Resumen

El carcinoma hepatocelular en los pacientes afectados de hemocromatosis hereditaria en fase de cirrosis es una de las complicaciones con mayor mortalidad y puede aparecer a pesar de haber movilizado el exceso de hierro con sangrías. Habitualmente se considera que el carcinoma hepatocelular incide siempre sobre hígados cirróticos por lo que las medidas para el diagnóstico precoz de esta complicación tan sólo se recomiendan en este grupo de pacientes. Presentamos el caso de una paciente de 69 años afectada de hemocromatosis sin cirrosis, que habiendo seguido tratamiento deplectivo adecuado, desarrolló 6 años más tarde un carcinoma hepatocelular. Esta observación debe añadirse a 12 casos publicados en la bibliografía y justifica examinar cuál debe ser la actitud más adecuada para el diagnóstico precoz del carcinoma hepatocelular, en los pacientes afectados de hemocromatosis hereditaria, independientemente de si padecen o no cirrosis.

Abstract

Hepatocellular carcinoma in patients with hereditary hemochromatosis in the cirrhotic phase is one of the complications causing greatest mortality and may present in spite of removal of excess iron by bloodletting. Hepatocellular carcinoma is usually considered to occur in cirrhotic livers and consequently measures for the early diagnosis of this complication are only recommended in this type of patient. We present the case of a 69-year-old female patient with noncirrhotic hemochromatosis who, 6 years after undergoing successful treatment, developed hepatocellular carcinoma. This observation should be added to the 12 cases published in the literature. Criteria should be established for the early diagnosis of hepatocellular carcinoma in patients with hereditary hemochromatosis, irrespective of whether they have cirrhosis.
