Professional quality of life in pediatric services: A cross-sectional study

The overburden of work, the extended shifts, the lack of autonomy, the difficulty in combining family and working life make this a group at risk of having a lower QWL,<a class="elsevierStyleCrossRef" href="#bib0020">4</a> especially when they are up against complex, highly emotionally charged situations,<a class="elsevierStyleCrossRef" href="#bib0015">3</a> in areas such as oncology, paediatrics and palliative paediatric care (PPC).<a class="elsevierStyleCrossRef" href="#bib0025">5</a> Additionally, although the literature on BO in healthcare professionals is extensive, few cases have made a study of the QWL of the paediatric nurse in oncology or palliative care environments, despite the fact the oncology and PPC nurses are at particular risk of CF, although they may also experience enough CS to mitigate the emotional challenges due to the professional satisfaction afforded by their work.<a class="elsevierStyleCrossRef" href="#bib0030">6</a>The aim of this study was to determine the QWL of paediatric nursing professionals, with consideration of sociodemographic factors, the working context, and the BO, CS and CF relationships between the different jobs.MethodA cross-sectional QWL study on nurses working in general paediatrics, oncology and PPC in Spain. Data were collected in different Hospital units (general hospitalisation, oncology and PPC), where all the nurses of these units were invited to participate. In order to carry out our objective, which would lead to comparison between the different units and obtain a representative sample of the nurses working in PPC units, we had to extend the sample nationally, since there are currently only a few PPC units and between 1 and 3 nurses form part of them. We contacted the association of paediatric palliative care to determine how many units existed in Spain and to speed up the process and they collaborated in distributing contact information of the different units. The people in charge were contacted to request access to contact details by email.The questionnaire was sent via email through the Google forms link, with an invitation to participate and with information regarding anonymity and informed consent. Data were collected ensuring anonymity. Furthermore, the study was approved and authorised by the Provincial Ethics Committee of Malaga (10154-N-21). A total of 75 nurses were invited to participate and the response rate was 91%.Participants supplied demographic data including gender, age and religious beliefs. They also supplied organisational variables such as job location, length of time in that particular job, length of service in the profession and type of contract (the response categories are contained in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).<elsevierMultimedia ident="tbl0005"></elsevierMultimedia>The QWL was assessed using the (ProQOL) scale, version 4 (V4), which consisted of 30 items for assessing the CS, BO, and CF (10 items for each subscale) in the last 30 days, using a Likert type scale of 5 points (0 = never, 5 = very often). Raw and standardised scores were estimated using the ProQOL manual,<a class="elsevierStyleCrossRef" href="#bib0005">1</a> which establishes scores by quartiles for the different subscales, allowing scores to be categorised into high, medium and low scores (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>), whilst also offering mean scores of comparison: CS (mean score 37 (SD 7.30; Cronbach’s α = .88). Approximately 25% of the people obtained scores above 42 and approximately 25% of the people obtained scores under 33. If the highest range was found, good professional satisfaction was probably likely for that job. The BO (mean score 22 (SD 6.80; Cronbach’s α = .75), and CF (mean score 13 (SD 6.30; Cronbach’s α = .87)). The highest scores in these two last scales mean that there is a higher risk of exhaustion and fatigue.Statistical analysis was performed using the SPSS v.25 programme, including descriptive and bivariate statistics of the variables, depending on the nature of the same, and the normality of the distribution was assessed using the Kolmogorov–Smirnov test, for parametric tests (Student’s t-test and chi square), or non-parametric (Wilcoxon test and Man–Whitney U test). Thus, ANOVA was used for quantitative and qualitative variable relationships, with measurements of central robustness in the case of non-homocedasticity (Levene test) using the Welch and Brown–Forsythe test. As descriptors, joint and marginal distributions were calculated, with the mean, standard deviation, measurements of association (relative and absolute risk reduction) and confidence intervals of 95%. No data was lost.ResultsThe final sample comprised 68 professionals, mostly women (85.3%; n = 58) within the age range of 50–59 years, with a permanent employment contract, and a mean length of service of 21.97 (SD 9.06) years, as may be observed in (<a class="elsevierStyleCrossRef" href="#tbl0005">Table1</a>).The different dimensions were analysed according to the sociodemographic characteristics, gender, age and religious beliefs, with the finding that religious healthcare professionals perceived of a higher CF (t = 2.437; p = .017). Regarding organisational variables, we observed a higher CS in healthcare professionals with a temporary employment contract (t = −2.033, p = 0.046), whilst for the other organizational variables no significance was found (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).<elsevierMultimedia ident="tbl0010"></elsevierMultimedia>In contrast, in the analysis by professional groups according to the unit of work we found there were significant differences in the CS dimension between oncology professionals and hospitalisation professionals, with a mean difference of −4.55 (95% CI −7.9 to −1.18; p = .005); (F = 5.223; p = .008).DiscussionThe results obtained show a higher CS in temporary contracts. This could be due to the fact temporary contracts are more common in professionals who have not been working for so long, which would mean less exposure to difficult situations and lower prolonged stress. According to a previous study, the length of professional experience in a social healthcare environment, or the level of care demand of the service appears to be associated with CS.<a class="elsevierStyleCrossRef" href="#bib0010">2</a>In our study, the mean scores obtained in the QWL questionnaire are similar to the cut-off scores established as a mean, except for the high level of CF,<a class="elsevierStyleCrossRef" href="#bib0005">1</a> which was also found in a previous study in paediatric nurses.<a class="elsevierStyleCrossRef" href="#bib0025">5</a> This could be associated with repeated exposure to the suffering of other people, highly stressful environments and the continuous giving of self which not only affects one professionally but is also emotionally exhausting, leading to depersonalization and low levels of personal achievement.<a class="elsevierStyleCrossRef" href="#bib0035">7</a> Having religious beliefs was associated with a lower presence of CF, although no previous studies were found that flagged up this association. The protective factor was reported to be the treatment with patients, based on the satisfaction that helping generates, especially in challenging situations, which involve severity and suffering. <a class="elsevierStyleCrossRef" href="#bib0040">8</a> Here, the oncology nurses of our sample presented a higher CS compared with hospital ward nurses, and this is consistent with the findings from previous studies,<a class="elsevierStyleCrossRefs" href="#bib0025">5,9</a> just as the PPC professionals report of positive professional experiences relating to CS,<a class="elsevierStyleCrossRef" href="#bib0030">6</a> although these were not significant in our study.This study had limitations relating to sample size, to study design which did not allow for the establishment of causal relationships and possible predictive factors of the personal (resilience, empathy, and self-care),<a class="elsevierStyleCrossRefs" href="#bib0025">5,8,10</a> and working (resources, complexity and shifts) QWL<a class="elsevierStyleCrossRefs" href="#bib0035">7,10</a> which should be taken into account in future studies, not merely for the analysis of these phenomena but also for designing methods of prevention. Moreover, we have to take into account the difficulty of interpreting the variable on religious beliefs as these beliefs may be related to other experience variables. However, the main limitation was the comparison of results. This was due to the fact that national studies use the Spanish version of ProQOL IV, with a Likert type scale of 0 to 5, whilst international studies use Version V, with a scale from 1 to 5. The different studies have to consider which version of the questionnaire is being used, since interpretation of scores differs, and comparison is not possible.Finally, having a temporary employment contract, and working in oncology was associated with a higher CS, whilst having religious beliefs reported a lower CF in paediatric nurses. Although paediatric care has unique characteristics, we have to be capable of identifying the predictors which could impact the QWL for their direct relationship with the quality of care and thus develop comprehensive strategies and improve health at work and optimize working conditions. MethodCross-sectional descriptive observational study carried out between January–March 2019 at the national territory. The professional quality of life was measured with ProQOL IV and sociodemographic characteristics were recorded. The data was encoded in Microsoft Office Excel, and analyzed with SPPS considering statistically significant differences p < 0.05. ResultsA total of 68 nurses were included in the study. The results obtained show less satisfaction due to compassion in those professionals who have a permanent employment contract, as well as those who carry out their work in hospitalization positions compared to those in the oncology area. In our sample there are very high levels of compassion fatigue, in addition we observed a significant relationship between compassion fatigue and religious belief, being higher in believing professionals. ConclusionsIt’s necessary to identify associated factors in the professional quality of life to propose intervention strategies and improvement conditions, requiring greater involvement of health institutions to improve the professional quality of life in pediatric nursing." MétodoEstudio observacional descriptivo transversal desarrollado entre enero-marzo de 2019 en el ámbito nacional. Se recopilaron características sociodemográficas y el Cuestionario ProQOL IV. Los datos se codificaron en Microsoft Office Excel, y analizaron con SPSS considerándose diferencias estadísticamente significativas para valores p < 0,05. ResultadosParticiparon 68 enfermeras. Los resultados obtenidos muestran una menor satisfacción por compasión en aquellos profesionales que tienen un contrato de trabajo fijo, así como los que desempeñan su labor en puestos de hospitalización respecto a los del área oncología. En nuestra muestra existe una elevada fatiga por compasión, observando una relación significativa entre la fatiga por compasión y la creencia religiosa, siendo superior en los profesionales creyentes. ConclusionesEs necesario identificar factores asociados en la calidad de vida profesional para plantear estrategias de intervención, y condiciones de mejora siendo preciso una mayor implicación de las instituciones sanitarias para la mejora de la calidad de vida profesional en la enfermería pediátrica." encompasses burnout syndrome &#40;BO&#41;&#58; emotional exhaustion&#44; depersonalisation and a lower sense of personal achievement&#59; compassion fatigue &#40;CF&#41;&#58; psycho-emotional distress&#44; reduced compassion in response to chronic exposure to suffering&#59; and compassion satisfaction &#40;CS&#41;&#58; professional gratification in caring&#46;<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> It has been reported that the QWL involves having professionals who are emotionally exhausted and have health problems&#44; which leads to the loss of their ability to provide adequate care<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> and may affect healthcare institutions in maintaining quality standards&#46;<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The nursing profession demands high levels of social and professional responsibility&#46; The overburden of work&#44; the extended shifts&#44; the lack of autonomy&#44; the difficulty in combining family and working life make this a group at risk of having a lower QWL&#44;<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> especially when they are up against complex&#44; highly emotionally charged situations&#44;<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> in areas such as oncology&#44; paediatrics and palliative paediatric care &#40;PPC&#41;&#46;<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> Additionally&#44; although the literature on BO in healthcare professionals is extensive&#44; few cases have made a study of the QWL of the paediatric nurse in oncology or palliative care environments&#44; despite the fact the oncology and PPC nurses are at particular risk of CF&#44; although they may also experience enough CS to mitigate the emotional challenges due to the professional satisfaction afforded by their work&#46;<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">The aim of this study was to determine the QWL of paediatric nursing professionals&#44; with consideration of sociodemographic factors&#44; the working context&#44; and the BO&#44; CS and CF relationships between the different jobs&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Method</span><p id="par0020" class="elsevierStylePara elsevierViewall">A cross-sectional QWL study on nurses working in general paediatrics&#44; oncology and PPC in Spain&#46; Data were collected in different Hospital units &#40;general hospitalisation&#44; oncology and PPC&#41;&#44; where all the nurses of these units were invited to participate&#46; In order to carry out our objective&#44; which would lead to comparison between the different units and obtain a representative sample of the nurses working in PPC units&#44; we had to extend the sample nationally&#44; since there are currently only a few PPC units and between 1 and 3 nurses form part of them&#46; We contacted the association of paediatric palliative care to determine how many units existed in Spain and to speed up the process and they collaborated in distributing contact information of the different units&#46; The people in charge were contacted to request access to contact details by email&#46;</p><p id="par0025" class="elsevierStylePara elsevierViewall">The questionnaire was sent via email through the Google forms link&#44; with an invitation to participate and with information regarding anonymity and informed consent&#46; Data were collected ensuring anonymity&#46; Furthermore&#44; the study was approved and authorised by the Provincial Ethics Committee of Malaga &#40;10154-N-21&#41;&#46; A total of 75 nurses were invited to participate and the response rate was 91&#37;&#46;</p><p id="par0030" class="elsevierStylePara elsevierViewall">Participants supplied demographic data including gender&#44; age and religious beliefs&#46; They also supplied organisational variables such as job location&#44; length of time in that particular job&#44; length of service in the profession and type of contract &#40;the response categories are contained in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>&#41;&#46;</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0035" class="elsevierStylePara elsevierViewall">The QWL was assessed using the &#40;ProQOL&#41; scale&#44; version 4 &#40;V4&#41;&#44; which consisted of 30 items for assessing the CS&#44; BO&#44; and CF &#40;10 items for each subscale&#41; in the last 30 days&#44; using a Likert type scale of 5 points &#40;0&#8239;&#61;&#8239;never&#44; 5&#8239;&#61;&#8239;very often&#41;&#46; Raw and standardised scores were estimated using the ProQOL manual&#44;<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> which establishes scores by quartiles for the different subscales&#44; allowing scores to be categorised into high&#44; medium and low scores &#40;<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>&#41;&#44; whilst also offering mean scores of comparison&#58; CS &#40;mean score 37 &#40;SD 7&#46;30&#59; Cronbach&#8217;s &#945;&#8239;&#61;&#8239;&#46;88&#41;&#46; Approximately 25&#37; of the people obtained scores above 42 and approximately 25&#37; of the people obtained scores under 33&#46; If the highest range was found&#44; good professional satisfaction was probably likely for that job&#46; The BO &#40;mean score 22 &#40;SD 6&#46;80&#59; Cronbach&#8217;s &#945;&#8239;&#61;&#8239;&#46;75&#41;&#44; and CF &#40;mean score 13 &#40;SD 6&#46;30&#59; Cronbach&#8217;s &#945;&#8239;&#61;&#8239;&#46;87&#41;&#41;&#46; The highest scores in these two last scales mean that there is a higher risk of exhaustion and fatigue&#46;</p><p id="par0040" class="elsevierStylePara elsevierViewall">Statistical analysis was performed using the SPSS v&#46;25 programme&#44; including descriptive and bivariate statistics of the variables&#44; depending on the nature of the same&#44; and the normality of the distribution was assessed using the Kolmogorov&#8211;Smirnov test&#44; for parametric tests &#40;Student&#8217;s t-test and chi square&#41;&#44; or non-parametric &#40;Wilcoxon test and Man&#8211;Whitney U test&#41;&#46; Thus&#44; ANOVA was used for quantitative and qualitative variable relationships&#44; with measurements of central robustness in the case of non-homocedasticity &#40;Levene test&#41; using the Welch and Brown&#8211;Forsythe test&#46; As descriptors&#44; joint and marginal distributions were calculated&#44; with the mean&#44; standard deviation&#44; measurements of association &#40;relative and absolute risk reduction&#41; and confidence intervals of 95&#37;&#46; No data was lost&#46;</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0045" class="elsevierStylePara elsevierViewall">The final sample comprised 68 professionals&#44; mostly women &#40;85&#46;3&#37;&#59; n&#8239;&#61;&#8239;58&#41; within the age range of 50&#8211;59 years&#44; with a permanent employment contract&#44; and a mean length of service of 21&#46;97 &#40;SD 9&#46;06&#41; years&#44; as may be observed in &#40;<a class="elsevierStyleCrossRef" href="#tbl0005">Table1</a>&#41;&#46;</p><p id="par0050" class="elsevierStylePara elsevierViewall">The different dimensions were analysed according to the sociodemographic characteristics&#44; gender&#44; age and religious beliefs&#44; with the finding that religious healthcare professionals perceived of a higher CF &#40;t&#8239;&#61;&#8239;2&#46;437&#59; p&#8239;&#61;&#8239;&#46;017&#41;&#46; Regarding organisational variables&#44; we observed a higher CS in healthcare professionals with a temporary employment contract &#40;t&#8239;&#61;&#8239;&#8722;2&#46;033&#44; p&#8239;&#61;&#8239;0&#46;046&#41;&#44; whilst for the other organizational variables no significance was found &#40;<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>&#41;&#46;</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall">In contrast&#44; in the analysis by professional groups according to the unit of work we found there were significant differences in the CS dimension between oncology professionals and hospitalisation professionals&#44; with a mean difference of &#8722;4&#46;55 &#40;95&#37; CI &#8722;7&#46;9 to &#8722;1&#46;18&#59; p&#8239;&#61;&#8239;&#46;005&#41;&#59; &#40;F&#8239;&#61;&#8239;5&#46;223&#59; p&#8239;&#61;&#8239;&#46;008&#41;&#46;</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0060" class="elsevierStylePara elsevierViewall">The results obtained show a higher CS in temporary contracts&#46; This could be due to the fact temporary contracts are more common in professionals who have not been working for so long&#44; which would mean less exposure to difficult situations and lower prolonged stress&#46; According to a previous study&#44; the length of professional experience in a social healthcare environment&#44; or the level of care demand of the service appears to be associated with CS&#46;<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">In our study&#44; the mean scores obtained in the QWL questionnaire are similar to the cut-off scores established as a mean&#44; except for the high level of CF&#44;<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> which was also found in a previous study in paediatric nurses&#46;<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> This could be associated with repeated exposure to the suffering of other people&#44; highly stressful environments and the continuous giving of self which not only affects one professionally but is also emotionally exhausting&#44; leading to depersonalization and low levels of personal achievement&#46;<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> Having religious beliefs was associated with a lower presence of CF&#44; although no previous studies were found that flagged up this association&#46; The protective factor was reported to be the treatment with patients&#44; based on the satisfaction that helping generates&#44; especially in challenging situations&#44; which involve severity and suffering&#46; <a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> Here&#44; the oncology nurses of our sample presented a higher CS compared with hospital ward nurses&#44; and this is consistent with the findings from previous studies&#44;<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5&#44;9</span></a> just as the PPC professionals report of positive professional experiences relating to CS&#44;<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> although these were not significant in our study&#46;</p><p id="par0070" class="elsevierStylePara elsevierViewall">This study had limitations relating to sample size&#44; to study design which did not allow for the establishment of causal relationships and possible predictive factors of the personal &#40;resilience&#44; empathy&#44; and self-care&#41;&#44;<a class="elsevierStyleCrossRefs" href="#bib0025"><span class="elsevierStyleSup">5&#44;8&#44;10</span></a> and working &#40;resources&#44; complexity and shifts&#41; QWL<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7&#44;10</span></a> which should be taken into account in future studies&#44; not merely for the analysis of these phenomena but also for designing methods of prevention&#46; Moreover&#44; we have to take into account the difficulty of interpreting the variable on religious beliefs as these beliefs may be related to other experience variables&#46; However&#44; the main limitation was the comparison of results&#46; This was due to the fact that national studies use the Spanish version of ProQOL IV&#44; with a Likert type scale of 0 to 5&#44; whilst international studies use Version V&#44; with a scale from 1 to 5&#46; The different studies have to consider which version of the questionnaire is being used&#44; since interpretation of scores differs&#44; and comparison is not possible&#46;</p><p id="par0075" class="elsevierStylePara elsevierViewall">Finally&#44; having a temporary employment contract&#44; and working in oncology was associated with a higher CS&#44; whilst having religious beliefs reported a lower CF in paediatric nurses&#46; Although paediatric care has unique characteristics&#44; we have to be capable of identifying the predictors which could impact the QWL for their direct relationship with the quality of care and thus develop comprehensive strategies and improve health at work and optimize working conditions&#46;         "resumen" => "<span id="abst0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Objetivo</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">Determinar la calidad de vida laboral entre los diferentes profesionales de enfermer&#237;a pedi&#225;trica&#44;teniendo en cuenta factores sociodemogr&#225;ficos y de contexto laboral&#44; y la relaci&#243;n del Burnout&#44; satisfacci&#243;n por compasi&#243;n y fatiga por compasi&#243;n entre los diferentes puestos de trabajo&#46;</p></span> <span id="abst0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">M&#233;todo</span><p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">Estudio observacional descriptivo transversal desarrollado entre enero-marzo de 2019 en el &#225;mbito nacional&#46; Se recopilaron caracter&#237;sticas sociodemogr&#225;ficas y el Cuestionario ProQOL IV&#46; Los datos se codificaron en Microsoft Office Excel&#44; y analizaron con SPSS consider&#225;ndose diferencias estad&#237;sticamente significativas para valores p&#8239;&#60;&#8239;0&#44;05&#46;</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0050" class="elsevierStyleSimplePara elsevierViewall">Participaron 68 enfermeras&#46; Los resultados obtenidos muestran una menor satisfacci&#243;n por compasi&#243;n en aquellos profesionales que tienen un contrato de trabajo fijo&#44; as&#237; como los que desempe&#241;an su labor en puestos de hospitalizaci&#243;n respecto a los del &#225;rea oncolog&#237;a&#46; En nuestra muestra existe una elevada fatiga por compasi&#243;n&#44; observando una relaci&#243;n significativa entre la fatiga por compasi&#243;n y la creencia religiosa&#44; siendo superior en los profesionales creyentes&#46;</p></span> <span id="abst0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Conclusiones</span><p id="spar0055" class="elsevierStyleSimplePara elsevierViewall">Es necesario identificar factores asociados en la calidad de vida profesional para plantear estrategias de intervenci&#243;n&#44; y condiciones de mejora siendo preciso una mayor implicaci&#243;n de las instituciones sanitarias para la mejora de la calidad de vida profesional en la enfermer&#237;a pedi&#225;trica&#46;</p></span>"
