Pediatric age (<16 years of age) transplants account for 2.7% of the total transplant activity in Spain, with 118 patients.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">From the moment the patient is diagnosed with a serious pathology requiring transplant, a care plan needs to be established. This care plan has to cover both the pre and post transplant periods. The success of the transplant and the patient's quality of life depends, to a large extent on correct follow-up of this therapeutic plan. It should be personalised and adapted to patient needs and should also take into account the carers and all those factors influencing their environment. Clinical management of a transplanted child is complex and requires a multidisciplinary team.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">2,3</span></a> This is only possible through assistance continuity and coordination work between different levels of care (hospital, primary care (PC), social workers, environment and school, and even the tertiary sector) and the family. If the patient is a child or teenager it is also essential for transition work to be undertaken,<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">4–6</span></a> whereby the patient is trained in their own self-care and responsibility prior to transferring them to adult teams. It is only in this way that adherence may be maintained. The aim of this article is to describe the PC nursing care plan and the need for multidisciplinary coordination in cases of pediatric transplant.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Case description</span><p id="par0015" class="elsevierStylePara elsevierViewall">A girl aged 14, who was born in Spain of Gambian parents and who had lived intermittently in Gambia. The pathological background was as follows: she was diagnosed and treated for malaria in Gambia in 2013. She is the third of 6 siblings who are aged between 18 years and 3 months. Socio-cultural level of the parents is low. Their mother tongue is Soninke. The father has a partial language barrier and the mother a total language barrier. The patient has no language barrier and has been educated irregularly, suffering from low academic performance, alternating between schools, depending on the place of residence.</p><p id="par0020" class="elsevierStylePara elsevierViewall">In March 2014 the patient presented at the pediatrician in the primary health care centre with dizziness, asthenia, and bleeding gums. Haemostasis was difficult to achieve in the wounds. She was referred to the emergency hospital services where portal hypertension and non-alcoholic fatty liver disease was observed. She was referred to a tertiary level hospital centre for further study and treatment. Polycystic liver disease was diagnosed and she was added to the list for a liver transplant from cadaveric donor in December 2014.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Prior to the transplant, the pediatric solid organ transplant nursing manager got in touch with the PC pediatric nurse to establish a follow-up and cooperation plan within the care plan.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Assessment</span><p id="par0030" class="elsevierStylePara elsevierViewall">The data generally grouped into the following areas for Marjory Gordon from different interviews conducted by PC and case management nurses are as follows.</p><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Perception and maintenance of health</span><p id="par0035" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Pre transplant.</span> Non smoker, no passive smoking in the family. Presents with fatigue and dizziness. Does not state pain. Good vaccination control.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The house is clean, it has 3 bedrooms where 8 people live, she shares a room with her sister.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Bad adherence to treatment with non compliance of agreed visits, lack of awareness of the patient with regards to pathology and irregular adherence to pharmacological treatment (omeprazol, phytomenadione, atenolol, amlodipine, losartan and vitamin D). 2–3 weekly controls are needed for blood pressure in the PC centre.</p><p id="par0050" class="elsevierStylePara elsevierViewall">There is an absence of any adult capable of assuming the main carer role and the family is at risk of social exclusion. Income depends exclusively on the father.</p><p id="par0055" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Post transplant.</span> On several occasions the patient has forgotten to arrange health transport to the hospital for the controls or has attended without the required urine samples. It is the teenager herself who is in charge of this task.</p><p id="par0060" class="elsevierStylePara elsevierViewall">She forgets about the check-up visits with the PC nurse and the hospital. When asked about this she says she has forgotten or she has been lapse about it.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Poor therapeutic adherence: she sometimes forgets to take the mediation or update the prescription electronically and occasionally has run out of medication. She shows no concern about this although she realises what the consequences are. The father is in charge of collecting the medication from the pharmacy in question (subsidised pharmacy). He does not check them when he collects them.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The Morisky–Greeen test for treatment compliance was used, where she scores 3 (suspicion of non compliance).</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Nutrition and metabolism</span><p id="par0075" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Pre transplant</span>. Weight: 50<span class="elsevierStyleHsp" style=""></span>kg. Height: 160<span class="elsevierStyleHsp" style=""></span>cm. BMI: 19.5<span class="elsevierStyleHsp" style=""></span>kg/m<span class="elsevierStyleSup">2</span>. Diet is basically based on rice and with few fresh products. Fried food is frequently on the menu.</p><p id="par0080" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Post transplant</span>. Weight: 48<span class="elsevierStyleHsp" style=""></span>kg. Diet with no salt, low in fat. Her sister aged 17 usually cooks. Dietary compliance is doubtful.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Elimination of waste products</span><p id="par0085" class="elsevierStylePara elsevierViewall">No problems.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Activity-exercise</span><p id="par0090" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Pre-transplant</span>. She had been feeling tired and had a raised heart beat when walking of 3-month duration. Physical activity is included in the school curriculum. She is active in household tasks.</p><p id="par0095" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Post transplant</span>. She complies with the guidelines for relative rest and gradual incorporation into the family routine.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Sleep-rest</span><p id="par0100" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Pre-transplant</span>. She had insomnia and frequently woke up.</p><p id="par0105" class="elsevierStylePara elsevierViewall">Post-transplant. No problems.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Cognitive-perceptive</span><p id="par0110" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Pre-transplant</span>. No sensory changes, she has doubts about the donor process.</p><p id="par0115" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Post-transplant</span>. She asks no question about her health situation. She is responsible for self administration of medication. She acknowledges the drugs recommended.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Self-perception-self concept</span><p id="par0120" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Pre-transplant</span>. She does not usually express her feelings. She has no perception of disease.</p><p id="par0125" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Post-transplant</span>. She has no perception of a chronic situation nor vulnerability.</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Role-relationship</span><p id="par0130" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Pre-transplant</span>. She was not concerned. She did not understand what was happening. At school there were behavioural problems. Her class mates were mostly gypsies and Moroccans. She stated she only had one friend, who was from Gambia.</p><p id="par0135" class="elsevierStylePara elsevierViewall">The mother is practically absent throughout, the whole process coincides with the birth of her sixth child. She does not usually accompany the teenager on visits.</p><p id="par0140" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Post-transplant</span>. In several interviews with the father, the family perception is that the teenager is adult enough to manage her medical visits and control her own medication. He occasionally refers to supervision for self administration of medication.</p><p id="par0145" class="elsevierStylePara elsevierViewall">There is an impact at school, due to post-transplant isolation measures. The class does not accept this. There are several incidents because she has to wear a mask.</p><p id="par0150" class="elsevierStylePara elsevierViewall">Due to 3 months relative isolation she was unable to be present in places with a lot of people, and therefore did not attend class.</p><p id="par0155" class="elsevierStylePara elsevierViewall">She states that her expectations for the future are to have a family. She has no hobbies nor shows any interest in any leisure activity. She is sad and irritable.</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Sexuality-reproduction</span><p id="par0160" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Pre-transplant</span>. No menstrual bleeding. The mother underwent female genital mutilation. The protocol for prevention of female genital mutilation is activated. The commitment to the prevention of female genital mutilation has been signed. She does not have any sexual relations.</p><p id="par0165" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Post-transplant</span>. Menstruation begins at 15.</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Confrontation-tolerance of stress</span><p id="par0170" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Pre-transplant</span>. She shows no signs of nervousness or concern.</p><p id="par0175" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Post-transplant</span>. She is worried about incorporation and acceptance at school.</p></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Values and beliefs</span><p id="par0180" class="elsevierStylePara elsevierViewall">Practising Muslim.</p></span></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Plan and execution of care</span><p id="par0185" class="elsevierStylePara elsevierViewall">Following initial evaluation the therapeutic objectives of the team were put to the patient and the family, with a specific care plan being established. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the summary of the nursing diagnostics (NANDA), the expected results (NOC) and interventions (NIC) to be carried out by the corresponding nurses in the pre-transplant phase. Also, in the re-assessment of the post-transplant, phase new care strategies were defined which are shown in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>, maintaining the first phase plan.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Assessment of outcome</span><p id="par0190" class="elsevierStylePara elsevierViewall">Coordination between the case management nurse and the PC nurse took place through joint actions in each of the diagnoses and the preparation of parallel activities during the post-transplant period. This coordination between levels simplified the early detection of problems and effective cooperation with social services, which allowed the family almost immediate free pharmacy services and thus minimised the risk of non compliance of treatment due to financial problems, as has been identified by other authors.<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">7,8</span></a> A social programme was also offered which was rejected by the family, although the option was left open to facilitate the incorporation within the class group, professionals of the transplant team of the hospital, with the support of the PC pediatric nurse, went to the school to give an educational session to classmates and teachers. A home education service was arranged to continue with secondary education and gradual incorporation into classes, with the child terminating the year with good academic performance, in view of the fact that, as reported by Karayurt et al.,<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">9</span></a> 33.3% of transplanted children repeat the year and 44.4% are absent from school for over 20 days.</p><p id="par0195" class="elsevierStylePara elsevierViewall">The desired level of family involvement regarding treatment compliance was not achieved, although many tools were offered: personal and telephone contact to clarify doubts, medication guidelines which were easily understandable for the patient, weekly review of pharmacological compliance for the detection of forgetfulness, and telephonic reminders of programmed visits to achieve good compliance which, according to the literature, is closely related to the survival of these patients.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">9</span></a> Support was requested from the GP. The language and cultural barrier of the parents has impeded interviews and the fostering of family support, even with the help of a cultural mediator. For this reason, the majority of activities have been focused on giving autonomy to the teenager.</p><p id="par0200" class="elsevierStylePara elsevierViewall">Despite the difficulties, the nursing interventions aimed at control of the monitoring of the physical variables have remained stable. Several advances have been reached with regards to self care, acceptance of the disease, involvement and compliance of treatment and family involvement. Further work needs to be achieved in this area however.</p><p id="par0205" class="elsevierStylePara elsevierViewall">On termination of this article, the patient had started to be attended by the adult family team because she had reached the age of 15. A transfer was gradually made by maintaining duality between adult and pediatric nurses to encourage her adherence to treatment and encourage support from her family.</p></span><span id="sec0085" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Discussion. Implications for clinical practice</span><p id="par0210" class="elsevierStylePara elsevierViewall">The pediatric patient of this case underwent a liver transplant due to polycystic liver disease. Her socio-economic situation and lack of adherence to treatment made healthcare collaboration between different levels essential. Inter-level work tries to guarantee that the family accepts the new health situation of one of its members, and ensure the safety and quality of life of the child.</p><p id="par0215" class="elsevierStylePara elsevierViewall">Working between levels in pediatrics is not very common but does open the way to important new lines of nursing. The PC nurse is a connecting link between different health levels. Objectives may be achieved in healthcare plans, as all the different agents participate.</p><p id="par0220" class="elsevierStylePara elsevierViewall">Collaborative work led to intensive follow-up of the pediatric transplant patient by the nurses, avoiding disconnection of the patient to their PC references and facilitating follow-up by the hospital case manager nurse and other professionals.
