, hypertension, and metabolic syndrome in adults.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">Thomas et al. propose the BRI geometric index that includes height and waist circumference to predict body fat percentage and assess health status.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> This index is based on the premise that a higher score, 16 being the maximum, corresponds to a body shape with a tendency to roundness and therefore a higher percentage of body fat and health implications from a nutritional perspective. There is evidence for using the BRI as a tool for detecting insulin resistance.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">The Conicity Index is another index based on abdominal silhouette and its relationship with abdominal fat accumulation on the hypothesis that abdominal fat accumulation gives people a silhouette like a double cone silhouette and those with low accumulation will have a cylinder shape.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> This index has been used to determine cardiovascular risk in adults and to evaluate body fat in young people.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">As can be seen, this wide range of indices is accessible to nurses because training in their use is simple compared to the complexity of standardising techniques that require very specific training, such as skinfold thickness measurement.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> They also estimate the percentage of body fat without using specialised equipment such as computed tomography, which involves a high level of radiation and high cost, or dual X-ray absorptiometry that allows visualisation of body fat distribution by means of images, but which, like the former, requires specialised instruments.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">Because body fat and its distribution are a risk factor for various metabolic diseases, its measurement is an important part of nursing, however, imaging methods are expensive. Therefore, anthropometric indices that correlate significantly with body fat percentage are alternative measurement tools providing simple, fast, and less expensive methods for calculating an individual’s body fat that can be used in primary care.</p><p id="par0080" class="elsevierStylePara elsevierViewall">However, to our knowledge, there is little information on an appropriate index for this purpose in the university student population. Despite the evidence on the anthropometric indices and their correlation with body fat, studies present results in the adult population in the main. Therefore, further research is needed in young people, as this is a critical stage in habit formation.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Furthermore, because obesity is a multifactorial problem, there are other approaches to understanding weight gain in relation to dietary and physical activity habits. Self-regulation is one such approach, as the evidence indicates that interventions to improve this mental process have an impact on anthropometric measurements in the long term.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> Self-regulation is a process that occurs either deliberately or subconsciously that provides the information to set goals and make a diagnosis based on comparison and plays a specific role in self-motivating individuals to establish mechanisms to reach the goals set.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">Considering that self-regulation relates to forming habits of healthy eating and physical activity, it could also function as a good predictor of the anthropometric index that best determines the presence of obesity, considering that young people with obesity will have poorer self-regulation than those of normal body weight. However, the relationship between obesity and self-regulation has only been studied using BMI and due to the abovementioned limitations of this measure it is important to analyse the relationship from the perspective of other anthropometric indices to determine how useful they are in university students.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Therefore, the aim of this study was to determine the anthropometric indicator with the strongest association with body fat percentage and self-regulation of eating and physical activity in young university students, which can be used by nurses for the timely detection of excess weight problems.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Method</span><p id="par0100" class="elsevierStylePara elsevierViewall">We performed a quantitative, descriptive, cross-sectional study.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Participants</span><p id="par0105" class="elsevierStylePara elsevierViewall">A total of 3869 students were included, of whom 53.9% were female and 46.1% male, with a minimum age of 18 years, a maximum age of 48 years, and a mean of 19.2 years (SD<span class="elsevierStyleInf">age</span> = 2.3), from the Universidad Veracruzana, Veracruz Region, from the academic years 2017 and 2018. Pregnant students were excluded.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Instruments</span><p id="par0110" class="elsevierStylePara elsevierViewall">The self-regulation of eating habits scale consists of 14 Likert-type items with scores ranging from 14 to 70, the higher the score the better the self-regulation. It asks how much attention young people pay to their diet, whether they compare it with what they consider to be appropriate eating, and whether they create any strategies to eat more healthily. It was developed among Mexican university students and has shown adequate validity and reliability.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">The self-regulation of physical activity scale developed in Mexican university students consists of 12 Likert-type items with a score ranging from 12 to 60, the higher the score, the better the self-regulation. It seeks to determine whether young people are attentive to their physical activity, evaluate their performance and are motivated to continue.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">Height was measured with the students barefoot and bare headed. Body weight was taken with as little clothing as possible, without shoes and with an empty urinary bladder, using an OMRON scale with a body composition monitor, which was also used to obtain the percentage of body fat. Waist circumference was measured with the torso uncovered, locating the midpoint of the costal margin and the iliac crest, checking that the abdomen was relaxed. The formulas proposed by their developers were used to calculate the anthropometric indices (BMI, ABSI, waist-height, BRI, Conicity Index).<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Procedure</span><p id="par0125" class="elsevierStylePara elsevierViewall">Prior to participating in the study, the students were informed of the objectives and the processes within the study. Those who agreed to participate signed the informed consent form and completed the scales, after which they took part in the anthropometric test.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Statistical analysis</span><p id="par0130" class="elsevierStylePara elsevierViewall">SPSS Inc. V 24 software was used for data analysis. Since the quantitative variables followed a normal distribution, parametric methods such as means, standard deviation and Pearson correlations were used. Qualitative variables were presented as percentages.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Results</span><p id="par0135" class="elsevierStylePara elsevierViewall">The group of men had higher means in the anthropometric indicators and indices compared to the women, except for fat percentage and waist-to-height ratio, as well as higher scores in self-regulation of eating habits and self-regulation of physical activity (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0140" class="elsevierStylePara elsevierViewall">Differences were observed in the prevalence of overweight and obesity by sex according to BMI; the men had a combined prevalence of overweight and obesity of 43.2%, while in the women it was 36.1% (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0145" class="elsevierStylePara elsevierViewall">Inverse correlations were found between most of the anthropometric indices and self-regulation of eating habits and self-regulation physical activity, however these were weak even with body fat percentage (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0150" class="elsevierStylePara elsevierViewall">In the men’s group, body fat percentage correlated weakly with ABSI and strongly with the rest of the anthropometric indicators, especially with BMI and waist circumference (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0155" class="elsevierStylePara elsevierViewall">In the women’s group, fat percentage had no correlation with ABSI, medium correlation with the Conicity Index and high correlation with the rest of the anthropometric indicators, the highest correlation being found with BMI (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0160" class="elsevierStylePara elsevierViewall">Body fat and its distribution contribute greatly to the development of chronic non-communicable diseases. Therefore, we need to determine the anthropometric indices that can estimate body distribution, and that are also applicable in health units where medical technologies are not highly sophisticated.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> Nurses routinely use anthropometric indices in primary care to check for obesity, therefore we need evidence to support those that are the most reliable in each population group.</p><p id="par0165" class="elsevierStylePara elsevierViewall">In the present study, we found BMI to be higher in men than women, contrary to the findings of a previous study in Colombian students where the means were similar between the sexes.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> Similarly, contrary to national reports, the prevalence of overweight and obesity was found to be higher in men; other studies in university students have found more overweight and obesity in men.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> However, it is undeniable that these young people, both men and women, have a high prevalence of overweight and obesity that could worsen over time, which is why precise measures are needed to monitor and address this problem.</p><p id="par0170" class="elsevierStylePara elsevierViewall">The correlations identified between body fat percentage and self-regulation were stronger in the men’s group, especially in physical activity; however, all these correlations can at best be considered weak, showing that university students have few self-regulatory qualities for them to associate it with their nutritional status; it is possible that many of their eating habits are still dependent on their family situation. As highlighted earlier, low levels of self-regulation are related to the individual’s inability to use mechanisms of censure or self-motivation effectively, which could in the future translate into eating habits that favour weight gain.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a></p><p id="par0175" class="elsevierStylePara elsevierViewall">On the other hand, although significant correlations were found between body fat and all the anthropometric indices in both men and women, except for ABSI, BMI remains the index with the strongest correlation in this group. Because BMI is widely known, easy to calculate and requires little specialised equipment, it is important to maintain its use in this population.</p><p id="par0180" class="elsevierStylePara elsevierViewall">Our findings are consistent with those of Ehrampoush et al. in an Iranian population with a mean age of 33 years, although they suggest using waist circumference and waist-to-height ratio.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> For our research in young people, because BMI is the best known and most widely used index for evaluating nutritional status, and the variables necessary to calculate it are financially and technically accessible, we recommend that nurses in primary care continue to use it in this population group, even though other indices with better results have been observed in adult groups.<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,9</span></a></p><p id="par0185" class="elsevierStylePara elsevierViewall">Longitudinal studies are needed to monitor changes in nutritional status, physical activity, and self-regulation in the group under study to identify variations so that specific action plans can be developed. On the other hand, the levels of self-regulation in men indicate a clear mental process that favours physical activity.
