Introducción

La gestación humana produce múltiples cambios en el organismo materno; la mayoría han sido bien estudiados, pero en lo referente a la circulación hepatosplénica estos cambios son poco conocidos. El empleo sistemático de la ecografía y de la eco-Doppler abre nuevas expectativas para el conocimiento de la circulación esplácnica en esta etapa de la vida.

Material y Método

Estudiamos a 30 pacientes gestantes sanas previo consentimiento informado, y les realizamos analítica, ecografía obstétrica, ecografía hepática y eco-Doppler hepatosplénico durante las semanas de gestación 8-12, 20-24 y 32-36, así como en el puerperio inmediato, para valorar los cambios morfológicos y hemodinámicos que podrían producirse en los vasos esplácnicos.

Resultados

Se observa un aumento del calibre de la vena porta y sus tributarias, así como de sus ramas intrahepáticas, y una ligera disminución del calibre de los vasos suprahepáticos durante el período de embarazo.

Los cambios hemodinámicos encontrados con ecografía Doppler fueron: aplanamiento del trazado Doppler pulsado de los vasos suprahepáticos al ir progresando la gestación; disminución progresiva, más evidente en el tercer trimestre de la velocidad media portal, y descenso de los índices de resistencia de las arterias hepática y mesentérica superior.

Conclusiones

Durante la gestación normal se producen cambios hemodinámicos en la circulación hepatosplénica que pueden ser evaluados de forma adecuada y sin riesgo para las pacientes, mediante la ecografía en tiempo real y la eco-Doppler, cambios que son necesarios conocer para evaluar estos vasos en circunstancias patológicas.

Abstract
Introduction

Pregnancy produces multiple changes in the mother’s body, most of which have been studied. However, changes in hepatosplenic circulation are not well-known. The routine use of ultrasonography and of echo-Doppler has created new possibilities for the knowledge of splenic circulation during pregnancy.

Material and Method

We studied 30 healthy pregnant women who had given their informed consent. To evaluate the morphological and hemodynamic changes that might occur in the splenic vessels during pregnancy and immediate puerperium, laboratory investigations, obstetric and hepatic ultrasonography, and hepatosplenic echo-Doppler were performed between weeks 8-12, 20-24, and 32-36, as well as in the immediate puerperium.

Results

The caliber of the vena porta and its tributaries, as well as that of its intrahepatic branches, increased while the caliber of the suprahepatic vessels slightly decreased during pregnancy. The hemodynamic changes detected by Doppler ultrasonography were: progressive flattening of the pulsed Doppler trace of the suprahepatic vessels during the course of pregnancy; a progressive reduction in mean portal velocity, which was more marked in the third trimester, and a decrease in the markers of resistance in the hepatic artery and superior mesenteric artery.

Conclusions

Hemodynamic changes in hepatosplenic circulation are produced during pregnancy that can be safely and effectively evaluated in real time by ultrasonography and echo-Doppler. Knowledge of these changes is required to evaluate these vessels in pathological conditions.
