Spain" "etiqueta" => "d" "identificador" => "aff0020" ] ] "correspondencia" => array:1 [ 0 => array:3 [ "identificador" => "cor0005" "etiqueta" => "⁎" "correspondencia" => "Corresponding author." ] ] ] ] "titulosAlternativos" => array:1 [ "es" => array:1 [ "titulo" => "Nivel de autocuidados en pie diabético y concordancia de 3 sistemas de estratificación de riesgo en una zona básica de salud de Gran Canaria" ] ] "resumenGrafico" => array:2 [ "original" => 0 "multimedia" => array:8 [ "identificador" => "fig0005" "etiqueta" => "Figure 1" "tipo" => "MULTIMEDIAFIGURA" "mostrarFloat" => true "mostrarDisplay" => false "figura" => array:1 [ 0 => array:4 [ "imagen" => "gr1.jpeg" "Alto" => 3678 "Ancho" => 2167 "Tamanyo" => 339477 ] ] "detalles" => array:1 [ 0 => array:3 [ "identificador" => "at0005" "detalle" => "Figure " "rol" => "short" ] ] "descripcion" => array:1 [ "en" => "<p id="spar0005" class="elsevierStyleSimplePara elsevierViewall">Study outline.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall"><elsevierMultimedia ident="tb0005"></elsevierMultimedia></p><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect1065">Introduction</span><p id="par0030" class="elsevierStylePara elsevierViewall">By 2014, 422 million adults worldwide had diabetes, with a global prevalence (age-standardised) of diabetes estimated at 8.5% of the adult population.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> In Spain, public data according to the World Health Organisation showed worrying figures in 2016, with a prevalence of diabetes of 10%–6% in men and 8.2% in women (a total prevalence of 9.4% in 2016).<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">One of the most frequent complications associated with diabetes mellitus is so-called diabetic foot (DF).<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,4</span></a> DF is responsible for a high number of amputations among people with diabetes, which results in a great loss of health in these people, in addition to entailing enormous economic and social costs.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,4</span></a> In the autonomous region of the Canary Islands there is a high prevalence of diabetes associated with high rates of lower limb amputation.<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5,6</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">A fundamental aspect to avoid the onset of DF, as well as amputations deriving from it, is to perform periodic examinations aimed at the early detection of risk factors.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,4,7,8</span></a> However, there are indications that foot examination and risk stratification in people with diabetes is still an unresolved issue in general, and in the area of primary care in particular.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">Different risk stratification systems have been proposed for DF, such as the University of Texas Foot Risk Stratification, the Scottish Foot Ulcer Risk Score, that of the International Working Group on Diabetic Foot (IWGDF) and the American Diabetes Association.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,7</span></a> Most of these systems have seen modifications to their original structure over the years,<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> which sometimes leads to practical difficulties in choosing the system to use. In addition, there are doubts as to which of the multiple DF risk stratification systems proposed is the most appropriate, since, due to the lack of prospective studies, we have little data on the efficacy of these systems in the medium to long term.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">Neither does it appear that foot self-care is adequately promoted among the population with diabetes. It is an under-used primary prevention measure,<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> despite its being a strategy that has been shown to be effective in reducing the onset of foot problems in diabetics.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,12</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">For all these reasons, it is recommended that risk stratification should be introduced into daily practice in primary care for the population with diabetes on a routine basis, as well as assessment of the level of self-care, with the aim of detecting deficient areas that require specific educational measures.</p><p id="par0060" class="elsevierStylePara elsevierViewall">The aims of this study were to evaluate the level of self-care in people with diabetes in a basic health area of Gran Canaria, and to determine the risk of suffering DF through the use of 3 different DF risk stratification systems, and to establish the degree of agreement between these stratification systems.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Method</span><p id="par0065" class="elsevierStylePara elsevierViewall">An observational, cross-sectional and descriptive study was proposed.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The data collection was carried out in the Santa Brígida health centre, which belongs to the Santa Brígida basic health area (<span class="elsevierStyleItalic">ZBS de Santa Brígida</span>), and which depends on the primary care management of Gran Canaria (Canary Islands, Spain). One of the centres nursing clinics provided the physical setting for the study. The data collection period was 6 months (1 January 2018–30 June 2018).</p><p id="par0075" class="elsevierStylePara elsevierViewall">As a guideline, a previous sample calculation was made from a reference population of 1500 people with diabetes criteria in the service portfolio of the Santa Brígida basic health area. Therefore a sample of 178 individuals was sufficient to estimate, with 95% confidence and precision of ±6 percentage units, an expected population percentage of around 25%, taking the mean percentage between the 2 groups of the study by Elías-Viramontes and González-Juárez into account in the pretest.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> A consecutive non-probabilistic type of sampling was chosen for the recruitment.</p><p id="par0080" class="elsevierStylePara elsevierViewall">The patients were recruited in the usual course of medical and nursing consultations by direct referral to the investigators.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The inclusion criteria for the study were people diagnosed with diabetes ICD codes: type 1 DM 1 250,01/type 2 DM 2 250,00. The exclusion criteria were, under 18 years of age, pregnant women, patients with known cognitive impairment, and a diagnosis of diabetes of less than 6 months at the time of recruitment.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The data collection process lasted approximately 30 min for each of the persons under study. The researcher in charge of data collection had specific training in the field of DF care. This process consisted of several phases: phase 1, interview and review of the electronic medical records in order to collect information on certain variables and confirm the inclusion-exclusion criteria; phase 2, completion of the Diabetic Foot Self-Care questionnaire of the University of Malaga (DFSQ-UMA)<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a>; phase 3, individual physical examination of each subject consisting of manual palpation of the posterior tibial and pedal pulses of both extremities, visual examination of both feet, 5.07 mm–10 g Semmes-Weinstein monofilament test on 10 pre-set sites (9 on the plantar area and one on the dorsal area); the points of application of the monofilament were: the heel, inner arch, outer area of the foot, 1st, 2nd, and 3rd metatarsal heads, first, third and fifth toes and upper area of the first toe. In addition, an examination was carried out with a graduated Ryder-Seiffer tuning fork on 3 points (pulp of the big toe, first metatarsal head and internal malleolus) and calculation of the ankle-brachial index (ABI).</p><p id="par0095" class="elsevierStylePara elsevierViewall">The variables included in the study were, age, sex, educational level, type of diabetes, years with diabetes, presence of active ulcer, history of ulcer, history of amputation, type of amputation, existence of skin/nail disorders, existence of hygiene deficit, existence of deformities, ABI, impaired vibration sensitivity, presence of neuropathy and peripheral pulses.</p><p id="par0100" class="elsevierStylePara elsevierViewall">A specific instrument was used to assess the level of self-care, the DFSQ-UMA.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> This instrument measures the level of foot self-care of people with diabetes, giving values ranging from 16 points (low level of self-care) to 80 points (high level of self-care). It consists of 16 questions with 5 possible answers (from 1 to 5 points per answer), distributed over 3 domains: Personal self-care (questions 1–7), Podiatry care (questions 8–11) and Shoes and socks (questions 12–16). The authors report a Cronbach Alpha of .89 for the instrument and a kappa value of .84 (95% confidence Interval [CI] for the mean: .37–.98) to .97 (95% CI for the mean: .93–1.0) plus a significant correlation between the questionnaire scores and HbA1c levels (r = –.16; p < .01) and plasma glucose levels (r = –.226; p < .05).<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">To estimate level of self-care, the number of correct responses obtained was calculated; the results were weighted through cut-off points with the 25th and 75th percentiles, so that those below the 25th percentile were considered to have a low level, between the 25th and 75th percentile a medium level, and above the 75th percentile a high level of self-care. The percentage of people at these levels was then calculated.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Three different systems were used for the risk stratification: the risk classification proposed by the National Institute for Health Care Excellence (NICE) for 2015<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">15</span></a> (which proposes 4 levels of risk: low risk, moderate risk, high risk and DF); the IWGDF<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a> classification (which establishes 6 levels of risk: 0, 1, 2a, 2b, 3a and 3b) and the Diabetic Foot-60-Second Tool© (2012)<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> (which only distinguishes 2 levels of risk: positive-high risk screening, or negative-no risk screening).</p><p id="par0115" class="elsevierStylePara elsevierViewall">SPSS 19.0 (IBM SPSS Statistics 19.0) and the EPIDAT 4.1 programme (Xunta de Galicia, Organización Panamericana de la Salud and Universidad CES de Colombia) were used for the statistical analysis of the data. A descriptive analysis of all the variables studies was conducted, therefore quantitative variables are presented with mean, standard deviation and minimum and maximum values. The frequencies and percentages of each of the qualitative variables were obtained. The kappa index was calculated to study the concordance between the DF stratification methods. In the inferential analysis, the 95% confidence intervals were calculated for the variables under study. A kappa comparison was conducted using the chi-square test and the overall kappa calculation. The relative risk of negative screening from one method compared to another and Fisher’s exact test were estimated to establish whether there were differences between screening methods.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Authorisation was requested and obtained from the Primary Care Management of Gran Canaria (Registration No. 264508/186) in order to undertake the study, after evaluation by the Management’s technicians. In order to carry out the data collection, informed consent forms were given, which were read, understood and signed on a completely voluntary basis by each of the participants selected for the study. Likewise, any queries arising in this regard were resolved. No nominal data were used at any stage of the study, therefore the data collected could not be associated or identified with specific individuals.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0125" class="elsevierStylePara elsevierViewall">The sample studies comprised a total of 182 subjects, from 10 primary care teams from the Santa Brígida health centres (n = 160), Monte Lentiscal clinic (n = 1) and La Atalaya clinic (n = 21) (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). The mean age of the participants was 66.93 years with a standard deviation (SD) of 10.85 years (minimum age = 22, maximum = 88 years, 95% CI for the mean: 65.34–68.52). <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the frequencies and percentages for each of the variables considered.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0130" class="elsevierStylePara elsevierViewall">The mean ABI calculation for the sample was 1.01, with SD = .16 (minimum score = .38, maximum score = 1.69, 95% CI for the mean: .99–1.04); 14 subjects (7.7%) had ABI values below .80 and 17 (9.3%) above 1.20. The rest of the participants (151 [83.0%]) had values equal or greater than .80 but below 1.20.</p><p id="par0135" class="elsevierStylePara elsevierViewall">With regard to level of self-care, the mean DFSQ-UMA questionnaire score was 60.88 points, with SD = 7.93 points (minimum score = 38, maximum score = 77 points, 95% CI for the mean: 59.72–62.05). <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the distribution of the response frequency obtained for each of the questions included in the DFSQ-UMA questionnaire. Calculation of the percentiles yielded scores of 56.00 for the 25th percentile and 66.25 for the 75th percentile. Thus, 30.2% had a low level, 45.1% a medium level and 24.7% a high level of self-care.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0140" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a> shows the risk stratification of the population under study according to the 3 systems. In this table, the values have been recoded to classify patients as at risk or positive screening or the opposite (recoded to dichotomous data). Thus, the risk detected was recoded according to the NICE stratification system into negative screening (low risk on the original scale) and positive screening (moderate-high DF risk on the original scale), and according to the IWGDF system into negative screening (0 risk on the original scale) or positive screening (risk groups 1, 2a, 2b, 3a and 3b on the original scale).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0145" class="elsevierStylePara elsevierViewall">Thus, the concordance between these 3 risk stratification systems is shown in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> where, in general terms, we can see that they have high or very high concordance, where the overall kappa comparison gives us a value of .84 (95% CI for the mean: .79–.89, p = .044), and the High-Risk Diabetic Foot-60-Second Tool© (2012) classifies a higher percentage of people with positive screening for DF risk than the other two systems.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0150" class="elsevierStylePara elsevierViewall">Risk stratification is a key aspect in preventing foot lesions in people with diabetes.<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,7,8</span></a> In this study, we chose the simultaneous use of 3 different risk stratification systems.</p><p id="par0155" class="elsevierStylePara elsevierViewall">The main variables used by these systems are similar, although the importance attached to each parameter considered is different.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> A vulnerable aspect is that the quality of evidence for these systems is low, since there are few validation studies that support their predictive capacity.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> This aspect was one of the reasons that we decided to carry out a stratification with several systems in this study.</p><p id="par0160" class="elsevierStylePara elsevierViewall">The risk classification system proposed by NICE is recommended in our setting by the recent diabetic foot care guidelines of the autonomous region of the Canary Islands.<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> It is based on the Scottish Foot Ulcer Risk Score<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> and proposes 4 levels of risk (low risk, moderate risk, high risk and DF).</p><p id="par0165" class="elsevierStylePara elsevierViewall">An attractive aspect of this system is that it is based on the idea that there is no risk 0 (unlike, for example, the IWGDF classifications), and relies on classical aspects when assessing the risk of lesions in people with diabetes (presence of neuropathy, vascular disease or deformity) and ignores other important parameters, such as the presence of helomas or visual problems, if they were initially collected in the <span class="elsevierStyleItalic">S</span>cottish Foot Ulcer Risk Score.<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,18</span></a></p><p id="par0170" class="elsevierStylePara elsevierViewall">However, the High-Risk Diabetic Foot-60-Second Tool© does consider elements that may be important in the genesis of lesions in people with diabetes, such as nail disorders, corns or other skin problems, and is designed as a simple and quick tool to identify the most important risk factors in the feet of people with diabetes.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a></p><p id="par0175" class="elsevierStylePara elsevierViewall">The results obtained in our study in reference to DF risk stratification in the context of primary care are more conservative than those reported by other authors in our country, such as Alonso-Fernández et al., who found 40% of patients at 0 risk, according to the simplified IWDFG system.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> In a previous study conducted in a different health centre on our island, with a sample of 96 people with diabetes, 44.49% (43 cases) were detected in the level 1 risk category (which would correspond to a 0 foot risk using the previous system).<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a></p><p id="par0180" class="elsevierStylePara elsevierViewall">Despite the importance of risk screening in the feet of people with diabetes, it is still not an established practice. According to the abovementioned study by Alonso-Fernández et al., 87.6% (95% CI for the mean: 84.5–90.7) of the histories reviewed in their study did not include risk stratification and only 65% (95% CI for the mean: 60.6–69.5) of the histories contain data that allow it to be calculated.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a></p><p id="par0185" class="elsevierStylePara elsevierViewall">In addition to risk screening, assessment of foot self-care behaviours is another key point in the care of people with diabetes.<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,20</span></a> In our sample, the level of self-care was higher than in the population where Navarro-Flores et al. validated the DFSQ-UMA questionnaire,<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> and similar to that found by Elías-Viramontes and González-Juárez.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a></p><p id="par0190" class="elsevierStylePara elsevierViewall">To our knowledge, this is the first study, apart from the one mentioned to validate it, that uses the DFSQ-UMA to assess foot self-care levels in the context of primary care in our country, and therefore it is not possible to compare results. Even though this questionnaire has already been used in another country to assess the effectiveness of educational interventions to increase the level of DF foot self-care in the context of a quasi-experimental study.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a></p><p id="par0195" class="elsevierStylePara elsevierViewall">Some previous studies appear to indicate that, similar to screening, assessment of foot self-care levels in people with diabetes is not widespread practice.<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12,19,21</span></a> This assessment is fundamental, since it is the step that should be taken before implementing a specific health education programme for the feet of people with diabetes.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a></p><p id="par0200" class="elsevierStylePara elsevierViewall">We should highlight that question number 5 of the DFSQ-UMA had the lowest percentage of adequate answers, which referred to whether patients had received information and recommendations on how to take care of their feet. Only 19 patients (10.4%) indicated that they had received information and were taking care of their feet, compared to 90 (49.5%) who responded that they had not received information and did not know how to take care of their feet.</p><p id="par0205" class="elsevierStylePara elsevierViewall">In contrast, question 14, which referred to new shoes, had the best percentage of adequate answers (93.4%). The use of inappropriate footwear is an extrinsic factor that often causes lesions in this population,<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,22</span></a> and therefore increasing importance is being given to assessing this factor. <a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,23</span></a></p><p id="par0210" class="elsevierStylePara elsevierViewall">As with any research study, this one is not without its limitations. The first lies in the type of sampling used. Because it is non-probabilistic it can affect the precision of the results and therefore, they should not be used in generalisations regarding the rest of the population. An attempt was made to mitigate this aspect by using consecutive sampling that sought to include all accessible subjects as part of the sample, and which is considered the best non-probability sampling technique. In addition, the exploratory nature of this study was well suited to this type of sampling. We believe that this choice and including a somewhat larger number of subjects that the initial sample estimate, if not eliminates, at least moderates this limitation.</p><p id="par0215" class="elsevierStylePara elsevierViewall">Another limitation is that the High-Risk Diabetic Foot-60-Second Tool© has not been validated in the Spanish language and context and, although in principle we have not detected problems with its use, validation studies are required in our environment to ensure that its use is appropriate in our context.</p><p id="par0220" class="elsevierStylePara elsevierViewall">Finally, as another possible limitation, we can highlight the low number of participants with type 1 diabetes, below the estimated prevalence for this type of diabetes.</p><p id="par0225" class="elsevierStylePara elsevierViewall">This study shows that risk screening for patients with diabetes is a feasible strategy in primary care and that, regardless of the stratification system used for this purpose, a significant percentage of people with diabetes (between 28.6% and 37.4%, depending on the system used) will be at moderate to high risk of foot lesions.</p><p id="par0230" class="elsevierStylePara elsevierViewall">This study is the first to use the High-Risk Diabetic Foot-60-Second Tool© as a risk screening method in our country and the DFSQ-UMA as a tool for assessing the level of self-care in the primary care setting. In addition, although it was not the main objective of this study, the high concordance of this system compared to the other 2 may be a first approximation to its validity, in this case as a convergent criterion, although it is necessary that in future research more exhaustive validation is carried out of the High-Risk Diabetic Foot-60-Second Tool©, as well as confirming the general predictive capacity of the DF risk stratification systems.</p><p id="par0235" class="elsevierStylePara elsevierViewall">Although the 3 risk stratifications have good concordance with each other, the High-Risk Diabetic Foot-60-Second Tool© is able to detect increased risk, which makes it potentially suitable for diabetic foot risk screening, although it is based on a dichotomous view (the existence or non-existence of risk).
