It causes readmissions in 47% of patients and increases their risk of mortality.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Currently, the detection of PICS is complex due to the wide variety of components affected,<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> requiring the use of multiple instruments that assess them independently.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> Therefore, it is necessary to know these instruments and their applicability, since the early identification of this event is the first step to treating it and reducing its impact on patients. Thus, this review aims to determine the most widely used assessment instruments for detecting PICS, according to the scientific evidence of the last five years.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methodology</span><p id="par0015" class="elsevierStylePara elsevierViewall">The present scoping review sought to organise the evidence on the subject of the study, answering the question: What are the instruments present in the scientific evidence for detecting PICS in adults discharged from intensive care? The following databases were consulted: Academic Search, ScienceDirect, Scielo, Biblioteca Virtual en Salud (BVS), Medline and Springer Link. The MeSH term “post intensive care syndrome”, the acronym “PICS”, the Spanish term “síndrome poscuidado intensivo” and the Portuguese term “síndrome pós cuidados intensivos” were used. Boolean operators AND and OR were used.</p><p id="par0020" class="elsevierStylePara elsevierViewall">After this process, filters were applied according to the following criteria: research, review and opinion articles were included, with access to full text that referred to the subject matter, published in Spanish, English or Portuguese between 2015 and 2020 to obtain the most current information; studies in the paediatric population and those that did not describe any form of PICS measurement were excluded.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The selection and screening of articles were made according to the Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) methodology through three phases. In the first phase, titles and abstracts were reviewed, separating studies into the categories “definitely excluded”, “definitely included”, and “more information is needed to decide”. In the second phase, two investigators independently assessed the abstracts. They labelled them according to phase one, applying the selection criteria, and then met and jointly agreed on the final inclusion or exclusion of each study. In cases of disagreement, the third investigator was consulted, and the research group did a new revision. Finally, the texts were analysed in full in the third phase, and the partial or total contributions were identified in an Excel database to answer the review question.</p><p id="par0030" class="elsevierStylePara elsevierViewall">No quality assessment of the articles was carried out as recommended in scoping reviews to provide a comprehensive description of all the instruments found. <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> summarises the process of identification, screening, suitability assessment and selection of articles to be included.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0035" class="elsevierStylePara elsevierViewall">The included articles were characterised by the majority being published in 2018 and 2019 (27.2% for each year), in English (95.4%), and only one article in Spanish (4.6%). The articles were published originating mainly in the United States (27.2%) and the Netherlands (18.1%), and with an observational design (59%). The study population ranged from 68 to 4943 participants, with a total of 12166 and a mean of 640 patients (see <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0040" class="elsevierStylePara elsevierViewall">It was found that 10 articles (52.6%) presented instruments that assessed the physical component, 15 (78.9%) the mental component and 5 (26.3%) the cognitive component, as follows:</p><p id="par0045" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Physical component</span>: the walking test, which assesses the respiratory, cardiovascular, metabolic, musculoskeletal and neurosensory systems during exercise, was used<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2–4</span></a>; Medical Research Council (MRC) scale<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3–8</span></a> which detects mobility impairment; respiratory muscle function with measurement of peak inspiratory and expiratory pressure and spirometry<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a>; Katz index (KI) for dependence in Activities of Daily Living (ADL): bathing, dressing, toilet use, transferring, continence and feeding,<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,4,9,10</span></a> Barthel index and Lawton's Instrumental Activities of Daily Living (IADL)<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> (see <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0050" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Mental component:</span> the Hospital Anxiety and Depression Scale (HADS)<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,10–17</span></a>; the Beck Anxiety Inventory (BAI); the Depression Inventory scale second edition (BDI-II)<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a>; the Post Traumatic Stress Syndrome scale with 14 items (PTSS 14)<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,12,14</span></a> were frequently used; the Event Impact Scale (EIE) which assesses intrusive thoughts and avoidance; the Event Impact Scale-Revised (EIE-R) which measures hyperalertness<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3,11,13</span></a><span class="elsevierStyleSup">,</span><a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15,17–19</span></a>; and the Post-Traumatic Stress Disorder Checklist<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> - Civilian (PCL-C) instrument.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> In this component, quality of life has been analysed through the Short-Form Health Survey Scale (SF-36), Euroqol (EQ-5D), and Health-Related Quality Of Life (HRQOL) instruments<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2–4,14,15,17</span></a><span class="elsevierStyleSup">,</span><a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> (see <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0055" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Cognitive component:</span> the Repeatable Battery for the Assessment of Neuropsychological Status (RBANS) scale,<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,22,23</span></a> the Montreal Cognitive Assessment (MOCA) test, and the Telephone Interview for Cognitive Status (TICS)<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,17</span></a> were used. Moreover, in 2019 Wang et al.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> validated the self-report version of the Healthy Aging Brain Care Monitor (HABC-M) to screen for post-acute care syndrome, which has 27 items to assess the physical, mental and cognitive (see <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0060" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleBold">Minority PICS components:</span> social and economic features were involved in this phenomenon, measured through some instruments. A social worker assesses the financial and employment reality of the family <a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> (see <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><p id="par0065" class="elsevierStylePara elsevierViewall">Pain was another component assessed by some studies, because it is a frequent component in patients discharged from intensive care. Those who were exposed to restricted mobility, a variety of medical devices and multiple injections. In the study by Busico et al.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> pain was measured within the Euroqol EQ-5D-3L instrument in question 4, recommending its assessment as a treatment once the patient is discharged from the ICU (see <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion and recommendations</span><p id="par0070" class="elsevierStylePara elsevierViewall">The results of the present review showed complete similarity with the approach components outlined by LaBuzetta et al.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> and the PICS symptoms found by Sevin et al.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> At the same time, there were partial similarities with the six attributes of PICS described by Yuan et al.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a>: multidimensional impairments, physical dysfunction, psychological disorder, cognitive impairment, failed social reconstruction and persistent symptoms. Of these, the last two were not found in the included studies, an aspect to be considered in future research.</p><p id="par0075" class="elsevierStylePara elsevierViewall">This review found three studies<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,12,14</span></a> that used the PTSS scale to measure Post-Traumatic Stress Disorder (PTSD). However, Rosendahl et al.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a> recommend the addition of the Diagnostic Manual of Mental Disorders (DMS-5) checklist to further refine the diagnosis of PTSD. The evidence also showed that for assessing the cognitive component, Wood et al.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> using robotic technology detects 2% more neurocognitive impairments at three months and 8% more at 12 months, compared to the RBANS scale. However, the difference in economic costs that the implementation of such technology may generate is not specified.</p><p id="par0080" class="elsevierStylePara elsevierViewall">In addition to the PICS screening instrument by Wang et al.,<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> the Post-Intensive Care Syndrome Questionnaire (SPCIQ) by Jeong et al.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> with 31 items (11 mental, 11 cognitive and 9 physical components) was found to have adequate validity and reliability (Cronbach’s alpha 0.93). These instruments are a suitable option, as they allow all components to be assessed in a single format, which would facilitate their implementation and adherence by health personnel.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Finally, no instrument or scale was found that assessed social or occupational impairment, which has a high impact on people’s quality of life. The assessment of PICS should be promoted, as currently due to COVID-19, there are more people in the ICU with prolonged use of sedatives, analgesics and relaxation, which greatly favours post-intensive care syndrome.</p><p id="par0090" class="elsevierStylePara elsevierViewall">A limitation of this review was the inclusion of articles that were available in full text, which may have led to the loss of information published in other studies. The scope of this review was the description of the instruments available to assess this event; however, future reviews may focus on the evaluation of the psychometric properties of the tools, as well as studies of cultural adaptations to local languages that favour the timely assessment of PICS.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Taking these aspects into account, it is possible to conclude that, although PICS is a recently conceptualised and identified phenomenon, progress has been made in the development of instruments that allow its recognition. However, the evidence shows little consensus and great diversity of tools, probably due to the complexity of the phenomenon, since it is made up of alterations in components of notable impacts, such as the physical, mental and cognitive.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Thus, the PICS assessment could be wasteful, running the risk of not being taken into account or being omitted. Therefore, instruments that measure the three components comprehensively are needed , such as those currently available, or to unify or validate new tools that measure the phenomenon reliably and efficiently through a single synthesised questionnaire.</p><p id="par0105" class="elsevierStylePara elsevierViewall">Other components should be considered, such as those found in this review, related to pain, quality of life, social and economic environment, and even others from the family functionality, taking into account that this phenomenon occurs at home, in a family, social, cultural and even work environment. This review included 22 articles that met the criteria of: research or review typology, English, Spanish or Portuguese language, with access to the full text and published between 2015 and 2020.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusions</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">Most of the instruments used to measure post-intensive care syndrome are divided according to the components of physical affectation (the Medical Research Council scale, the Katz index and the Barthel index); cognitive (Repeatable Battery for the Assessment of Neuropsychological Status, and the Montreal Cognitive Assessment test); and mental (Hospital Anxiety and Depression Scale, Beck’s anxiety test, Depression Inventory Second Edition scale and Post Traumatic Stress Syndrome-14 scale).
