elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Introduction</span><p id="par0015" class="elsevierStylePara elsevierViewall">Alcohol abuse is an important determinant of public health outcomes and contributes significantly to the development of associated diseases, such as liver disease.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Alcohol-associated liver disease (ALD) is the most prevalent cause of advanced liver disease and liver cirrhosis (HC) in Europe, including Spain.<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> ALD includes a wide range of stages ranging from simple hepatic steatosis, alcoholic steatohepatitis with or without fibrosis, LC, and hepatocellular carcinoma (HCC).<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> In the United States, in 2020, 8906 liver transplants (LT) were performed and 6917 LT<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> were performed in the European Union. In Spain, in 2021, 1078 LTs were performed, a rate of 22.7 per million inhabitants (pmp).<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a> In adult populations, in relation to diagnoses for inclusion on the waiting list and LT, CH was the first (55%), followed by tumours (20%) and chronic cholestasis (6%).<a class="elsevierStyleCrossRef" href="#bib0025"><span class="elsevierStyleSup">5</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">The presence of a disease and the complete therapeutic process of LT are key determinants to know each patient’s individual vision of their well-being.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> Patients with ALD have poor health-related quality of life.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> Furthermore, psychological problems along with smoking, loss of social status and unemployment are presented as risk factors for the post-transplant process. The nurse must holistically evaluate the health status of the patients using specific instruments and including specific variables of the liver transplant process. Nursing interventions, after the evaluation, should be aimed at guaranteeing the viability of the liver transplant and the health of the transplanted patient.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The purpose of this study is to address the lack of knowledge about differences in the health status of patients undergoing liver transplantation for ALD, compared to those patients who have received liver transplantation due to other aetiologies of liver disease. Furthermore, we seek to explore these differences both in the pre-transplant period and in the post-transplant period. Research that systematically compares the effects of liver transplantation in these two groups of patients with different underlying causes of liver disease is limited. Our hypothesis is that there are differences in the impact on the health of patients with liver transplants due to ALD versus those patients with other liver disease aetiologies. The objective of this study is to study the health status of a group of patients with liver transplants due to ALD versus other aetiologies, before and after transplant.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Method</span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Study design</span><p id="par0030" class="elsevierStylePara elsevierViewall">A single-centre, prospective, cohort study was conducted in the liver transplant unit of Gregorio Marañón General University Hospital. The Strengthening the Reporting of Observational Studies in Epidemiology (STROBE)<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> statement was followed for study reporting.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Sample/participation</span><p id="par0035" class="elsevierStylePara elsevierViewall">The study population was all adult patients, included on the waiting list for a first elective and/or urgent LT, emergency 0 in the liver transplant unit of Gregorio Marañón General University Hospital from November 2019 to July 2022. The urgent transplant, or emergency 0, is considered within the clinical criteria of the National Transplant Organization (NTO) as a national priority that includes acute liver failure and acute retransplantation.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> All patients who received these types of transplants during said period of time were included. Patients who had previously received an organ transplant and those patients who had required a retransplant with a difference of more than 1 month from the first LT were excluded. In addition, those patients who did not complete the data collection form within 6 months were excluded.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Variables</span><p id="par0040" class="elsevierStylePara elsevierViewall">Sociodemographic variables were collected: age, in years; sex: male/female; civil status: single, married-partner, separated-divorced or widowed; educational level: primary, secondary, high school, vocational or university training; employment status: not working-unemployed, active, retired/pensioner; and religion: non-religious, practicing Catholic, non-practicing Catholic, Muslim or other religions.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> Also clinical variables associated with cardiovascular risk factors: obesity, diabetes mellitus (DM), arterial hypertension, dyslipidemia and smoking. Alcohol consumption was not recorded as a habit, only as an aetiology related to liver disease, since to be a candidate for a liver transplant an abstinence period of at least 6 months is necessary, so all patients at the time of inclusion on the waiting list were abstainers. Variables related to liver disease were collected at the time of inclusion on the waiting list: prognosis of the patient with cirrhosis, liver fragility index, level of anxiety and depression, and aetiology, determined through functional analytical tests and liver serologies. After liver transplantation, the level of anxiety and depression and the emotional–behavioural effects of the transplant were evaluated.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The data collection process was carried out by coordinating the patients’ follow-up visits from inclusion on the waiting list until the months after the transplant. At the time of inclusion, an initial assessment was carried out, collecting sociodemographic, clinical, prognostic variables, liver fragility and anxiety/depression. In the subsequent reviews, corresponding to the first month and the sixth month after the transplant, the anxiety/depression evaluations and the questionnaire on emotional–behavioural effects of the transplant were completed.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Indexes</span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Model for End-Stage Liver Disease (MELDNa)</span><p id="par0050" class="elsevierStylePara elsevierViewall">The Model for End-Stage Liver Disease (MELDNa) is a prognostic index that facilitates the evaluation of liver cirrhosis severity and is used to prioritise patients on the liver transplant waiting list. Its calculation is a logarithmic combination of the laboratory values of bilirubin, INR, creatinine and serum sodium. The MELDNa score accurately predicts mortality at 90 days in patients with cirrhosis. Values greater than 15 points are an indication for liver transplantation, a score <9 predicts 1.9% mortality and a score >40 predicts 71.3% mortality.<a class="elsevierStyleCrossRefs" href="#bib0055"><span class="elsevierStyleSup">11,12</span></a> This instrument was used in the pre-transplant period, at the time of inclusion on the waiting list for liver transplant.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Liver frailty index</span><p id="par0055" class="elsevierStylePara elsevierViewall">The liver frailty index (LFI) is an easy-to-use tool, designed to predict mortality in patients with advanced liver disease. The prognosis resulting from the addition of the liver fragility index and MELDNa, compared to that provided by the MELDNa at 3 months, improved the mortality classification on the waiting list by 17%.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> The LFI consists of three simple physical frailty tests (grip strength, rising from a chair, and balance test). The result obtained allows the patient to be classified as robust (≤3.2), pre-frail (3.3–4.4) and frail (≥4.5). The liver fragility index has excellent reliability with an infra-class coefficient of 0.93.<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14–16</span></a> This instrument was used in the pre-transplant period, at the time of inclusion on the waiting list for liver transplant.</p></span></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Instruments</span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Hospital anxiety and depression scale</span><p id="par0060" class="elsevierStylePara elsevierViewall">The hospital anxiety and depression scale (HADS) is one of the most used self-assessment instruments to detect emotional distress (anxiety and depression) in populations with physical illness.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> It is made up of two subscales (HADA: anxiety and HADD: depression) of seven items that are evaluated with a 4-point Likert scale (0–3). The score of each subscale can vary between 0 and 21, with lower values corresponding to absence of clinically relevant anxiety and/or depression. Eight to ten require follow-up and from 11 to 21 the presence of symptoms is relevant and patients are considered a probable case of anxiety and/or depression.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> Internal consistency, evaluated by Cronbach’s alpha, was 0.90 for the full scale, 0.84 for the depression subscale and 0.85 for the anxiety subscale.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> The validation data of our study was a global Cronbach’s alpha of 0.88, a 0.81 for the depression subscale and a 0.83 for the anxiety subscale. This instrument was used in the pre-transplant period, at the time of inclusion on the waiting list for liver transplant, and subsquently one month and six months after the liver transplant.</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Transplant Effects Questionnaire (TxEQ) Spanish</span><p id="par0065" class="elsevierStylePara elsevierViewall">The Transplant Effects Questionnaire (TxEQ) is a questionnaire that contains five emotional–behavioural subscales that evaluate everything from worry about the transplant, guilt regarding the donor and disclosure or revelation of their new status as a transplant recipient, to adherence to immunosuppressive treatment and responsibility regarding the transplant. This questionnaire allows us to evaluate and compare these emotional–behavioural effects and can help optimise treatment. The Spanish version consists of 22 items scored on a 5-point Likert-type scale ranging from “totally agree” to “totally disagree”, with the maximum score being 5 and the minimum being 1. The score for each subscale is the average of the scores of the items included. All dimensions in the TxEQ-Spanish met internal consistency, and Cronbach’s alpha varied from 0.77 to 0.91.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> In our study, Cronbach’s alpha values varied from 0.65 to 0.76. This instrument was used in the post-transplant period, at one month and six months.</p></span></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Data collection</span><p id="par0070" class="elsevierStylePara elsevierViewall">The questionnaires were hand-delivered by the liver transplant advanced practice nurse and were self-administered by the participating patients. Data were collected at the time of listing and one month and six months after discharge from the liver transplant hospital, coinciding with regular medical check-ups. For clinical reasons, such as severe liver failure and/or health status that made it impossible to carry out the evaluation, it was not possible to self-administer the HADS questionnaire to the emergency 0 transplant patients, nor to perform the LFI before the liver transplant. Despite this situation, we considered including these patients in accordance with the fact that the objective was to evaluate the impact of the aetiology of liver disease in the post-liver transplant period.</p></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Data analysis</span><p id="par0075" class="elsevierStylePara elsevierViewall">An exploratory analysis of the data was performed to identify outliers or extreme values and characterise differences between groups of cases. The most appropriate statistical techniques were identified and it was checked whether the data followed a Gaussian distribution with the Kolmogorov–Smirnov normality test. Data were stored in a database and analysed using SPSS software, statistical package version 26.0 (SPSS Inc., Chicago, Illinois). A descriptive analysis of the data was carried out, using the student’s t test and the chi-square test to compare the different variables. For these non-parametric samples, the Wilcoxon, Mann–Whitney and Kruskal–Wallis signs were used. In all cases, an <span class="elsevierStyleItalic">α</span> value ≤0.05 was established as statistical significance for this study.</p></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0140">Ethical considerations</span><p id="par0080" class="elsevierStylePara elsevierViewall">This study, and its written consent was approved by the Gregorio Marañón General University Hospital Ethics Committee (code IMPACT_TH of the minutes 02/2021) and was carried out in accordance with the principles articulated in the Declaration of Helsinki.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a></p></span></span><span id="sec0085" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0145">Results</span><p id="par0085" class="elsevierStylePara elsevierViewall">A total of n = 74 patients were included in the study, n = 7 were excluded because they did not complete the second measurement at the sixth month. The sample consisted of n = 67 patients with a mean age of 56.37 years (SD 10.79), with n = 45 (67.2%) being men. Of the n = 67 transplant patients, n = 61 (91%) were included as elective transplants and n = 6 (9%) were urgent transplants. ALD, as an aetiological factor, was present in 39% (n = 26), compared to other aetiologies of liver disease (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">In the mental health waiting list evaluation (n = 61), the median anxiety score was 7 [RI 0–16], with n = 37 (60.70%) absence of anxiety, n = 11 (18%) patients who require follow-up and n = 13 (21.30%) patients considered cases of anxiety. The median depression score was 4 [RI 0–16], with n = 48 (78.7%) absence of depression, n = 8 (13.10%) patients requiring follow-up and n = 5 (8, 20%) patients considered cases of depression. Patients transplanted in emergency 0 (n = 6) were excluded from this analysis due to the emergency situation that made it difficult to carry out the pre-transplant evaluation.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Regarding physical health, LFI (n = 50), the mean score was 3.84 (SD 0.69), with robust n = 6 (12%), pre-frail n = 35 (70%) and frail n = 9 (18%). The mean MELDNa scale score was 16.79 (SD 8.14), of which n = 33 (49.30%) had a MELDNa value greater than 15 points. The median number of days on the waiting list was 99 days [4–395].</p><p id="par0100" class="elsevierStylePara elsevierViewall">Of the entire sample n = 67, n = 4 (6%) required immediate retransplantation, n = 2 due to primary malfunction and n = 2 due to arterial thrombosis. The length of stay from LT to hospital discharge was a median of 17 days [RI6–124].</p><span id="sec0090" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0150">Comparison of transplant patients with and without ALD in sociodemographic and clinical variables prior to transplant</span><p id="par0105" class="elsevierStylePara elsevierViewall">The mean age of patients with ALD was 60.35 years (SD 5.39) compared to patients with other types of liver disease whose mean was 54.78 years (SD 12.70) (p = 0.038). Furthermore, the existence of ALD was significantly associated with being male, diabetic, an active smoker, and not being an active worker (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">Regarding the LFI n = 50, the ALD group (n = 21) had a mean of 4.11 (SD 0.46) vs without ALD, which obtained a lower mean of 3.64 (SD 0.76), these differences being statistically significant (p = 0.012).</p></span><span id="sec0095" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0155">Comparison of transplant patients with and without ALD in sociodemographic and clinical variables after transplant</span><p id="par0115" class="elsevierStylePara elsevierViewall">Regarding behaviour in the first six months after liver transplant in the five areas of the TxEQ questionnaire, we found differences in the effect of LT depending on whether the aetiology was ALD or non-ALD (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>). In those patients with ALD, worry increased significantly six months after transplant (p = 0.024), disclosure (p = 0.111) and treatment adherence (p = 0.360) also increased. On the other hand, the results showed a non-statistically significant decrease in guilt and responsibility compared to the first month after transplant.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0120" class="elsevierStylePara elsevierViewall">In contrast, without being statistically significant, in the group of patients without ALD the score in the areas of worry, disclosure and adherence to immunosuppressive treatment decreased six months after LT. What’s more, the blame and responsibility score increased (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). In relation to mental health, a common, non-significant downward trend was observed in both study groups in the depression score. However, the anxiety score in the group of patients with ALD decreased, while that without ALD increased, not significantly (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span></span><span id="sec0100" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0160">Discussion</span><p id="par0125" class="elsevierStylePara elsevierViewall">The results of the study describe that, throughout the liver transplant process, before and after, the aetiology of ALD was associated with different aspects of the patient’s health. This study sought to continue expanding knowledge about the impact of ALD in liver transplantation.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">At the time before transplantation, the results show a statistical significance of ALD non-modifiable risk factors such as age, male sex and diabetes mellitus. The profile of patients with ALDs in men in adulthood, coincided with the data published by the Spanish Association for the Study of the Liver.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> This report warns of the change in the pattern of alcohol consumption in Spain and excessive binge drinking of alcohol in the young population, aged 15–29, of both sexes.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> Although our research does not analyse consumption patterns and the age of our sample is older, it would be interesting to consider these aspects in future research to determine their impact on their health. In this sense, the results of the study should not be extrapolated to Spanish society in general or to European society since other countries, such as a study carried out in Poland, also describes a changing pattern of alcohol consumption and ALD related to the younger population.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> Regarding diabetes mellitus, Krittanawong et al. described the synergistic effect between alcohol consumption and the development of diabetes mellitus and an increased risk of cardiovascular diseases.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> Furthermore, diabetes mellitus in cirrhotic patients is associated with a higher rate of hospital admissions and higher mortality.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a></p><p id="par0135" class="elsevierStylePara elsevierViewall">Modifiable risk factors associated with the presence of ALD were smoking and unemployment. The nurse must focus her interventions on risk factors such as smoking.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> The study by Skladany et al. describes similar results and we share the conclusion that a stratification of risk factors in patients with ALD would guide interventions both in the waiting list period and in the post-transplant period.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">26</span></a> Nursing interventions aimed at addressing smoking in patients with liver problems are essential to reduce the risk of complications, improve the effectiveness of treatment and promote the general health and well-being of these patients. The nurse plays a vital role in providing education, emotional support and follow-up to help patients give up smoking and improve their liver and general health. Unemployment can have adverse effects on a person’s physical and mental health. The stress and anxiety associated with unemployment can have a negative impact on liver function and overall health. The nurse can provide emotional support and coping strategies to help the patient manage stress and minimise its impact on liver health. Additionally, lack of income can impact a patient’s ability to maintain a healthy lifestyle, which in turn can impact liver health.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Regarding the emotional–behavioural effects of the transplant, the patients presented greater concern in the post-transplant process. The score on the worry subscale at one month was higher in patients with non-ALD aetiology. However, at six months worry is similar in both groups. The results coincide with those published where worry correlates positively with guilt and responsibility and inversely with disclosure.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">27</span></a> In their study, Tarabeih et al., described the influence of the type of donor, since patients transplanted by a living donor had a higher level of guilt, and those from a cadaveric donor had higher levels of worry.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> In this study, all patients received LT from deceased donors. Nursing interventions had to focus on providing resources to the patient and reinforcing health education to reduce the level of post-transplant worry. On the other hand, Chuncharunee et al., reported that patients with ALD may be at greater risk of non-adherence to immunosuppressive treatment and, therefore, of graft rejection.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">29</span></a> In this study, the value of adherence to immunosuppressive treatment followed a non-significant upward trend from the first month to the sixth month after transplant, being higher at one month and six months in the non-ALD group. In the study Tarabeih et al., in kidney transplant patients using the TxEQ instrument, the adherence score was significantly lower.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> Regarding religion, in this study the majority of the sample was Catholic and non-religious, compared to the study by Tarebeih et al., who were Arabs and Jews.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">28</span></a> Future studies should evaluate whether this large difference may be related to attitudes towards organ donation and transplantation and trust in the health system and the donation and transplant process. Therefore, Tarabeih et al.’s recommendation on the importance of focusing interventions on patients’ guilt and worry is applicable to this study, to improve correlated aspects such as responsibility and disclosure that will facilitate healthy coping after the incident transplant. After the transplant, an improvement was observed in the areas of mental health, similar to that described in the scientific literature.<a class="elsevierStyleCrossRefs" href="#bib0115"><span class="elsevierStyleSup">23,27</span></a> This study focuses on the analysis of the evolution of anxiety and depression after liver transplantation. Non-ALD patients had a higher anxiety score at one month and six months than ALD patients. It should be noted that the HADS is more sensitive to depression than to anxiety in patients with liver transplants.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a> In this study, those patients with ALD have had a higher rate of liver fragility, so after transplantation, physical improvement along with maintaining abstinence from alcohol consumption could be the cause of the improvement in the patient’s anxiety. The anxiety of those patients whose liver disease could recur in the transplanted graft, may increase.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">30</span></a> Future studies should specify the effects on mental health due to the different aetiologies of liver disease.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Strengths in this study can be identified, such as the use of new scores and questionnaires that are innovative and help us expand learning more about post-transplant processes and thus be able to focus our nursing interventions and design new lines of research. The collection and analysis of Patient Reported Outcomes (PROMs) through the TxEQ-Spanish provides valuable information for healthcare professionals and researchers on how patients perceive their experience after a liver transplant. However, its implementation and use in Spanish is less widespread than in other countries.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">31</span></a> This can influence treatment decisions, planning follow-up care, and identifying areas where further support can be provided to improve the quality of life and well-being of transplant patients. It should be noted that the Liver Transplant Unit at Gregorio Marañón General University Hospital has a high-resolution consultation managed by an Advanced Practice Nurse, which has been able to influence the impact of LT on patients. The presence of an Advanced Practice Nurse to educate, inform and perform clinical follow-up of patients in our study reduces the risks described above. On the other hand, this study has had several limitations such as being a study carried out in a single hospital centre. Data could not be collected from those patients included in emergency 0 and pre-transplant LFI evaluation was lost. This study included consecutive patients seen in a consultation and the main clinical characteristics related to liver disease were collected, so it cannot be ruled out that other variables could be important or act as confounding factors that explain some of our findings. Furthermore, a multivariate statistical study was not carried out to determine the existence of confounding variables. In our study we did not analyse adherence to medical treatment at the time of pre-LT, which a previous study describes as significant.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">32</span></a></p><p id="par0150" class="elsevierStylePara elsevierViewall">Differences in the health impact after liver transplantation have been described between patients transplanted for ALD and patients transplanted for other aetiologies of liver disease. Patients with ALD associated non-modifiable risk factors such as advanced age and DM, and modifiable risk factors such as smoking, which is detrimental to liver disease and their general health, together with social risk factors such as non-work status, giving rise to a sedentary lifestyle and impaired psychosocial sphere. Following liver transplant, patients with ALD report increased adherence, disclosure, and a significant increase in concern about their transplant. In addition, they maintained a downward trend in anxiety and depression levels during the first six months after liver transplant. Interventions in the transplant process such as health education and the promotion of correct adherence to treatment are key elements for these patients.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Patients transplanted for ALD require multidisciplinary follow-up,<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">22</span></a> including physicians, advanced practice nurses, psychologists, and pharmacists, to achieve optimal results. These interdisciplinary teams must take ALD aetiology into consideration, addressing alcohol consumption as the primary objective. The nurse can lead and collaborate in education/counselling programmer and support groups. Adult patients attended in the unit of transplantation of a hospital for a first liver transplant, both elective and urgent, were included. Patients who already had a transplanted organ and those who required liver re-transplantation in the first month after the first transplant were excluded. Sociodemographic and clinical variables, MELDNa, liver frailty index, emotional–behavioral effects of transplantation, level of anxiety and depression were collected. Pearson’s chi-square, Student’s t, Mann–Whitney U, and Wilcoxon sign tests were used for statistical analysis.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0075" class="elsevierStyleSimplePara elsevierViewall">The sample was n = 67 liver transplant patients with a mean age of 56.37 years, 67.2% being men and 39% due to alcohol-related liver disease. 9% of all included patients were urgent transplants. Alcohol consumption was associated with older age, a high rate of liver frailty, and a non-active work situation. Se incluyeron pacientes atendidos para un primer trasplante hepático tanto electivo como urgente en la Unidad de Trasplantes de un hospital. Se excluyeron aquellos pacientes que ya portaban un órgano trasplantado y aquellos que necesitaron un retrasplante hepático en el primer mes tras el primer trasplante. Se recogieron variables sociodemográficas y clínicas, MELDNa, índice de fragilidad hepática, efectos emocionales-conductuales del trasplante, nivel de ansiedad y depresión. En el análisis estadístico se utilizaron las pruebas de chi-cuadrado de Pearson, t de Student, U de Mann-Whitney y el signo de Wilcoxon.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0095" class="elsevierStyleSimplePara elsevierViewall">La muestra fue n = 67 pacientes trasplantados hepáticos, con una edad promedio de 56,37 años, siendo el 67,2% hombres y el 39% por enfermedad hepática relacionada con el alcohol.
