Unless contraindicated, anticoagulation should be considered to reduce the risk of thromboembolic outcomes in the majority of patients with AF.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">1</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Although effective, vitamin K antagonists have many disadvantages that have limited their use in clinical practice (i.e. narrow therapeutic window, interactions with drugs and food, delayed onset and offset of anticoagulant effect, need for frequent coagulation monitoring and dosage adjustments).<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">2,3</span></a> A meta-analysis showed that DOACs on the high-dose treatment significantly reduced stroke or systemic embolic events compared with warfarin, mainly driven by a reduction in hemorrhagic stroke. Mortality was lower and intracranial hemorrhage was halved, while gastrointestinal bleeding events were more frequent.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">4</span></a> In the last years, the prescription of DOACs has been progressively increased.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">5,6</span></a> As a result, it is mandatory to analyze the use of these drugs in clinical practice. In Spain, there are some autonomous communities in which the primary care physician can prescribe DOACs directly and other autonomous communities in which the primary care physician has to refer the patient to other specialists to start treatment with DOACs.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">7</span></a> This circumstance could have a relevant impact not only on the clinical profile and management of these patients among different autonomous communities, but also on outcomes.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Hypertension and AF are very common. Both conditions frequently coexist and their prevalence increases rapidly with aging.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">8,9</span></a> Importantly, hypertension is the commonest pathogenic risk factor for the development of AF. Thus, hypertension promotes structural and electrical changes, neurohormonal activation, fibrosis and atherosclerosis that facilitate the development of AF.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">8,9</span></a> In addition, hypertension is a well-known risk factor for stroke not only in the overall population, but also in patients with AF. In fact, hypertension has a cumulative effect on the risk of stroke in this population.<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">9–11</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">To reduce the risk of stroke in patients with AF is mandatory a comprehensive approach that includes both antithrombotic therapy and risk factors control, particularly hypertension.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">1,9,12,13</span></a> Many studies have analyzed the impact of reducing blood pressure (BP) levels to recommended targets in patients with hypertension, including those with AF.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">1,14</span></a> However, the majority of these studies have been limited to patients taking vitamin K antagonists.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">15</span></a> As a result, the information currently available about the clinical profile and management of patients with AF and hypertension taking DOACs according to BP control is very scarce.</p><p id="par0025" class="elsevierStylePara elsevierViewall">The SILVER-AP study was an observational, cross-sectional and multicenter study performed in those autonomous communities in which the primary care physician could prescribe DOACs directly and indeed initiated the prescription of DOACs.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">16</span></a> The BRONCE-AP study was an observational, cross-sectional and multicenter study performed in those autonomous communities in which the primary care physician had to refer the patient to the specialist to start treatment with DOACs.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">17</span></a> In this manuscript, data from both studies were combined to analyze the clinical profile, antithrombotic and bleeding risk and management of AF patients with hypertension according to BP control.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">Patients aged 18 years or older, with a diagnosis of nonvalvular AF at high risk of stroke or systemic embolism, attended in primary care setting, on chronic treatment with anticoagulants, and that were on current treatment with DOACs at least for 3 months were included. Before inclusion, patients had to provide signed informed consent. Patients who had modified their anticoagulant treatment within the 3 months before inclusion or whether they were unable to understand or fulfill the informed consent or the questionnaires of the study were excluded. In this study, the subgroup of patients with hypertension was analyzed.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The SILVER-AP study was approved by the Clinical Research Ethics Committee of IDIAP, Fundación Jordi Gol and was endorsed by the local ethic committees of the participating centers. The recruitment started on May 2015 and ended on December 2015. The BRONCE-AP study was approved by the Clinical Research Ethics Committee of Principado de Asturias and was endorsed by the local ethic committees of the participating centers. The recruitment started on September 2015 and ended on December 2015.</p><p id="par0040" class="elsevierStylePara elsevierViewall">In both studies data were collected from the medical history and physician interview and recorded by each investigator in an electronic case report form specifically performed for these studies. All patients fulfilled the questionnaire of satisfaction with anticoagulant treatment Anti-Clot Treatment Scale (ACTS).<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">18</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">The biodemographic data (age, sex), BP levels, risk factors, cardiovascular disease, type of DOACs and concomitant medication were recorded. In addition, the history of prior major bleeding, anemia or bleeding risk, prior labile INR, chronic obstructive pulmonary disease, sleep apnea, active cancer and depression were also recorded.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Renal insufficiency was defined as an estimated glomerular filtration rate<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>60<span class="elsevierStyleHsp" style=""></span>ml/min (MDRD-4 equation).<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">14</span></a> Labile INR was defined as a time in therapeutic range<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>60% according to the direct method or<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>65% according to Rosendaal.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">7</span></a> Polymedication was considered as a daily intake of 5 or more preparations.</p><p id="par0055" class="elsevierStylePara elsevierViewall">BP determination was performed according to 2013 ESH/ESC Hypertension guidelines.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">14</span></a> BP was determined with 3 separated measurements using validated oscillometric semiautomatic devices and the mean was recorded. Adequate BP was defined as a systolic BP<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>140<span class="elsevierStyleHsp" style=""></span>mmHg and a diastolic BP<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>90<span class="elsevierStyleHsp" style=""></span>mmHg (<140/85<span class="elsevierStyleHsp" style=""></span>mmHg in case of diabetes).<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">14</span></a> The thromboembolic risk was determined by the CHADS<span class="elsevierStyleInf">2</span> and CHA<span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">2</span>-VASc scales and the risk of bleeding by the HAS-BLED scale.<a class="elsevierStyleCrossRefs" href="#bib0265"><span class="elsevierStyleSup">19,20</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">The ACTS is a self-reported measure of satisfaction with anticoagulant treatment during the last 4 weeks of therapy. The ACTS analyzes both the burdens and the benefits of anticoagulant treatment. The ACTS Burdens total score ranges from 12 to 60 (higher score implies lesser burden, consequently, higher satisfaction with anticoagulant treatment), and the ACTS Benefits total score ranges from 3 to 15 (higher score indicates higher benefit, consequently, higher satisfaction with anticoagulant treatment).<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">18</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">Data from the SILVER-AP and BRONCE-AP studies were analyzed separately and together in order to analyze the clinical characteristics and management of hypertensive patients according to BP control. For the descriptive analysis, quantitative variables were described with measures of central tendency and dispersion (mean and standard deviation) and qualitative variables were described as absolute (<span class="elsevierStyleItalic">n</span>) and relative (%) frequencies.</p><p id="par0070" class="elsevierStylePara elsevierViewall">In the bivariate analysis to compare 2 means, parametric (Student's <span class="elsevierStyleItalic">t</span> test) or nonparametric (Mann–Whitney <span class="elsevierStyleItalic">U</span> test) statistical tests were performed based on the sample distribution. To compare percentages, the chi-square test or Fisher test were used, according to the sample size. A logistic regression analysis was performed to identify the variables associated with a poor BP control. Those variables with a <span class="elsevierStyleItalic">P</span>-value<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.10 in the univariate analysis were included in the multivariate analysis. Statistical significance was set at a <span class="elsevierStyleItalic">P</span>-value<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05. The statistical analysis was performed using the SAS statistics package, version 9.4 (SAS Institute Inc., Cary, NC).</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0075" class="elsevierStylePara elsevierViewall">A total of 138 investigators included 1036 patients (790 from the SILVER-AP study and 246 from the BRONCE-AP study) of whom 881 (85%) had hypertension (682 [86.3%] and 199 [80.9%] from the SILVER-AP and BRONCE-AP studies, respectively). Among patients taking vitamin K antagonists before switching to DOACs, a poor INR control was the main reason for change (78.4% in SILVER-AP study and 54.1% in BRONCE-AP study).</p><p id="par0080" class="elsevierStylePara elsevierViewall">The baseline clinical characteristics of the hypertensive population were reported in <a class="elsevierStyleCrossRef" href="#tbl0010">Table 1</a>. Overall, the presence of other risk factors and cardiovascular disease was common. Nearly 60% of patients had a history of prior labile INR. At the moment of the visit, mean BP was 132.6<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>14.3/75.2<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>9.2<span class="elsevierStyleHsp" style=""></span>mmHg (3.3% and 4.5% of patients from SILVER-AP and BRONCE-AP studies had systolic BP<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>160<span class="elsevierStyleHsp" style=""></span>mmHg). Compared with those patients included in the BRONCE-AP study, those patients included in the SILVER-AP study were older and had more frequently dyslipidemia, diabetes, heart failure, renal insufficiency, prior labile INR, and chronic obstructive pulmonary disease. By contrast, the presence of active cancer was more common and BP levels were higher in the BRONCE-AP study.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">Overall, 70.5% of patients achieved BP goals, without significant differences among both studies (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). Baseline concomitant medication is reported in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 2</a>. The most frequent antihypertensive drugs prescribed were diuretics (61.2%), followed by beta blockers (48.7%) and angiotensin receptor blockers (45.2%). Angiotensin-converting enzyme inhibitors were more commonly prescribed in the SILVER-AP study than in the BRONCE-AP study.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">The clinical characteristics of patients according to BP control are shown in <a class="elsevierStyleCrossRef" href="#tbl0020">Table 3</a>. Those patients with a poor BP control had more frequently diabetes, and history of prior labile INR. Patients had a high thromboembolic risk, but without significant differences between those patients with a good BP control compared with those patients with a poor BP control (CHA<span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">2</span>-VASc ≥2: 98.7% vs 98.5%, respectively; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.770). By contrast, more patients with a poor BP control had a higher bleeding risk (HAS-BLED ≥3: 24.0% vs 35.4%, respectively; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 4</a>). With regard to antihypertensive therapy, diuretics (58.9% vs 66.5%; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.035) and angiotensin receptor blockers (42.0% vs 52.7%; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.004) were more frequently prescribed in those patients with a poor BP control (<a class="elsevierStyleCrossRef" href="#tbl0030">Table 5</a>). In addition, oral antidiabetic drugs (24.8% vs 33.8%; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.006) and nonsteroidal anti-inflammatory drugs (6.1% vs 14.2%; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) were also more commonly taken in the group of patients with a poor BP control (<a class="elsevierStyleCrossRef" href="#tbl0030">Table 5</a>). The prescription of DOACs was independent of BP control status (rivaroxaban: 57.5% vs 54.2%, respectively; dabigatran: 22.2% vs 24.2%, respectively; apixaban: 20.3% vs 21.5%, respectively; <span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.668). By contrast, our study included a wide sample of hypertensive patients with AF treated with DOACs in clinical practice. In fact, the hypertensive patients included in our study had more cardiovascular risk factors and comorbidities than those reported in the general population with AF treated with DOACs.<a class="elsevierStyleCrossRefs" href="#bib0250"><span class="elsevierStyleSup">16,17,21–23</span></a> In addition, our data showed that thromboembolic risk and bleeding risk was higher in AF patients with hypertension compared to AF patients without hypertension, both anticoagulated with DOACs.<a class="elsevierStyleCrossRefs" href="#bib0250"><span class="elsevierStyleSup">16,17,21–23</span></a> Thus, whereas median CHA<span class="elsevierStyleInf">2</span>DS<span class="elsevierStyleInf">2</span>-VASc score was 3.0 and median HAS-BLED score was 1.0 in GARFIELD-AF registry,<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">23</span></a> in our study these numbers were 4.4 and 2.0, respectively. Therefore, the results of our study are clinically relevant, since they can be applied to a specific population of patients with AF, such as those with hypertension.</p><p id="par0105" class="elsevierStylePara elsevierViewall">On the other hand, our data showed that compared with those patients included in the BRONCE-AP study, those patients included in the SILVER-AP study had more frequently dyslipidemia, diabetes, heart failure, renal insufficiency, prior labile INR, and chronic obstructive pulmonary disease. As a result, those patients in which the primary care physician could prescribe DOACs directly had a worse clinical profile than those patients in which the primary care physician had to refer the patient to the specialist to start treatment with DOACs. On the other hand, a recent retrospective study from the Veterans Health Administration has shown that in patients with newly diagnosed AF, cardiology care was associated with improved outcomes, potentially mediated by early prescription of oral anticoagulation therapy.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">24</span></a> All these data suggest that the early prescription of oral anticoagulation is associated with better outcomes, and that the management of patients with AF and hypertension can potentially be made by both, the primary care physicians and the cardiologists, including the early prescription of DOACs.</p><p id="par0110" class="elsevierStylePara elsevierViewall">To reduce the risk of stroke in AF patients with hypertension it is necessary not only anticoagulation, but also decreasing BP levels to recommended targets.<a class="elsevierStyleCrossRefs" href="#bib0295"><span class="elsevierStyleSup">25–27</span></a> Even more, it has been reported that achieving BP control seems more important than a history of hypertension at preventing stroke and bleeding in patients with nonvalvular AF.<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">15,27</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">In the last years, BP control has significantly improved in the general hypertensive population worldwide.<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">28</span></a> However, it seems that this improvement has been higher in some subgroups of patients such as those with AF treated with vitamin K antagonists. Thus, data from the FIRE database in Swiss showed that treatment with vitamin K antagonists was independently associated with better systolic and diastolic BP control.<a class="elsevierStyleCrossRef" href="#bib0315"><span class="elsevierStyleSup">29</span></a> Although many reasons can explain this fact, it is likely that the periodic follow-up of these patients due to anticoagulation monitoring and the concern about stroke and bleeding risk may play a significant role.</p><p id="par0120" class="elsevierStylePara elsevierViewall">Since DOACs do not require periodic monitoring of anticoagulant activity, it is important to ascertain whether BP control remains high in patients treated with DOACs. The phase III clinical trials involving DOACs have specifically analyzed the hypertensive population. In the RE-LY study, 78.9% of patients had hypertension. In this study BP control was excellent (mean BP at baseline in patients with hypertension was approximately 133/78<span class="elsevierStyleHsp" style=""></span>mmHg).<a class="elsevierStyleCrossRef" href="#bib0320"><span class="elsevierStyleSup">30</span></a> In the ROCKET-AF trial, more than 90% of patients were hypertensives and two thirds of them achieved BP goals. Importantly, uncontrolled BP was independently associated with stroke and systemic embolism.<a class="elsevierStyleCrossRef" href="#bib0325"><span class="elsevierStyleSup">31</span></a> In the ARISTOTLE study, 87.5% of patients had hypertension. Of these patients, 20% had systolic BP ≥140<span class="elsevierStyleHsp" style=""></span>mm Hg, 6% had diastolic BP ≥90<span class="elsevierStyleHsp" style=""></span>mm Hg, and 17% had both at baseline. Remarkably, high BP levels at any point during the study was independently associated with a higher risk of stroke or systemic embolism.<a class="elsevierStyleCrossRef" href="#bib0330"><span class="elsevierStyleSup">32</span></a></p><p id="par0125" class="elsevierStylePara elsevierViewall">Because clinical trials have strict inclusion and exclusion criteria, the information provided from these studies cannot always be extended to the general population. In this context, studies performed in “real-life” patients are necessary. In our study, 70.5% of patients achieved BP goals. This number is in the line with those reported in clinical trials involving DOACs, but clearly superior to that found in the general hypertensive population in Spain.<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">27</span></a> This is even more important taking into account that hypertensive patients with AF have a worse clinical profile than AF patients without hypertension.<a class="elsevierStyleCrossRefs" href="#bib0250"><span class="elsevierStyleSup">16,17,21–23</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">Remarkably, the patients with nonvalvular AF and hypertension of our study had a higher thromboembolic risk, but without significant differences according to BP control. The group of patients with a poor BP control had more diabetes, as well as a higher bleeding risk, and a history of prior labile INR. However, only HAS-BLED score was an independent predictor of poor BP control. This is not surprising, since uncontrolled BP is associated with a higher HAS-BLED score, and vice versa.<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">20</span></a> On the other hand, the prescription of nonsteroidal anti-inflammatory drugs was more common in the group of patients with a poor BP control. As it is well known, the use of nonsteroidal anti-inflammatory drugs worsens BP control and increases the risk of bleeding.<a class="elsevierStyleCrossRefs" href="#bib0270"><span class="elsevierStyleSup">20,33</span></a> Avoiding the use of these drugs seems mandatory to improve BP control and to reduce bleeding risk.</p><p id="par0135" class="elsevierStylePara elsevierViewall">It has been suggested the existence of a J-curve during hypertension management in patients with AF, with worse outcomes in patients with a BP around 110/60<span class="elsevierStyleHsp" style=""></span>mmHg.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">11</span></a> However, in our study mean BP was 132.6/75.2<span class="elsevierStyleHsp" style=""></span>mmHg, very far from these numbers. Therefore, it seems that reducing BP values to current targets should be recommended also in patients with hypertension and AF.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">1,14</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">Our study showed that satisfaction with anticoagulant treatment was high and independent of BP control. This is in accordance with other studies that have reported a high satisfaction with treatment with DOACs, probably higher than with vitamin K antagonists.<a class="elsevierStyleCrossRef" href="#bib0340"><span class="elsevierStyleSup">34</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">Our study had an observational and cross-sectional design. This design does not allow controlling some potential confounders or establishing causality. Nevertheless, this design is the best to provide a picture of the clinical profile and management of patients in clinical practice. On the other hand, data from 2 different studies were combined in our study. However, the design of both studies was very similar, the number of patients included was high, patients were consecutively included and data were meticulously recorded.
