Paciente hipertenso joven con mal control de la presión arterial y evidencia de hipopotasemia en el seno de tratamiento farmacológico con tiazidas y carvedilol. Después de suspender el diurético el potasio era de 2,8 mEq/l y en orina de 124 mEq/día.

El estudio del sistema renina-aldosterona mostró (basal): renina, 6,9 ng/ml; aldosterona, 148 ng/dl (cociente: 21,4); con ortostatismo de 2 horas: renina, 5,7 ng/ml, y aldosterona, 237 ng/dl (cociente: 41,5).

Un escáner abdominal demostró una lesión nodular en la glándula suprarrenal izquierda. El estudio confirmó la existencia de un hiperaldosteronismo primario por adenoma suprarrenal izquierdo y se realizó adrenelectomía izquierda. Un mes más tarde la presión arterial era de 110/70 mmHg y el potasio sérico 4,5 mEq/l. Se revisan las causas de hipertensión e hipopotasemia y el protocolo de estudio escalonado de un paciente con dicha patología.

Young hypertensive patient with a poor control of blood pressure and evidence of hypopotasemia in the setting of drug therapy with thiazide compounds and carvedilol. After cessation of the diuretic agent, the potassemia value was 2.8 mEq/l and urine potassium 124 mEq/day.

The study of the renin-aldosterone system showed (basal): renin, 6.9 ng/ml; aldosterone, 148 ng/dl (ratio: 21.4); two-hour orthostatism: renin, 5.7 ng/ml, and aldosterone, 237 ng/dl (ratio: 41.5). An abdominal scanner showed a nodular lesion in the left adrenal gland. The investigation confirmed the existence of primary hyperaldosteronism due to left adrenal adenoma and a left adrenalectomy was performed. One month later, blood pressure was 110/70 mmHg and serum potassium 4.5 mEq/l. Causes of hypertension and hypopotassemia are reviewed as well as a protocol for stepwise investigations of a patient with such condition.
