La transmisión se produce por vía vectorial, a través de insectos triatominos hematófagos, o por vía vertical (madre-hijo), transfusiones sanguíneas, trasplante de órganos o por vía digestiva<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">1</span></a>. Las migraciones humanas han llevado la infección fuera del área de transmisión vectorial, inclusive fuera de Latinoamérica; sin embargo, se considera zona endémica aquella donde persiste la presencia del triatomino. En la eco-región del Chaco, el triatomino <span class="elsevierStyleItalic">Triatoma infestans</span> se ha adaptado a la vivienda humana, encontrando allí alojamiento y alimento<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">1</span></a> y transmitiendo la infección a los humanos y a otros mamíferos. En esta Eco-región coexisten las 2 principales vías de transmisión: vectorial y vertical<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a>. Ciertos factores relacionados con la forma de vida de los humanos pueden actuar facilitando o dificultando su contacto con estos vectores, y con ello, el riesgo de transmisión. Es importante identificar estos factores, tanto aquellos que pueden ser modificados por la población para promover cambios de conducta, como aquellos que no pueden ser modificados, para ser considerados en su asociación con la presencia de la infección en la toma de decisiones.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Se realizó el siguiente trabajo, con el objetivo de poseer un instrumento válido y confiable para la identificación de los riesgos sobre la presencia de EC en comunidades del área endémica de transmisión.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Se elaboró una encuesta sobre factores de riesgo modificables y no modificables de la EC. Se llevó a cabo la evaluación psicométrica mediante validez y confiabilidad del instrumento.</p><p id="par0020" class="elsevierStylePara elsevierViewall">El trabajo de campo se realizó en el distrito Huanqueros, Santa Fe, Argentina. Este distrito tiene 665 habitantes y 205 hogares, distribuidos entre la zona urbana y la rural<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a>.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Se definieron las dimensiones que debía incluir el instrumento de recolección de datos: características sociodemográficas (CSD), factores de riesgo no modificables (FRNM) y factores de riesgo modificables (FRM).</p><p id="par0030" class="elsevierStylePara elsevierViewall">Las preguntas sobre las CSD se orientaron hacia la caracterización de la población encuestada. Dentro de las preguntas sobre los FRNM para contraer la enfermedad se consideraron las vías vectorial, vertical, transfusional y por trasplante de órganos, siendo factores que no pueden o difícilmente se pueden modificar. No se consideró la transmisión digestiva, ya que no fue descrita esta vía en Argentina. Para los FRM se incluyeron los usos y comportamientos que podrían exponer a los encuestados a la presencia del vector en las viviendas, y que estos podrían modificar.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Se realizó una búsqueda bibliográfica de artículos publicados en los buscadores Pubmed y Scopus, utilizando los descriptores <span class="elsevierStyleItalic">Argentina; Chagas Disease; Health Practice; Risk Factors</span>. Se excluyeron los artículos sobre encuestas realizadas en países no endémicos, y los relacionados con los riesgos de tripanosomiasis en animales no humanos.</p><p id="par0040" class="elsevierStylePara elsevierViewall">En función de esta búsqueda se tomaron como guía para la realización de las preguntas los trabajos publicados por Sanmartino y Crocco<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">4</span></a> y Crocco et al.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">5</span></a>.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Se elaboró un breve manual para la cumplimentación de las encuestas, con instrucciones y un glosario para los encuestadores.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Se realizó una prueba piloto de la encuesta en el Distrito Ñanducita, vecino a Huanqueros, y con características socioambientales similares. Para la prueba piloto se visitó al azar 13 viviendas, invitando a participar a una persona mayor de 18 años por vivienda y solicitando su consentimiento para participar en la prueba piloto. Con la prueba piloto se realizaron los ajustes necesarios en relación con su duración y vocabulario utilizado.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Se evaluó la validez de contenido de la encuesta, basándose en la metodología propuesta por Carvajal et al.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">6</span></a>. Se escogió un panel multidisciplinario de 5 expertos, con formaciones de grado en bioquímica, medicina y veterinaria, posgrados en ciencias biológicas, epidemiología y salud pública y con experiencia mayor de 5 años en investigación y gestión de la EC. La invitación se realizó por vía correo electrónico. Se envió la encuesta con su manual para cumplimentarla, y se les solicitó realizar las sugerencias necesarias y valorar cada pregunta en cuanto a su suficiencia, claridad, coherencia y relevancia, mediante una escala de Likert de 1 a 4, siendo 4 el valor máximo: 1. no cumple con el criterio; 2. bajo nivel; 3. moderado nivel; y 4. alto nivel.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Para el análisis de la consistencia interna se consideró lo propuesto por Streiner<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">7</span></a>. Se aplicó el instrumento a 67 habitantes de las viviendas urbanas y rurales de Huanqueros, entre marzo y noviembre de 2019. Se realizaron visitas domiciliarias al azar a una de cada 4 viviendas de la zona urbana y a la totalidad de las viviendas de la zona rural.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Como criterios de inclusión se consideró a una persona mayor de 18 años de edad por vivienda, que acepte participar en la encuesta en el momento de la visita y que pertenezca a la comunidad de Huanqueros. Todas las encuestas fueron realizadas por alguno de los integrantes del equipo.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Los datos obtenidos se cargaron en una base de datos en Excel y se analizaron con el software Minitab Statistical y Epi Info 7.2.5.0.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Con las respuestas del panel de expertos se evaluó la validez de contenido del instrumento mediante el coeficiente de concordancia externa W de Kendall: cuando W tiende a 0 existe ausencia de concordancia en la evaluación emitida por los expertos; si W se aproxima a 1 existe unidad de concordancia, lo que otorga validez al instrumento para ser utilizado para los fines para los cuales fue diseñado. Se calculó el W de Kendal para la encuesta en general y para suficiencia, claridad, coherencia y relevancia.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Para el análisis de la consistencia interna de la encuesta se calculó el estadístico alfa de Cronbach, que permite estimar la fiabilidad del instrumento para evaluar una misma dimensión<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">6</span></a>. Se realizó el cálculo para cada una de las dimensiones y para la encuesta total. Para su interpretación se consideró mayor a 0,9 excelente, de 0,8 a 0,9 bueno, de 0,7 a 0,8 aceptable, de 0,6 a 0,7 cuestionable, de 0,5 a 0,6 pobre y menor de 0,5 inaceptable.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Se solicitó la firma del formulario de consentimiento informado a los participantes. El proyecto fue aprobado por el Comité asesor de ética y seguridad biológica de la Facultad de Bioquímica y Ciencias Biológicas, Universidad Nacional del Litoral (Acta 04 6).</p><p id="par0090" class="elsevierStylePara elsevierViewall">La encuesta inicial quedó conformada por 24 preguntas: 5 sobre CSD (edad, sexo, zona rural o urbana, escolaridad y ocupación), 10 sobre FRNM (antecedentes familiares, material de la vivienda actual y anterior, evaluación de la vivienda por técnicos de control vectorial, transfusiones, trasplantes…), 8 sobre FRM (se hizo análisis de Chagas, reconoce al vector, presencia de animales o escondrijos para el vector en la vivienda…), más una pregunta sobre si escuchó hablar de Chagas. De estas 19 fueron de respuesta dicotómica, 3 fueron nominales, una cuantitativa discreta y una ordinal. La duración de la encuesta fue de aproximadamente de 20 minutos.</p><p id="par0095" class="elsevierStylePara elsevierViewall">A estas preguntas se le agregaron 3 ítems para recopilar de los registros de evaluación vectorial de los técnicos de la Coordinación Nacional de Control de Vectores. Se realizaron los cambios sugeridos por los expertos.</p><p id="par0100" class="elsevierStylePara elsevierViewall">La evaluación de las preguntas por los expertos, sobre un puntaje máximo de 4 puntos, presentó un puntaje promedio de 3,68, siendo 3,61 para el ítem suficiencia, 3,80 para claridad, 3,65 para coherencia y 3,65 para relevancia. Se obtuvo un nivel de concordancia en la evaluación de todos los ítems de 0,736, siendo 0,661 para suficiencia, 0,832 para claridad, 0,764 para coherencia y 0,689 para relevancia.</p><p id="par0105" class="elsevierStylePara elsevierViewall">Se obtuvo un alfa de Cronbach de 0,96 para la dimensión FRNM, de 0,99 para FRM y de 0,88 para la encuesta total. Para la encuesta total la consistencia puede ser considerada buena (entre 0,8 y 0,9) y para cada una de las dimensiones excelente (mayor a 0,9).</p><p id="par0110" class="elsevierStylePara elsevierViewall">El proceso de elaboración y validación de la encuesta del presente trabajo permite obtener información sobre riesgos modificables y no modificables de enfermedad de Chagas en poblaciones endémicas, en función de la percepción de los habitantes de esta región, con un alto grado de confianza. Esta encuesta permite identificar factores sobre los que se puede trabajar en la prevención, tanto por parte de las comunidades afectadas como por las autoridades sanitarias. Los resultados obtenidos en cuanto a la fiabilidad del instrumento, basados en el juicio de expertos, mostraron una muy buena valoración de la encuesta de forma general, y en el análisis de la suficiencia, claridad, coherencia y relevancia de las preguntas, con un grado de concordancia alto en sus evaluaciones de acuerdo con el coeficiente W de Kendall. Por otro lado, el instrumento mostró una excelente consistencia interna para las 2 dimensiones incluidas, basándonos en los resultados de alfa de Cronbach.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Otros trabajos han utilizado encuestas para evaluar riesgos epidemiológicos de EC en la región<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">5,8</span></a>. Sin embargo, pocos artículos han utilizado técnicas psicométricas para la validación del instrumento<a class="elsevierStyleCrossRefs" href="#bib0095"><span class="elsevierStyleSup">9,10</span></a>. Estos procesos de validación brindan confiabilidad a los instrumentos utilizados para la indagación de variables basadas en la percepción de los entrevistados y se hacen necesarios toda vez que sean empleados. Debe tenerse en cuenta que la encuesta validada en el presente trabajo es aplicable en poblaciones similares a la utilizada en este artículo.
