La supervivencia del paciente que ingresa con una enfermedad crítica es cada vez mayor, debido, por un lado, a un mayor conocimiento científico y, por otro, a la disponibilidad de mejores recursos asistenciales y medidas de soporte vital<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>. La capacidad de los profesionales de críticos para adaptarse a las nuevas realidades asistenciales ha llevado a un cambio en el enfoque, no sólo centrándose en la supervivencia, sino también en destacar las cuestiones relacionadas con la situación física, psicológica y social de los pacientes después de su estancia en una UCI<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>. Por consiguiente, la estancia del paciente en una UCI conlleva una serie de condiciones clínicas que implicarán dificultades, en mayor o menor medida, para realizar las actividades básicas de la vida diaria (ABVD) y que se enmarcan dentro de lo que se conoce como el Síndrome Post-UCI<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">La debilidad muscular es uno de los componentes incluidos en el Síndrome Post-UCI, y su abordaje requiere de cuidados profesionales por parte de las enfermeras. Se ha demostrado que los pacientes que experimentan esta debilidad muscular presentan peores resultados asistenciales y tasas de mortalidad más altas<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,5</span></a>. Por lo tanto, las enfermeras de esas áreas tienen la responsabilidad de implementar medidas que garanticen la cobertura de las necesidades y cuidados específicos que estos pacientes requieren<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a>.</p><p id="par0015" class="elsevierStylePara elsevierViewall">En la actualidad, se ha observado que las alteraciones en el proceso de la deglución están adquiriendo relevancia en las UCI. La disfagia es un trastorno que, aunque puede pasar desapercibido inicialmente, puede afectar a un porcentaje significativo de los pacientes ingresados en UCI, según los estudios realizados, que abarcan entre el 3% y el 62% de la población <a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,8</span></a>.</p><p id="par0020" class="elsevierStylePara elsevierViewall">La disfagia se caracteriza por la incapacidad parcial o absoluta de deglutir eficazmente, tanto los alimentos líquidos o sólidos, e incluso la saliva<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a>. Las causas son multifactoriales, entre ellas la debilidad muscular, los efectos secundarios de la medicación utilizada y la presencia de dispositivos en la vía aérea<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a>. Varios autores han demostrado la asociación de la disfagia con el proceso de postextubación<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,11</span></a>, la gravedad de la enfermedad (APACHE II y SAPS II), la enfermedad respiratoria, la presencia de traqueostomía, la duración de la ventilación mecánica, la estancia prolongada en UCI y en hospitalización; y cómo estos factores aumentan el riesgo de desarrollarla<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,12,13</span></a>. Por tanto, existe la necesidad de que los propios profesionales sanitarios asistenciales estén familiarizados con los factores de riesgo identificados en la literatura actual <a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,13,14</span></a>.</p><p id="par0025" class="elsevierStylePara elsevierViewall">La presencia de disfagia conlleva diversas complicaciones, como un mayor riesgo de aspiración, neumonía aspirativa, retraso en el inicio de la alimentación oral, desnutrición, prolongación de la estancia hospitalaria y un incremento de la mortalidad y la morbilidad<a class="elsevierStyleCrossRefs" href="#bib0035"><span class="elsevierStyleSup">7,9,14</span></a>. Algunos estudios han demostrado que la mortalidad se incrementa, a los 28 y a los 90 días del ingreso en UCI, en los pacientes que con un test de disfagia positivo durante su estancia en UCI, en comparación con los pacientes cuyo test fue negativo, lo cual resalta aún más la importancia de su detección y manejo adecuados<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a>.</p><p id="par0030" class="elsevierStylePara elsevierViewall">En el paciente crítico, se ha demostrado que mantener una alimentación adecuada por vía digestiva es fundamental para la recuperación de la salud. Por lo tanto, asegurar una vía aérea segura y un manejo adecuado de los alimentos debe ser un elemento clave en nuestra labor diaria. Para lograrlo, es necesario utilizar la evidencia científica disponible y los recursos a nuestra disposición, para prevenir complicaciones<a class="elsevierStyleCrossRefs" href="#bib0015"><span class="elsevierStyleSup">3,15</span></a>. En este sentido, se requieren dos elementos fundamentales. En primer lugar, la concienciación por parte de los profesionales sobre la importancia de la disfagia y su detección precoz. En segundo lugar, la implementación de medidas de cribado consensuados en las distintas unidades para confirmar o descartar la presencia de disfagia en los pacientes.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Para incrementar la sensibilización de los profesionales hacia la detección y actitud ante la disfagia, Nielsen et al. mencionan que tiene que existir una colaboración interprofesional con el objetivo de evitar la aspiración y avanzar hacia la rehabilitación de la deglución de forma lo más segura posible<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">16</span></a>. Sin embargo, en el estudio de Santana-Cabrera et al. a pesar de que los participantes consideraban la disfagia un problema importante, sólo el 26% de las UCI analizadas contaban con un procedimiento estandarizado para la atención de la disfagia y en menos del 10% se aplicaba de manera sistemática a todos los pacientes<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a>.</p><p id="par0040" class="elsevierStylePara elsevierViewall">En cuanto a los métodos de cribado que detectan este problema, la bibliografía determina varios modelos distintos: Functional Oral Intake Scale (FOIS score)<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a>, Modified Viscosity Volume Swallowing Test (mV-VST)<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> o el método de exploración clínica Volumen-Viscosidad (MECV-V)<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a>. Estas pruebas precisan de formación del profesional sanitario que las lleve a cabo y están basadas en los criterios <span class="elsevierStyleItalic">International Dysphagia Diet Standardisation Initiative</span> (IDDSI) que fueron actualizados en 2017<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> y que dividen la bebida y la comida en ocho niveles (0-7) centrados, ambos, en niveles de facilidad del proceso de deglución, desde elementos líquidos ligeros (nivel 0) hasta comida normal (nivel 7). Estas pruebas testean distintos elementos que vayan valorando la funcionalidad del proceso deglutorio. Para ello se deben implementar estrategias en las respectivas unidades, para unificar criterios, donde las enfermeras sean las profesionales que lleven a cabo estas valoraciones previas al inicio de la dieta oral. Para ello deben recibir formación previa al respecto<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">22,23</span></a>.</p><p id="par0045" class="elsevierStylePara elsevierViewall">El desconocimiento sobre la disfagia, de las enfermeras de críticos, y no seguir una metodología reglada de screening, puede provocar una gran variabilidad asistencial que puede incrementar el riesgo de aspiración<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a>. Ambos elementos, conocimientos y utilización de test validados, ofrecen a las enfermeras poder dar seguridad a los pacientes en el reinicio de la dieta oral.</p><p id="par0050" class="elsevierStylePara elsevierViewall">En la realidad actual se observa que las pruebas funcionales de deglución mediante pruebas de imagen son las más efectivas para detectar la disfagia, a pesar de que las pruebas basadas en los criterios IDDSI son los más utilizadas<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a>.Sin embargo, tanto estas pruebas como los enfoques secuenciales, después de obtener test con resultados positivos, no están ampliamente difundidos en la práctica clínica<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a>. Estos hallazgos deben hacernos reflexionar sobre el hecho de que la disfagia es un problema que dificulta la recuperación de los pacientes y puede generar necesidades de atención sanitaria debido a las complicaciones asociadas. En una investigación prospectiva realizada por Huang et al., se observó que los pacientes diagnosticados con disfagia y estratificados según la escala de gravedad de la disfagia (<span class="elsevierStyleItalic">Dysphagia Severity Scale</span>-DDS) y videofluoroscopia tenían un mayor riesgo estadísticamente significativo de desarrollar neumonías en el grupo de disfagia grave en comparación con el grupo de disfagia leve a los 3, 6 y 20 meses de haber sido reclutados para el estudio. En la medición a los 20 meses, el riesgo fue cinco veces mayor en el grupo de disfagia grave (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>5,12; IC del 95%<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>[1,70- 15,37]; p<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0,002)<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a>.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Dada la problemática identificada, tanto durante la hospitalización como en las semanas y meses posteriores al alta, resulta imprescindible implementar medidas basadas en la evidencia científica que aseguren una alimentación segura. Esto comienza con la detección adecuada de los casos, lo que permitirá aplicar medidas correctivas o de apoyo lo más temprano posible, con el objetivo de minimizar el tiempo sin recibir los cuidados adecuados y adaptados a las necesidades de estos pacientes. Al igual que en otros entornos, las enfermeras de las UCI españolas deben desarrollar guías de práctica clínica o procedimientos estandarizados para abordar esta circunstancia, tal como se ha observado en otros estudios<a class="elsevierStyleCrossRefs" href="#bib0080"><span class="elsevierStyleSup">16,26</span></a>.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Como enfermeras de cuidados críticos, es fundamental reconocer la importancia de nuestra actuación, tanto en la nutrición oral como en la enteral, garantizando la seguridad y eficacia en todo el proceso alimenticio. Debemos ser conscientes del riesgo de complicaciones potenciales que podríamos desencadenar al iniciar la tolerancia oral de todos los pacientes críticos, sin haber evaluado previamente su capacidad de deglutir adecuadamente.
