in the year 2014&#44; Spain again beat its donation and transplant record&#44; with a rate of 36 donors per million inhabitants and a total of 4360 solid organ transplant recipients&#46; Pediatric age &#40;&#60;16 years of age&#41; transplants account for 2&#46;7&#37; of the total transplant activity in Spain&#44; with 118 patients&#46;<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">From the moment the patient is diagnosed with a serious pathology requiring transplant&#44; a care plan needs to be established&#46; This care plan has to cover both the pre and post transplant periods&#46; The success of the transplant and the patient&#39;s quality of life depends&#44; to a large extent on correct follow-up of this therapeutic plan&#46; It should be personalised and adapted to patient needs and should also take into account the carers and all those factors influencing their environment&#46; Clinical management of a transplanted child is complex and requires a multidisciplinary team&#46;<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">2&#44;3</span></a> This is only possible through assistance continuity and coordination work between different levels of care &#40;hospital&#44; primary care &#40;PC&#41;&#44; social workers&#44; environment and school&#44; and even the tertiary sector&#41; and the family&#46; If the patient is a child or teenager it is also essential for transition work to be undertaken&#44;<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">4&#8211;6</span></a> whereby the patient is trained in their own self-care and responsibility prior to transferring them to adult teams&#46; It is only in this way that adherence may be maintained&#46; The aim of this article is to describe the PC nursing care plan and the need for multidisciplinary coordination in cases of pediatric transplant&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Case description</span><p id="par0015" class="elsevierStylePara elsevierViewall">A girl aged 14&#44; who was born in Spain of Gambian parents and who had lived intermittently in Gambia&#46; The pathological background was as follows&#58; she was diagnosed and treated for malaria in Gambia in 2013&#46; She is the third of 6 siblings who are aged between 18 years and 3 months&#46; Socio-cultural level of the parents is low&#46; Their mother tongue is Soninke&#46; The father has a partial language barrier and the mother a total language barrier&#46; The patient has no language barrier and has been educated irregularly&#44; suffering from low academic performance&#44; alternating between schools&#44; depending on the place of residence&#46;</p><p id="par0020" class="elsevierStylePara elsevierViewall">In March 2014 the patient presented at the pediatrician in the primary health care centre with dizziness&#44; asthenia&#44; and bleeding gums&#46; Haemostasis was difficult to achieve in the wounds&#46; She was referred to the emergency hospital services where portal hypertension and non-alcoholic fatty liver disease was observed&#46; She was referred to a tertiary level hospital centre for further study and treatment&#46; Polycystic liver disease was diagnosed and she was added to the list for a liver transplant from cadaveric donor in December 2014&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">Prior to the transplant&#44; the pediatric solid organ transplant nursing manager got in touch with the PC pediatric nurse to establish a follow-up and cooperation plan within the care plan&#46;</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Assessment</span><p id="par0030" class="elsevierStylePara elsevierViewall">The data generally grouped into the following areas for Marjory Gordon from different interviews conducted by PC and case management nurses are as follows&#46;</p><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Perception and maintenance of health</span><p id="par0035" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Pre transplant&#46;</span> Non smoker&#44; no passive smoking in the family&#46; Presents with fatigue and dizziness&#46; Does not state pain&#46; Good vaccination control&#46;</p><p id="par0040" class="elsevierStylePara elsevierViewall">The house is clean&#44; it has 3 bedrooms where 8 people live&#44; she shares a room with her sister&#46;</p><p id="par0045" class="elsevierStylePara elsevierViewall">Bad adherence to treatment with non compliance of agreed visits&#44; lack of awareness of the patient with regards to pathology and irregular adherence to pharmacological treatment &#40;omeprazol&#44; phytomenadione&#44; atenolol&#44; amlodipine&#44; losartan and vitamin D&#41;&#46; 2&#8211;3 weekly controls are needed for blood pressure in the PC centre&#46;</p><p id="par0050" class="elsevierStylePara elsevierViewall">There is an absence of any adult capable of assuming the main carer role and the family is at risk of social exclusion&#46; Income depends exclusively on the father&#46;</p><p id="par0055" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Post transplant&#46;</span> On several occasions the patient has forgotten to arrange health transport to the hospital for the controls or has attended without the required urine samples&#46; It is the teenager herself who is in charge of this task&#46;</p><p id="par0060" class="elsevierStylePara elsevierViewall">She forgets about the check-up visits with the PC nurse and the hospital&#46; When asked about this she says she has forgotten or she has been lapse about it&#46;</p><p id="par0065" class="elsevierStylePara elsevierViewall">Poor therapeutic adherence&#58; she sometimes forgets to take the mediation or update the prescription electronically and occasionally has run out of medication&#46; She shows no concern about this although she realises what the consequences are&#46; The father is in charge of collecting the medication from the pharmacy in question &#40;subsidised pharmacy&#41;&#46; He does not check them when he collects them&#46;</p><p id="par0070" class="elsevierStylePara elsevierViewall">The Morisky&#8211;Greeen test for treatment compliance was used&#44; where she scores 3 &#40;suspicion of non compliance&#41;&#46;</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Nutrition and metabolism</span><p id="par0075" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Pre transplant</span>&#46; Weight&#58; 50<span class="elsevierStyleHsp" style=""></span>kg&#46; Height&#58; 160<span class="elsevierStyleHsp" style=""></span>cm&#46; BMI&#58; 19&#46;5<span class="elsevierStyleHsp" style=""></span>kg&#47;m<span class="elsevierStyleSup">2</span>&#46; Diet is basically based on rice and with few fresh products&#46; Fried food is frequently on the menu&#46;</p><p id="par0080" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Post transplant</span>&#46; Weight&#58; 48<span class="elsevierStyleHsp" style=""></span>kg&#46; Diet with no salt&#44; low in fat&#46; Her sister aged 17 usually cooks&#46; Dietary compliance is doubtful&#46;</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Elimination of waste products</span><p id="par0085" class="elsevierStylePara elsevierViewall">No problems&#46;</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Activity-exercise</span><p id="par0090" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Pre-transplant</span>&#46; She had been feeling tired and had a raised heart beat when walking of 3-month duration&#46; Physical activity is included in the school curriculum&#46; She is active in household tasks&#46;</p><p id="par0095" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Post transplant</span>&#46; She complies with the guidelines for relative rest and gradual incorporation into the family routine&#46;</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Sleep-rest</span><p id="par0100" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Pre-transplant</span>&#46; She had insomnia and frequently woke up&#46;</p><p id="par0105" class="elsevierStylePara elsevierViewall">Post-transplant&#46; No problems&#46;</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Cognitive-perceptive</span><p id="par0110" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleItalic">Pre-transplant</span>&#46; maintaining the first phase plan&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Assessment of outcome</span><p id="par0190" class="elsevierStylePara elsevierViewall">Coordination between the case management nurse and the PC nurse took place through joint actions in each of the diagnoses and the preparation of parallel activities during the post-transplant period&#46; This coordination between levels simplified the early detection of problems and effective cooperation with social services&#44; which allowed the family almost immediate free pharmacy services and thus minimised the risk of non compliance of treatment due to financial problems&#44; as has been identified by other authors&#46;<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">7&#44;8</span></a> A social programme was also offered which was rejected by the family&#44; although the option was left open to facilitate the incorporation within the class group&#44; professionals of the transplant team of the hospital&#44; with the support of the PC pediatric nurse&#44; went to the school to give an educational session to classmates and teachers&#46; A home education service was arranged to continue with secondary education and gradual incorporation into classes&#44; with the child terminating the year with good academic performance&#44; in view of the fact that&#44; as reported by Karayurt et al&#46;&#44;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">9</span></a> 33&#46;3&#37; of transplanted children repeat the year and 44&#46;4&#37; are absent from school for over 20 days&#46;</p><p id="par0195" class="elsevierStylePara elsevierViewall">The desired level of family involvement regarding treatment compliance was not achieved&#44; although many tools were offered&#58; personal and telephone contact to clarify doubts&#44; medication guidelines which were easily understandable for the patient&#44; weekly review of pharmacological compliance for the detection of forgetfulness&#44; and telephonic reminders of programmed visits to achieve good compliance which&#44; according to the literature&#44; is closely related to the survival of these patients&#46;<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">9</span></a> Support was requested from the GP&#46; The language and cultural barrier of the parents has impeded interviews and the fostering of family support&#44; even with the help of a cultural mediator&#46; For this reason&#44; the majority of activities have been focused on giving autonomy to the teenager&#46;</p><p id="par0200" class="elsevierStylePara elsevierViewall">Despite the difficulties&#44; the nursing interventions aimed at control of the monitoring of the physical variables have remained stable&#46; Several advances have been reached with regards to self care&#44; acceptance of the disease&#44; involvement and compliance of treatment and family involvement&#46; Further work needs to be achieved in this area however&#46;</p><p id="par0205" class="elsevierStylePara elsevierViewall">On termination of this article&#44; the patient had started to be attended by the adult family team because she had reached the age of 15&#46; A transfer was gradually made by maintaining duality between adult and pediatric nurses to encourage her adherence to treatment and encourage support from her family&#46;</p></span><span id="sec0085" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Discussion&#46; Implications for clinical practice</span><p id="par0210" class="elsevierStylePara elsevierViewall">The pediatric patient of this case underwent a liver transplant due to polycystic liver disease&#46; Her socio-economic situation and lack of adherence to treatment made healthcare collaboration between different levels essential&#46; Inter-level work tries to guarantee that the family accepts the new health situation of one of its members&#44; and ensure the safety and quality of life of the child&#46;</p><p id="par0215" class="elsevierStylePara elsevierViewall">Working between levels in pediatrics is not very common but does open the way to important new lines of nursing&#46; The PC nurse is a connecting link between different health levels&#46; Objectives may be achieved in healthcare plans&#44; as all the different agents participate&#46;</p><p id="par0220" class="elsevierStylePara elsevierViewall">Collaborative work led to intensive follow-up of the pediatric transplant patient by the nurses&#44;
