The world health organisation estimates that less than 1 in 5 people have their hypertension under control.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">1</span></a> Risk factors for raised blood pressure include unhealthy diet, sedentary lifestyle, obesity, use of tobacco and alcohol and family history. The National Family Health Survey (NFHS-4) done in 2015–2016 estimates that 13.6% of males and 8.7%of females in the age group 15–49 years are suffering from elevated blood pressure. Considering that the estimate is for age below 50 years one can imagine the magnitude of prevalence of hypertension.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">Cardiovascular and renal complications of long-standing hypertension have been studied in detail and are well known. The effect of raised blood pressure on the brain has been studied but assessment has been difficult. Vascular factors including hypertension have been known to be risk factors for dementia. Cerebrovascular changes due to hypertension can lead to stroke and dementia.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">3</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Raised blood pressure alters haemodynamic of the cerebral blood flow which may culminate in structural changes in the vessel walls leading to pathological changes like plaque formation, infarcts, micro bleeds, and white matter damage. These inevitable consequences of hypertension can be suspected for the cognitive decline.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">4</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">Various neuropsychological tools are available for assessment of cognition. Tests are there to measure either global cognition or different domains of cognition such as memory, attention, executive function, processing speed, language etc. Many standardised forms differing in their reliability and validity are available.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">5</span></a></p><p id="par0025" class="elsevierStylePara elsevierViewall">In 2016 December, The American heart association came out with a scientific position statement on the effect of hypertension on cognitive functions.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">6</span></a> They found strong evidence about the effect of hypertension on cognition in later life. They further concluded that there was insufficient data to make evidence-based recommendations on the relationship between hypertension, cognition and benefits of treatment.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">6</span></a> This study was done with the aim to ascertain the impact of raised blood pressure on the cognitive domains in middle aged adults.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><p id="par0030" class="elsevierStylePara elsevierViewall">The study was designed as a cross sectional observational study of normotensives (controls) and hypertensives (cases). A total of 413 subjects consented to participate in the study out of which 21 dropped out. After reviewing against the exclusion criteria data from 4 subjects was excluded from the study. Thus, a total of 388 subjects (188 controls and 200 cases) were included in the study. Cognition was assessed using a battery of neuropsychological tests consisting of Ray Auditory Verbal Learning Test (RAVLT), Complex Figural Test (CFT), Digit Span (forward and backward), Spatial Span (forward and backward).</p><p id="par0035" class="elsevierStylePara elsevierViewall">The institutional ethics committee approved the protocol for the study. After the approval subjects were recruited by convenience sampling from the patients and their healthy attendants coming to the Department of Medicine, Indira Gandhi Institute of Medical Sciences (IGIMS), Patna, India, between January 2018 and October 2019.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Subjects included were males between the ages 40–60 years (middle aged adults) and having completed at least 10 years of schooling (High/Secondary School) under the 10<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>2<span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>3 system in India. Subjects suffering from hearing or visual impairment, neurodegenerative or psychiatric illness, history of stroke or heart failure, diabetes, undergoing dialysis and attending school for less than 10 years were excluded from the study. Subjects on medication known to affect cognition were also excluded.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Subjects who were known cases of hypertension and had been on antihypertensive drug therapy (one or more drugs) for at least more than six months were considered as cases.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Controls were those subjects who were having normal blood pressure and not suffering from any disease known to affect cognition. The best controls would be those who are from the same cohort as cases and thus the relatives accompanying the subjects were the best controls. This was done wherever possible. For this study the cut-off of<span class="elsevierStyleHsp" style=""></span>><span class="elsevierStyleHsp" style=""></span>140/90<span class="elsevierStyleHsp" style=""></span>mm of Hg was taken as hypertension. All the subjects were tested in the forenoon to avoid possible diurnal variations.</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Blood pressure measurement</span><p id="par0055" class="elsevierStylePara elsevierViewall">A detailed medical history and demographic data of the subjects was recorded after getting their informed written consent. The subjects were allowed to rest for 5<span class="elsevierStyleHsp" style=""></span>min before measuring their blood pressure. Blood pressure was measured with a mercury sphygmomanometer in the right arm with the subject in seated position. Three measurements with a gap of 2<span class="elsevierStyleHsp" style=""></span>min in between was recorded and their mean was taken as systolic and diastolic blood pressure.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">7</span></a></p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Neuropsychological battery</span><p id="par0060" class="elsevierStylePara elsevierViewall">The tests were chosen so as to evaluate certain domains of cognition in humans. The domains aimed to be tested included memory (auditory, visual, spatial), learning ability, attention, visuo-spatial construction, working memory.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Procedure for administration and details of tests used in the neuropsychological battery have been published elsewhere in literature.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">5,8</span></a> The tests used were Ray Auditory Verbal Learning Test [RAVLT]—average of 5 recall trials, immediate recall post other list, 30<span class="elsevierStyleHsp" style=""></span>min delayed recall of 15 words—, Complex figure test [CFT]—copy, immediate and delayed recall—, Forward and backward digit [DS] and spatial span [SS].<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">5,8–14</span></a> Learning ability and memory domain was assessed from RAVLT-Average scores, attention domain was tested from DS-Forward, executive function from scores of DS-Backward, Visuo-spatial memory from scores of CFT-Recall and working memory from average scores of SS-Forward and Backward.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">5,6</span></a></p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Statistical analysis</span><p id="par0070" class="elsevierStylePara elsevierViewall">Data was tabulated and analysed. Data distribution was determined using the Kolmogorov–Smirnov test. Continuous variables were expressed as mean and SD or median and range. The independent sample <span class="elsevierStyleItalic">t</span>-test was used to compare the normotensive (control) with hypertensive (cases) group. Data was converted into Z scores with normotensives as reference groups. Percentage distribution is used to represent categorical data. Chi-square test was used to test for significance of categorical data. Pearson's correlation was used for bivariate analysis. Multiple linear regression was used to find unique impacts of age, duration of hypertension and educational level. Statistical significance was taken as <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05.</p></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Results</span><p id="par0075" class="elsevierStylePara elsevierViewall">A total of 388 subjects (200 cases and 188 controls) were included in the study. The characteristics of the population are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0080" class="elsevierStylePara elsevierViewall">The mean age of the studied subjects is 52.69<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.20 years. The age distribution of the study subjects is more in the higher age group with almost two-thirds between the ages 51–60 years. The median age is 53.50 years and mode 54 years. When we look at the difference between mean age, height and weight of normotensives and hypertensives, we see that there is no significant difference between the two independent study groups. The mean value for systolic blood pressure (SBP) in normotensives was 113.26<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>5.19 and in hypertensive group was 138.48<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>15.15. For diastolic blood pressure (DBP) the mean values were 71.78<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>4.77 and 85.88<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>10.30 for controls and cases respectively. The mean SBP and DBP of hypertensives is significantly different from normotensives. There difference between SBP and DBP for the two groups is significant. (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05) This difference was significant for pulse pressure also.</p><p id="par0085" class="elsevierStylePara elsevierViewall">If we look at the results from RAVLT set of tests (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>) we see that in the first attempt of learning the list there is no significant difference between hypertensives and normotensives. The remaining four attempts, the recall of interference list and delayed recall scores differed significantly. Similarly, there are significant changes in attention, executive function, visuospatial memory and working memory. The hypertensives have reduced cognitive power in all the domains being tested (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">Different factors influence the different domains of cognition. Unadjusted correlation analysis shows that, age and duration of hypertension has negative correlations with cognitive functions. Whereas increasing educational level has positive correlation with cognitive function (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0095" class="elsevierStylePara elsevierViewall">Using multiple linear regression analysis, independent impact of age, duration of hypertension and educational level are seen. Age has a statistically significant negative influence on all the cognitive domains evaluated. Duration of hypertension has a significant negative impact on all the spheres except attention. Educational achievement has a statistically significant positive impact on learning ability but not on executive function, attention, visuo-spatial memory and working memory (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Discussion</span><p id="par0100" class="elsevierStylePara elsevierViewall">The results from our study show that people with raised blood pressure had lower cognition than people with normal blood pressure. Age, duration of hypertension, educational achievements have the greatest effect on cognitive achievements. In almost all the domains of cognition hypertensives performed poorly than normotensives. Moderate correlation is found between the duration of hypertension and decline in cognitive function. The effect of education has positive impact on cognition in the subjects. Long term effects of decline are well established but, in this study even middle-aged adults have changes in cognitive function</p><p id="par0105" class="elsevierStylePara elsevierViewall">The RAVLT is a test of working and long-term memory, learning ability, recognition, temporal order and motivation. The digit and spatial span test for working memory, attention, recognition of auditory and visual information. The CFT tests for visuo-spatial constructional ability, strategy, visual and long-term memory.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">5</span></a> In our study although there are significant differences between the populations, the changes are not similar for all the tests. If we look at the learning ability, long term memory domain, there is an almost consistent reduction in scores. For visuospatial construct, visual memory, attention after initial decrease there is not much change, maybe some improvement. Subjects in the study are not proficient in English as, it is not their first language. This may have led to hesitancy and demotivation in learning words as compared to drawing a figure or handling numbers in some of the subjects as reflected in results.</p><p id="par0110" class="elsevierStylePara elsevierViewall">Most of the studies looking for cognitive changes in hypertensives are cross sectional in design. Prospective studies have been few and far between. Memory, attention, learning, visuospatial skills, psychomotor abilities and language abilities have been the most commonly studied domains of cognition. Due to a variety of neuropsychological tests applied there is difficulty in comparing different tests.<a class="elsevierStyleCrossRefs" href="#bib0155"><span class="elsevierStyleSup">5,7,8</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">A cross sectional study tried to study blood pressure variability with cognitive decline. The study measured blood pressure of adults in different positions. It concluded that if there was greater variability of blood pressure in different positions and that, the subjects both normotensives and hypertensives had poorer cognitive functions.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">15</span></a> Cross sectional studies measure blood pressure at one point in time. Increased blood pressure leads to poor cognition in later life. Different studies have pointed out to different BP variable as a better predictor of cognitive function. Some of the studies consider systolic blood pressure, others diastolic blood pressure, mean pressure, etc. as determinants of cognition.<a class="elsevierStyleCrossRefs" href="#bib0210"><span class="elsevierStyleSup">16–21</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">Prospective studies have suggested that elevated blood pressure during the middle age is predictor of poor cognition in elderly.<a class="elsevierStyleCrossRefs" href="#bib0240"><span class="elsevierStyleSup">22,23</span></a> A few studies both cross sectional and prospective have shown a U-shaped relationship between hypertension and cognition. They conclude that both high and low blood pressure values are deleterious to mental function. A range of blood pressure is best suited for cognition.<a class="elsevierStyleCrossRefs" href="#bib0250"><span class="elsevierStyleSup">24,25</span></a></p><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Limitations of the study</span><p id="par0125" class="elsevierStylePara elsevierViewall">First, the primary limitation of the cross-sectional study design is that because the exposure and outcome are simultaneously assessed, there is generally no evidence of a temporal relationship between exposure and outcome. That is, although the investigator may determine that there is an association between an exposure and an outcome, there is generally no evidence that the exposure caused the outcome.<a class="elsevierStyleCrossRef" href="#bib0260"><span class="elsevierStyleSup">26</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">Second, neuroimaging tests were not performed which could have correlated about pathological changes in brain due to elevated blood pressure. The aim of the study was to study the effect of increased blood pressure on cognitive function.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">A cross sectional study of hypertensives (cases) and normotensive (controls) was done between January 2018 and December 2019. Subjects were recruited by convenience sampling. Subjects who were known cases of hypertension and had been on antihypertensive drug therapy for at least more than six months were considered as cases. Controls were those subjects who were having normal blood pressure and not suffering from any disease known to affect cognition. Out of 413 subjects, data from 388 subjects was used in the study (200 cases, 188 controls). The subjects were allowed to rest for 5<span class="elsevierStyleHsp" style=""></span>min before measuring their blood pressure. Blood pressure was measured with a mercury sphygmomanometer in the right arm with the subject in seated position. Neuropsychological battery consisted of tests for domains of memory (auditory, visual, spatial), learning ability, attention, visuo-spatial construction, working memory.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">On the neuropsychological tests, patients with hypertension had lower scores in the different domains. Dichos sujetos fueron reclutados de una muestra de conveniencia. Se consideró como casos a aquellos sujetos que fueran casos conocidos de hipertensión y siguieran una terapia antihipertensiva durante al menos 6 meses. Los controles fueron aquellos sujetos con presión arterial normal y que no padecieran ninguna enfermedad conocida que afectara a la cognición. De los 413 sujetos se utilizaron en el estudio los datos procedentes de 388 (200 casos y 188 controles). Se permitió a los sujetos que descansaran durante 5<span class="elsevierStyleHsp" style=""></span>minutos antes de medir su presión arterial, utilizando un esfigmomanómetro de mercurio en el brazo derecho, estando el sujeto en posición sentada. La batería neuropsicológica consistió en pruebas de los dominios de memoria (auditiva, visual, espacial), capacidad de aprendizaje, atención, construcción visoespacial y memoria funcional.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">En las pruebas neuropsicológicas los pacientes hipertensos tuvieron menores puntuaciones en los diferentes dominios.
