Las manifestaciones cardiovasculares de la TF no suelen ser graves aunque pueden llegar a serlo<a class="elsevierStyleCrossRef" href="#bib1"><span class="elsevierStyleSup">1</span></a>, por lo que nos ha parecido de interés presentar el caso de una paciente con cardiopatía tirotóxica reversible secundaria a TF.</p><p class="elsevierStylePara elsevierViewall">Mujer de 63 años que acudió a nuestro hospital en el año 2000 por nerviosismo e insomnio en relación con el tratamiento con Triacana<span class="elsevierStyleSup">®</span> (Triac o 3,5,3’ ácido triyodotiroacético) (1,05<span class="elsevierStyleHsp" style=""></span>mg/día), un metabolito de triyodotironina (T<span class="elsevierStyleInf">3</span>) utilizado como fármaco dietético. En la analítica destacaba tirotropina (TSH) inferior a 0,1<span class="elsevierStyleHsp" style=""></span>μU/ml (valores normales [VN]: de 0,5 a 4<span class="elsevierStyleHsp" style=""></span>μU/ml); tiroxina (T<span class="elsevierStyleInf">4</span>) total de 1,4<span class="elsevierStyleHsp" style=""></span>μg/dl (VN: de 4,5 a 12,5<span class="elsevierStyleHsp" style=""></span>μg/dl) y T<span class="elsevierStyleInf">3</span> de 69<span class="elsevierStyleHsp" style=""></span>μg/l (VN: de 0,7 a 2<span class="elsevierStyleHsp" style=""></span>μg/l), por lo que se le diagnosticó TF secundaria al uso de Triacana<span class="elsevierStyleSup">®</span>, y sus síntomas desaparecieron tras la retirada del fármaco.</p><p class="elsevierStylePara elsevierViewall">En 2007, en otro centro, se diagnosticó a la paciente de un trastorno dismórfico corporal debido a una pérdida de peso asociada al abuso crónico de Levothroid<span class="elsevierStyleSup">®</span> (levotiroxina) (500<span class="elsevierStyleHsp" style=""></span>μg/día) y furosemida (80<span class="elsevierStyleHsp" style=""></span>mg/día). Precisó un ingreso involuntario donde se determinó una TSH inferior a 0,1<span class="elsevierStyleHsp" style=""></span>μU/ml, T<span class="elsevierStyleInf">4</span> total de 15<span class="elsevierStyleHsp" style=""></span>μg/dl y T<span class="elsevierStyleInf">3</span> de 3,1<span class="elsevierStyleHsp" style=""></span>μg/l; aunque tras una mejoría inicial, la paciente mantuvo el abuso crónico de fármacos.</p><p class="elsevierStylePara elsevierViewall">En 2008 acudió a nuestro hospital por disnea progresiva, tos y edemas en extremidades inferiores tras la retirada de la medicación durante la semana previa debido a un cuadro autolimitado de vómitos. A su ingreso, la presión arterial era de 120/70<span class="elsevierStyleHsp" style=""></span>mmHg, la frecuencia cardíaca era de 110 latidos por minuto, la frecuencia respiratoria era de 32 respiraciones por minuto y la temperatura era de 35,6<span class="elsevierStyleHsp" style=""></span>°C. No se encontraron exoftalmos ni bocio, aunque destacaba una auscultación pulmonar con crepitantes bibasales y edemas en las extremidades inferiores. En el electrocardiograma (ECG) al ingreso se observó una taquicardia sinusal a 110 latidos por minuto con ondas T negativas persistentes en las derivaciones V<span class="elsevierStyleInf">3</span>-V<span class="elsevierStyleInf">6</span>. En la radiografía de tórax se observó un derrame pleural bilateral e infiltrados alveolares bibasales, mientras que la toracocentesis mostró un trasudado. En la analítica destacaba una velocidad de sedimentación globular de 15<span class="elsevierStyleHsp" style=""></span>mm/h, sodio de 130<span class="elsevierStyleHsp" style=""></span>meq/l, potasio de 3,4<span class="elsevierStyleHsp" style=""></span>meq/l, TSH inferior a 0,1<span class="elsevierStyleHsp" style=""></span>μU/ml, T<span class="elsevierStyleInf">4</span> total de 13,2<span class="elsevierStyleHsp" style=""></span>μg/dl, T<span class="elsevierStyleInf">3</span> de 2,1<span class="elsevierStyleHsp" style=""></span>μg/l, anticuerpos antitiroideos negativos (incluidos anticuerpos antimicrosómicos y antitiroglobulina) y tiroglobulina de 0,1<span class="elsevierStyleHsp" style=""></span>ng/ml (VN: de 0,2 a 70<span class="elsevierStyleHsp" style=""></span>ng/ml). La gammagrafía tiroidea con <span class="elsevierStyleSup">99m</span>Tc constató la ausencia completa de captación isotópica de la glándula. Se realizó un ecocardiograma que mostró una leve disfunción sistólica del ventrículo izquierdo (VI) con una fracción de eyección del VI (FEVI) del 48% (VN: superiores al 55%), moderada disfunción diastólica del VI y moderada hipertensión arterial pulmonar con una presión arterial pulmonar de 50<span class="elsevierStyleHsp" style=""></span>mmHg (VN: inferiores a 25<span class="elsevierStyleHsp" style=""></span>mmHg). La coronariografía no mostró lesiones significativas en las arterias coronarias y la ventriculografía mostró una presión telediastólica del VI de 22<span class="elsevierStyleHsp" style=""></span>mmHg (VN: de 5 a 9<span class="elsevierStyleHsp" style=""></span>mmHg).</p><p class="elsevierStylePara elsevierViewall">Se diagnosticó a la paciente de insuficiencia cardíaca congestiva en relación con una cardiopatía tirotóxica secundaria a TF y presentó una buena evolución con tratamiento diurético y bloqueadores β. Seis meses después del alta hospitalaria la paciente no presentaba signos de insuficiencia cardíaca, el ECG mostraba un ritmo sinusal a 65 latidos por minuto con normalización de las ondas T negativas en precordiales, y el estudio ecocardiográfico y las HT plasmáticas eran normales.</p><p class="elsevierStylePara elsevierViewall">Aunque la TF puede deberse a la ingesta accidental de HT<a class="elsevierStyleCrossRef" href="#bib2"><span class="elsevierStyleSup">2</span></a>, la mayoría de los casos corresponden a pacientes con una enfermedad psiquiátrica subyacente o a obesos en tratamiento con fármacos dietéticos<a class="elsevierStyleCrossRef" href="#bib3"><span class="elsevierStyleSup">3</span></a>.</p><p class="elsevierStylePara elsevierViewall">El hipertiroidismo produce múltiples alteraciones en el sistema cardiovascular que pueden causar insuficiencia cardíaca y que son potencialmente reversibles<a class="elsevierStyleCrossRef" href="#bib4"><span class="elsevierStyleSup">4</span></a>. Globalmente, consisten en un estado hiperdinámico con aumento de la FEVI en reposo y reducción de la FEVI durante el esfuerzo, hipertrofia del VI, taquiarritmias supraventriculares (la más frecuente es la fibrilación auricular), disminución de las resistencias vasculares sistémicas e hipertensión arterial pulmonar. La insuficiencia cardíaca congestiva es la forma de presentación clínica inicial en el 6% de los hipertiroideos de cualquier etiología y la mitad de los casos los pacientes tienen disfunción del VI. Estos casos se asocian a la presencia de fibrilación auricular, edad avanzada y cardiopatía preexistente, y en un tercio de los casos se desarrolla una miocardiopatía dilatada persistente<a class="elsevierStyleCrossRef" href="#bib5"><span class="elsevierStyleSup">5</span></a>. En nuestra paciente, el factor desencadenante de la cardiopatía tirotóxica fue la interrupción de la ingesta subrepticia de diuréticos.</p><p class="elsevierStylePara elsevierViewall">Como regla general, el tratamiento terapéutico de la cardiopatía tirotóxica debería iniciarse con diuréticos de asa para reducir la sobrecarga de volumen, mientras que los vasodilatadores como los nitratos no estarían recomendados debido a la disminución de las resistencias sistémicas, secundaria al hipertiroidismo. Los bloqueadores β reducen los síntomas de tirotoxicosis y controlan la frecuencia cardíaca, aunque pueden reducir la contractilidad cardíaca y deben iniciarse con precaución<a class="elsevierStyleCrossRef" href="#bib6"><span class="elsevierStyleSup">6</span></a>.</p><p class="elsevierStylePara elsevierViewall">En el caso presentado, después de descartarse otras etiologías causantes de cardiopatía y dada la normalización ecocardiográfica y del perfil tiroideo tras la suspensión del abuso de HT y con el tratamiento administrado, se orientó el cuadro como una cardiopatía tirotóxica reversible secundaria a TF en una paciente con un trastorno somatomorfo.</p><p class="elsevierStylePara elsevierViewall">Este caso es un ejemplo más de que la TF es una causa reversible de insuficiencia cardíaca y que el clínico debería sospechar una ingesta subrepticia de HT exógena en pacientes con un trastorno psiquiátrico subyacente o en obesos en tratamiento con fármacos dietéticos, incluso en pacientes ancianos<a class="elsevierStyleCrossRef" href="#bib7"><span class="elsevierStyleSup">7</span></a>.</p></span>" "pdfFichero" => "main.pdf" "tienePdf" => true "bibliografia" => array:2 [ "titulo" => "Bibliografía" "seccion" => array:1 [ 0 => array:1 [ "bibliografiaReferencia" => array:7 [ 0 => array:3 [ "identificador" => "bib1" "etiqueta" => "1" "referencia" => array:1 [ 0 => array:2 [ "contribucion" => array:1 [ 0 => array:2 [ "titulo" => "Thyroid disease and the heart" "autores" => array:1 [ 0 => array:2 [ "etal" => false "autores" => array:2 [ 0 => "I.
