Philadelphia: Elsevier Saunders; 2006.</span></p><p class="elsevierStylePara">En el transcurso de la última década se ha hecho patente que el aumento de la expectativa de vida ha cambiado las necesidades de atención de los pacientes en general, que presentan más procesos comórbidos y más problemas funcionales, psicosociales y mentales. Esa población de pacientes vulnerables, que conviven con enfermedades crónicas y disfunciones que se van acumulando, son objeto de una atención fragmentada y errática, que se está demostrando clínicamente ineficaz y económicamente ineficiente. En la actualidad, los pacientes con enfermedades múltiples y contexto psicosocial adverso (es decir, los pacientes <span class="elsevierStyleItalic">complejos</span>) están pasando a ser la norma en lugar de la excepción, así que los sistemas de atención sanitaria tendrán que dar una rápida respuesta a esa progresiva transformación de sus usuarios.</p><p class="elsevierStylePara">En este libro se compilan una docena de estudios sobre distintas enfermedades, en los que se proponen programas integrales de atención terapéutica. Estos estudios se han publicado en <span class="elsevierStyleItalic">The Medical Clinics of North America,</span> y analizan el tratamiento integral de distintas enfermedades, a partir del modelo biopsicosocial propuesto por Engel en 1960. Este modelo, totalmente compatible con el sentido común, permite evaluar la <span class="elsevierStyleItalic">complejidad</span> de cada paciente por el número de sistemas orgánicos que tiene afectados, por su estado psicológico y psiquiátrico y por la naturaleza del ecosistema (social) en que vive. Por lo tanto, la organización de los planes terapéuticos integrales dependerá del diagnóstico biopsicosocial de cada paciente, y el tipo de intervención (médica, psicológico/psiquiátrica o social) deberá articularse sobre los datos procedentes de la medicina y de la psiquiatría basada en pruebas y sobre los estudios de coste/efectividad. Son requisitos muy exigentes, de difícil cumplimiento en la actual organización de la atención sanitaria.</p><p class="elsevierStylePara">En el capítulo destinado a evaluar la eficacia de las intervenciones integrales (Smith y Clark<span class="elsevierStyleItalic">)</span> se ilustra esa dificultad para la demostración empírica de lo que parece clínicamente obvio. Recurriendo a la casuística, que siempre ha sido el procedimiento favorito de la medicina, sabemos que hay pacientes con varios problemas que son tratados por varios médicos, a veces de diferentes instituciones, y que las incidencias en cada subsistema orgánico vulnerable provocan desajustes en todos los demás. Es de sentido común que la atención fragmentada e inconexa ha de ser menos eficiente que la intervención coordinada de todos los profesionales, y que la asistencia centrada en las necesidades del paciente generará mejores resultados en general (clínicos, económicos y de satisfacción del usuario) que la asistencia secuenciada azarosamente. Sin embargo, la medicina basada en pruebas dice que la eficiencia de los tratamientos integrales no ha podido demostrarse satisfactoriamente en todos los casos, lo que nos informa de las dificultades metodológicas de este ámbito de investigación clínica. Este capítulo introductorio, con estructura de glosario, es muy interesante y muy informativo conceptualmente, y merecería ser conocido por todos los profesionales de la medicina.</p><p class="elsevierStylePara">Lo que sí queda probado en el capítulo sobre los costes de la asistencia fragmentada (Kathol, Saravay, Lobo y Ornel) es que entre el 10 y el 30% de los pacientes con trastornos conductuales no tienen fácil acceso a la atención psiquiátrica o no reciben la atención médica que necesitan, lo que redunda en una peor evolución clínica y funcional, en una mayor insatisfacción por la atención recibida y en superiores costes económicos. No obstante, el desacuerdo vuelve a aparecer cuando se quiere establecer comparaciones entre distintos sistemas de financiación sanitaria, aunque prevalece la hipótesis central de que la atención integrada forzosamente ha de ser más eficiente que la solución atomizada de problemas. Probablemente tenga sentido salvar la distinción entre ayuda médica y social en las cláusulas privadas y públicas de atención al bienestar del ciudadano, y establecer estimaciones de costes asimismo integrales.</p><p class="elsevierStylePara">El capítulo dedicado al tratamiento de los pacientes con síntomas somáticos sin explicación y comorbilidad médico-psiquiátrica (Kroenke y Rosmalen) se inicia con la estimación de nuestra ignorancia etiopatogénica de casi la mitad de todas las molestias físicas que presentan los pacientes, y de la existencia de una amplia población de pacientes médicos que presentan psicopatología asociada, que complica o condiciona el diagnóstico, el tratamiento y la evolución de su enfermedad orgánica. Estos pacientes acuden con más frecuencia a los dispositivos de salud, consumen recursos infructuosamente y tienen una peor calidad de vida en general. Así lo demuestran los capítulos sobre el anciano frágil (Slaets), sobre los pacientes con depresión asociada al síndrome metabólico y a la enfermedad cardiovascular (Gans), y sobre los pacientes con depresión asociada a la diabetes (Egede).</p><p class="elsevierStylePara">Por último, merecen ser destacados los capítulos que proponen modelos de cuidados integrales (Wulsin, Sollner y Pincus) y de tratamiento de pacientes médicos complejos (de Jorge, Huyse y Stiefel) y el capítulo dedicado a INTERMED (Stiefel, Huyse, Sollner et al), que es un instrumento clínico de aplicación individualizada para evaluar la complejidad psicosocial de los pacientes y diseñar estrategias multidisciplinarias de intervención. Estos sistemas de recogida de datos (pensados para solucionar problemas concretos de salud) difieren de la anamnesis médica tradicional (que está pensada para encontrar indicios que permitan un diagnóstico etipatogénico) y obligan a un notable cambio de hábitos en la práctica médica. El médico actual se está viendo obligado a sustituir <span class="elsevierStyleItalic"> su</span> historia clínica en papel por historiales médicos unificados e informatizados (es decir, la historia clínica ha pasado a ser <span class="elsevierStyleItalic">del</span> paciente), y en ella ha de habituarse a registrar, razonar y decidir en un lenguaje inteligible para el siguiente facultativo que intervenga en el proceso de atención. Además, la secuenciación de intervenciones tendría que transformarse en atención unificada, de manera que de las decisiones médicas multidisciplinarias se derive un plan de acción que incluya medidas psicosociales y programas de atención de enfermería. Todo eso supone para los médicos cambios de mentalidad y de herramientas, y también cambios en la relación con los pacientes, que recobran su voz para reclamar un trato competente y atento.
