Estudio de diagnóstico clínico mediante aparato de presión oscilométrico" ] ] "resumenGrafico" => array:2 [ "original" => 0 "multimedia" => array:7 [ "identificador" => "fig0005" "etiqueta" => "Figure 1" "tipo" => "MULTIMEDIAFIGURA" "mostrarFloat" => true "mostrarDisplay" => false "figura" => array:1 [ 0 => array:4 [ "imagen" => "gr1.jpeg" "Alto" => 806 "Ancho" => 1563 "Tamanyo" => 68483 ] ] "descripcion" => array:1 [ "en" => "<p id="spar0075" class="elsevierStyleSimplePara elsevierViewall">Prevalence of ortostatic hypotension according to different criteria.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Background</span><p id="par0005" class="elsevierStylePara elsevierViewall">Orthostatic hypotension (OH) is a common disorder, possibly underdiagnosed, especially if it is asymptomatic or presents with atypical symptoms.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">1</span></a> It may be the early sign of autonomic dysfunction,<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">2</span></a> with a varying symptomatology ranging from visual disturbances or instability, to even loss of consciousness (syncope) after a postural change.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">3</span></a> In the elderly, OH has been considered a common cause of falls, which can be associated with morbidity from fractures, as well as loss of confidence and fear of falling.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">4</span></a> Its prevalence increases with age, probably related to the number of drug treatments,<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">5</span></a> due to decreased fluid intake and the progressive dysfunction of the autonomic nervous system. Considering OH as an added routine assessment in elderly patients may prevent future falls and, consequently, its complications.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">6</span></a> Moreover, simple measures such as avoiding sudden postural change from supine to standing, sleeping with the head higher or taking into account the standing blood pressure (BP) values when antihypertensive medication is being revised, can lessen its effects and improve the quality of life of people who have this problem.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">7</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">In hypertensive population over 65 years of age, the prevalence of OH varies between 8% and 14%.<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">8,9</span></a> In older patients, it may exceed 55% if they are institutionalized<a class="elsevierStyleCrossRefs" href="#bib0255"><span class="elsevierStyleSup">10,11</span></a> although this depends on the technique and timing of the measurement.<a class="elsevierStyleCrossRef" href="#bib0265"><span class="elsevierStyleSup">12</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">OH is defined as a decrease of at least 20<span class="elsevierStyleHsp" style=""></span>mmHg in systolic blood pressure (SBP) and/or at least 10<span class="elsevierStyleHsp" style=""></span>mm Hg in diastolic blood pressure (DBP) when changing from supine to standing. There are discrepancies on what the most suitable time for the measurement of BP is. The European Neurological Society<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">13</span></a> or Freeman et al.<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">14</span></a> recommends measuring BP after 3<span class="elsevierStyleHsp" style=""></span>minutes of standing, while the European Society of Cardiology proposes to take several sequential measurements on standing and describes three types of OH: initial OH observed in the first 30<span class="elsevierStyleHsp" style=""></span>seconds of standing; classical OH observed after 3<span class="elsevierStyleHsp" style=""></span>minutes of standing, and delayed OH observed after 30 or 45<span class="elsevierStyleHsp" style=""></span>minutes of standing.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">15</span></a> In a recently published review, Ricci et al. propose a diagnostic algorithm through bedside BP supine/standing test (after 1–3–5<span class="elsevierStyleHsp" style=""></span>minutes).<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">16</span></a> After reviewing the literature, we identified no studies performed within our setting on the prevalence of OH in the population over 80 years of age treated in primary health care centre (PHCC). Thus, the study aims to determine the prevalence of OH in a non-institutionalized population aged 80 or older and treated in PHCC, according to different criteria based on sequential measurements taken at minute 0, 1, 3 and 5 of standing.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">This is a diagnostic intervention study carried out in an urban PHCC where take care of 11,373 patients. Eleven general practitioners included patients aged 80 and over who had been selected using the convenience sampling method. Patients who came to the PHCC and could remain in the standing position for 5 or more minutes or who were normally treated at home by their family doctor or by nurses took part in the study. Bedridden patients, patients with atrial fibrillation, hypovolemia (acute gastroenteritis, dehydration, fever) or patients with known disautonomic disorders such as Parkinson's disease or multiple sclerosis were excluded. For people who had difficulties to come to the centre, they were asked to participate by means of a prior telephone call and home visit. The sample size was estimated at 183 subjects, considering an accuracy of 5%, a prevalence of OH of 25% and a confidence level of 95%.</p><p id="par0025" class="elsevierStylePara elsevierViewall">BP was determined, after resting for 5<span class="elsevierStyleHsp" style=""></span>minutes, in the control arm at baseline <a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">17</span></a> in the supine position (2 separate readings one minute taken one minute apart) and then in the standing position using a validated and calibrated<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">18</span></a> oscillometric OMRON 705-CP device. The obese cuff was used where appropriate. Standing BP was determined in the same arm without removing the cuff, immediately after going into the standing position (minute 0) and the first, third and fifth minute (minute 1, minute 3 and minute 5, respectively). During this time the subject was standing, with only the support of the measuring arm at heart level.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The primary endpoints of the study were changes between baseline BP in supine and standing at minutes 0, 1, 3 and 5. OH was defined if there was a decrease of 20<span class="elsevierStyleHsp" style=""></span>mmHg or more in the SBP and/or 10<span class="elsevierStyleHsp" style=""></span>mmHg or more in the DBP, when going from supine to standing in the four sequential measurements listed previously.<a class="elsevierStyleCrossRefs" href="#bib0270"><span class="elsevierStyleSup">13,15</span></a></p><p id="par0035" class="elsevierStylePara elsevierViewall">The information collected included: age, sex, body weight and height, body mass index, sedentary lifestyle (not go out daily from his home). Smoking (regular daily consumption) and excessive alcohol consumption [>20<span class="elsevierStyleHsp" style=""></span>g/day (women) or >40<span class="elsevierStyleHsp" style=""></span>g/day (males)], diabetes based on criteria from the American Diabetes Association<a class="elsevierStyleCrossRefs" href="#bib0300"><span class="elsevierStyleSup">19,20</span></a> hypertension according to the criteria of the European Hypertension Society,<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">17</span></a> hypercholesterolemia,<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">21</span></a> associated cardiovascular disease (stroke), coronary disease, chronic kidney disease (defined by the estimation of glomerular filtration rate according to Levey's simplified formula <30<span class="elsevierStyleHsp" style=""></span>mL/min/1.73<span class="elsevierStyleHsp" style=""></span>m<span class="elsevierStyleSup">2</span> or proteinuria >300<span class="elsevierStyleHsp" style=""></span>mg/24<span class="elsevierStyleHsp" style=""></span>h)<a class="elsevierStyleCrossRef" href="#bib0315"><span class="elsevierStyleSup">22</span></a> peripheral artery disease or heart failure were assessed. Good control was defined when BP measurements taken in supine were below 140/90<span class="elsevierStyleHsp" style=""></span>mmHg (or below 130/80<span class="elsevierStyleHsp" style=""></span>mmHg in people with diabetes or with associated vascular disease) and good control of diabetes if glycated haemoglobin (HbA1c) was below 7%. Pulse pressure (PP) was defined as the difference between systolic and DBP. A PP of ≥60<span class="elsevierStyleHsp" style=""></span>mm Hg was considered as high cardiovascular risk.<a class="elsevierStyleCrossRef" href="#bib0320"><span class="elsevierStyleSup">23</span></a> The total consumption of drugs, and specifically of antihypertensive drugs or other drugs with effects on BP, were analysed.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">8</span></a> Patients were asked about a history of falling over the past year, its causes and symptoms of dizziness on standing up.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Statistical analysis</span><p id="par0040" class="elsevierStylePara elsevierViewall">Descriptive statistics are presented as means (standard deviation, SD) or as a number (percentage). On bivariate analysis the Chi Square was used for qualitative variables and the <span class="elsevierStyleItalic">t</span> Student-Fisher's test was used for comparing two measures with normal distribution or the Mann–Whitney test if appropriate. 95% confidence intervals were calculated and a 5% significance level was accepted. To study the association between the main independent variable (OH at minute 0) and related variables, non-conditional binary logistic regression models were adjusted. For the selection of variables in the multivariate model, those variables considered statistically significant on bivariate analysis (significance level 25%), potential confounding variables and those with greatest clinical relevance were considered.<a class="elsevierStyleCrossRef" href="#bib0325"><span class="elsevierStyleSup">24</span></a> The SPSS for Windows version 15.0 was used for statistical analysis.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Ethical aspects</span><p id="par0045" class="elsevierStylePara elsevierViewall">Study information will be provided orally to the subjects, and they will have the opportunity to ask questions regarding study details and oral consent had been obteined. Confidentiality and anonymity of the data will be ensured according to the 15/1999 Data Protection Act.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Results</span><p id="par0050" class="elsevierStylePara elsevierViewall">The sociodemographic characteristics, lifestyle and clinical characteristics of the 176 participants are shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0055" class="elsevierStylePara elsevierViewall"><a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> shows the prevalence of OH according to different criteria and <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a> the evolution of the mean of systolic and diastolic BP values in sequential measurements. <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the prevalence of HO as sequential measurements taken at minutes 0, 1, 3 and 5 of standing and possible related factors. At minute 0, clinically relevant and statistically significant associations were observed in the sense of increased frequency of OH in elderly patients with sedentary lifestyle, diabetes, higher measurements of SBP, DBP and PP in supine position, in individuals with poorly controlled hypertension and in cases of falling because of dizziness.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">The association between OH and poorly controlled hypertension was significant in all sequential measurements taken as well as in the mean supine DBP. The highest mean supine SBP showed significant association with OH at minute 0 and 1 of standing and marginal significance at minutes 3 and 5 of standing, respectively.</p><p id="par0065" class="elsevierStylePara elsevierViewall">There was no statistically significant association between OH and age, sex, tobacco and alcohol consumption, antihypertensive medication, cardiovascular disease and repeated presence of symptoms of dizziness on standing up at any measurements on standing.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The prevalence of OH in relation to PP, rated at less than 60<span class="elsevierStyleHsp" style=""></span>mmHg and ≥60<span class="elsevierStyleHsp" style=""></span>mmHg, shows a higher prevalence of OH, statistically significant in determinations made at minute 0 (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">In the logistic regression model (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>), after adjusting for confounding variables, the variables associated with higher prevalence of OH, according to criterion 0, were age, sedentary lifestyle, tobacco consumption, diabetes and SBP values.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Discussion</span><p id="par0080" class="elsevierStylePara elsevierViewall">This study, conducted on 176 people over 80 years of age with high comorbidity, showed OH prevalence of 30.7% at minute 0, and a much lower prevalence at minute 3 (18.2%) and at minute 5 (20.5%).</p><p id="par0085" class="elsevierStylePara elsevierViewall">The prevalence of OH obtained at minute 0 is comparable to the observed in other studies.<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">5,25,26</span></a> Weiss et al. observed a prevalence of immediate OH of 39.2% in elderly subjects institutionalized in an acute care centre<a class="elsevierStyleCrossRef" href="#bib0340"><span class="elsevierStyleSup">27</span></a> and Poon observed a prevalence of 55% in subjects aged ≥75 treated in a geriatric centre.<a class="elsevierStyleCrossRef" href="#bib0255"><span class="elsevierStyleSup">10</span></a> In our context, Saez et al.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">8</span></a> found an overall prevalence of 6.8% of OH and Vara et al. <a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">9</span></a> of 14.6% in a hypertensive population higher than 65 years, treated in a PHCC. The PARTAGE study shows a prevalence of 18% in subjects institutionalized ≥80 years.<a class="elsevierStyleCrossRef" href="#bib0345"><span class="elsevierStyleSup">28</span></a></p><p id="par0090" class="elsevierStylePara elsevierViewall">The differences in the prevalence of OH between our study and those described above can be explained by the characteristics of the population (most studies have included people with disabilities and chronic illnesses, institutionalized in long-stay centres or hospital), the techniques for measuring BP and according to the time of determination thereof.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Regarding the latter, the European Society of Neurology<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">13</span></a> recommends the determination of BP at 3<span class="elsevierStyleHsp" style=""></span>minutes of standing and most follow this recommendation<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">5,8,26,29</span></a> based on practical clinical sense. Although some authors have monitored the changes in BP up to 45<span class="elsevierStyleHsp" style=""></span>minutes,<a class="elsevierStyleCrossRefs" href="#bib0355"><span class="elsevierStyleSup">30–32</span></a> beyond three minutes, sharp drops in BP are less frequent.</p><p id="par0100" class="elsevierStylePara elsevierViewall">For the European Society of Cardiology<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">15</span></a> it would be better to rely on sequential measurements of BP in standing and to describe three types of OH: initial (the first 30<span class="elsevierStyleHsp" style=""></span>seconds of standing), classical (3<span class="elsevierStyleHsp" style=""></span>minutes), and delayed (30–45<span class="elsevierStyleHsp" style=""></span>minutes of standing).</p><p id="par0105" class="elsevierStylePara elsevierViewall">We have performed 4 measurements: immediate (minute 0), at minute 1, at minutes 3 and 5<span class="elsevierStyleHsp" style=""></span>minutes of standing, and like Hiitola et al.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">5</span></a> the highest prevalence of OH was the immediate, the lowest at minute 3 increasing back up again at minute 5.</p><p id="par0110" class="elsevierStylePara elsevierViewall">If the changes in SBP are analyzed, the highest declines in our study occurred at minute 0, which explains the higher prevalence of OH at this time. The decline in DBP was also higher at minute 0, although its influence on the prevalence of OH was lower. These results are consistent with those obtained by various authors.<a class="elsevierStyleCrossRefs" href="#bib0335"><span class="elsevierStyleSup">26,32,33</span></a></p><p id="par0115" class="elsevierStylePara elsevierViewall">In our study, we found higher prevalence of OH at minute 0 in elderly who meet more criteria for advanced arteriosclerosis (sedentary life style, diabetes, higher mean values of SBP, DBP and PP, with poorly controlled hypertension) and when they had sustained falls because of dizziness. On multivariate analysis, smoking at present or in the past, is also significantly associated with OH. The prevalence of OH in the determinations of minutes 1, 3 and 5 showed a clinically relevant and statistically significant relationship with mean DBP values at all times of measurement and only with SBP at minute 1<span class="elsevierStyleHsp" style=""></span>minute.</p><p id="par0120" class="elsevierStylePara elsevierViewall">It is unclear whether a sedentary lifestyle is a risk factor for OH or whether, OH is a marker of frailty and the elderly that have OH lead a more sedentary lifestyle.<a class="elsevierStyleCrossRefs" href="#bib0375"><span class="elsevierStyleSup">34,35</span></a> In our study, a lesser prevalence of immediate OH was observed in people with a non-sedentary lifestyle (OR<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.31, 95% CI: 0.12–0.8).</p><p id="par0125" class="elsevierStylePara elsevierViewall">We also found a higher frequency of OH in people with diabetes (44.4% versus 24.6%), consistent with the results of other authors.<a class="elsevierStyleCrossRefs" href="#bib0385"><span class="elsevierStyleSup">36–39</span></a> However, some studies have not observed this relationship<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">5,9,25</span></a> suggesting the need for further research in this field.</p><p id="par0130" class="elsevierStylePara elsevierViewall">Our results show higher mean supine SBP and DBP in people with OH. The greatest decreases in SBP appeared in the first few seconds of standing, without recovering supine BP values. The decreases in DBP were lower and supine values were recovered at minute 5. These findings are consistent with those of several authors<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">8,34,40</span></a> and could be explained by the arterial stiffness common in the elderly. The association of OH and poor BP control may be related to the definition of OH, as a 20<span class="elsevierStyleHsp" style=""></span>mmHg reduction of the SBP with orthostasis is more likely in people with higher baseline BP. Probably physicians in real clinical practice allow higher blood pressure values in those older hypertensive patients with known orthostatic hypotension, in order to prevent some complications, such as falls.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Arterial stiffness may also partly explain the relationship between OH and PP at minute 0, observed in our work. Hiitola et al.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">5</span></a> observed a higher PP in subjects with OH at 1<span class="elsevierStyleHsp" style=""></span>minute and 3.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Some authors have observed association between OH and antihypertensive drug treatments.<a class="elsevierStyleCrossRefs" href="#bib0250"><span class="elsevierStyleSup">9,10</span></a> In our study we have not found this association, which is consistent with the findings of other authors<a class="elsevierStyleCrossRefs" href="#bib0230"><span class="elsevierStyleSup">5,8</span></a> and suggests a lesser involvement of these treatments, as a risk factor for OH. Moreover, participants with good control of BP presented OH less frequently, which also coincides with the findings of Saez et al. and Vara et al.<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">8,9</span></a>.</p><p id="par0145" class="elsevierStylePara elsevierViewall">Falls occur as a result of multiple factors, although the role of OH in this problem is well accepted, there being some controversy on the issue.<a class="elsevierStyleCrossRefs" href="#bib0350"><span class="elsevierStyleSup">29,34,37</span></a> In our study, of the participants who sustained falls due to dizziness, 53.8% had OH versus 11.1% in the case of falls without dizziness.</p><p id="par0150" class="elsevierStylePara elsevierViewall">The setting of the study is limited, which could question its external validity. Initially the participation of other PHCC was considered, it was decided to restrict participation to facilitate accuracy of the measurements and the internal validity of results.<a class="elsevierStyleCrossRef" href="#bib0410"><span class="elsevierStyleSup">41</span></a> Given the characteristics of the study (need for standardization and experience in the determination of the BP to avoid inter-observer variability and the time for completion of field work) it was logistically complex to get other centres to participate. In relation to the above aspect, the size of the study population can be considered small, although it has included virtually the entire population over 80, who met inclusion criteria and accepted participation.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Another limitation of this study is the possible variability in BP measurements and the use of oscillometric devices in the diagnosis of OH. In our study, measures were standardized following the recommendations of Mancia et al.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">17</span></a> and Artigao et al.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">18</span></a> and the research team has extensive clinical and research experience in this area. In addition, the diagnosis of OH by oscillometric equipment is justified by its applicability in clinical practice.</p><p id="par0160" class="elsevierStylePara elsevierViewall">However, given the lower sensitivity and specificity of this method compared with Tilt Test, this test would be recommended in the case of clinical suspicion of OH without significant drop of BP on standing in the consultation<a class="elsevierStyleCrossRef" href="#bib0355"><span class="elsevierStyleSup">30</span></a>. Also worth noting is the possible role of ambulatory BP monitoring (ABPM) if prolonged periods of standing hypotension are suspected.<a class="elsevierStyleCrossRefs" href="#bib0340"><span class="elsevierStyleSup">27,40</span></a> It seems advisable to systematize the detection of OH in octogenarians and especially in diabetic patients with PPs over 60<span class="elsevierStyleHsp" style=""></span>mmHg and smokers. According to the results of our study, OH should be preferably defined depending on the drop in BP measured right at the moment when the patient raises from the supine position (minute 0).</p><p id="par0165" class="elsevierStylePara elsevierViewall">The present work extends the knowledge of potentially modifiable factors related to OH in octogenarians treated in PHCC: promoting physical activity, measuring BP in supine and standing and optimum control of hypertension. En ancianos, la HO puede causar caídas, fracturas y miedo a caerse. Su prevalencia aumenta con la edad, seguramente en relación con el número fármacos prescritos, la disminución de ingesta de líquidos y la disfunción progresiva de su sistema nervioso autónomo.</p></span> <span id="abst0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Objetivos</span><p id="spar0045" class="elsevierStyleSimplePara elsevierViewall">Evaluar la prevalencia de OH en ancianos mayores de 80<span class="elsevierStyleHsp" style=""></span>años no institucionalizados según el criterio diagnóstico de HO en medidas secuenciales tomadas en los minutos 0, 1, 3 y 5 de bipedestación.</p></span> <span id="abst0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">Sujetos y métodos</span><p id="spar0050" class="elsevierStyleSimplePara elsevierViewall">Diseño: estudio de diagnóstico mediante aparato de presión oscilométrico.</p></span> <span id="abst0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Muestra</span><p id="spar0055" class="elsevierStyleSimplePara elsevierViewall">muestreo a conveniencia de 176 pacientes ≥80 años atendidos en un centro de atención primaria urbano.</p></span> <span id="abst0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Medidas</span><p id="spar0060" class="elsevierStyleSimplePara elsevierViewall">Presión Arterial (PA) en supinación y a los minutos 0, 1, 3 y 5 mediante un tensiómetro OMRON 705-CP. Definición de HO: descenso ≥20<span class="elsevierStyleHsp" style=""></span>mmHg en la PA sistólica (PAS) y/o ≥10<span class="elsevierStyleHsp" style=""></span>mmHg en la PA diastólica.</p></span> <span id="abst0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Resultados</span><p id="spar0065" class="elsevierStyleSimplePara elsevierViewall">La media de edad (desviación estándar) fue de 85,2 (3,7) años, y el 60,2% fueron mujeres. Prevalencia de HO: 30,7% al minuto 0; 19,3% al minuto 1; 18,2% al minuto 3; 20,5% al minuto 5.
