In 1974 King and Mills performed the first percutaneous closure of ASD,<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">2</span></a> currently being the technique of choice whenever the anatomy is favorable.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">It is currently known that percutaneous closure of these defects is a safe technique, with a low rate of events in the short and medium term, although the large registries do not usually have a follow-up of more than 3 years.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">4,5</span></a> Our objective is to analyse progression and long-term incidents after the percutaneous closure of ASD in a series of patients referred to our site.</p><p id="par0015" class="elsevierStylePara elsevierViewall">We designed an observational, retrospective study based on the collection of clinical and echocardiographic data through the review of the clinical history of patients undergoing percutaneous closure of ASD or PFO, in which the following variables were analysed: total mortality, cardiovascular mortality, device thrombosis, stroke or transient ischaemic accident (TIA), new onset pericardial effusion, cardiac tamponade, arrhythmias and arrhythmia type (ventricular, atrial fibrillation [AF], <span class="elsevierStyleItalic">flutter</span> and others), peripheral embolism, infectious endocarditis (IE) and the need for a definitive pacemaker (PM) implant.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Between October/1999 and February/2010, 100 patients were referred to our centre to assess closure of ASD; 7 patients were finally ruled out for percutaneous closure. The final sample is made up of 93 patients (83 ASD and 10 PFO); 51% are paediatric patients (44 ASD and 2 PFO). All defects were closed with Amplatzer® type devices, with 100% success of the implant and a 90% rate of residual <span class="elsevierStyleItalic">shunt</span> absence at 12 months. A total of 88% of the patients were discharged 24<span class="elsevierStyleHsp" style=""></span>h after the implant.</p><p id="par0025" class="elsevierStylePara elsevierViewall">A follow-up of 13.2<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>8.2 years was performed (range: 8–18.3 years). Fourteen patients (15%) possibly had procedure-related events, with a total of 18 documented events. During follow-up, no case of thrombosis, erosion, IE, PM implantation, ventricular arrhythmias, or cardiac tamponade was found. <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> shows the events during follow-up.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0030" class="elsevierStylePara elsevierViewall">The most frequently observed event in our series was the development of supraventricular arrhythmias, with a total of 8 patients (8.6%, divided between 5 cases of AF and 3 of <span class="elsevierStyleItalic">flutter</span>), 3 of these episodes occurring during the first 24<span class="elsevierStyleHsp" style=""></span>h after implantation (range: 0 days to 11.2 years).</p><p id="par0035" class="elsevierStylePara elsevierViewall">In our follow-up, there were a total of 5 deaths (5.3%), with no apparent relationship with the heart disease or the intervention performed (4 of them were patients with multiple pathologies over the age of 75 years at implantation). Among the neurological events, we found 2 cases of stroke (2.1%, one of them due to arteriovenous malformation) and one episode of TIA (1.1%). Other recorded events were a case of uncomplicated pericardial effusion (1.1%) and a case of peripheral arterial embolism (1.1%, patient with previous paroxysmal AF).</p><p id="par0040" class="elsevierStylePara elsevierViewall">If we break down the patients according to heart disease, 15 events occurred in the group of patients with ASD (12/83 patients, 12.9% total) and 3 in the group with PFO (2/10 patients, 2.1% total).</p><p id="par0045" class="elsevierStylePara elsevierViewall">If the patients are divided according to age, in the group of pediatric patients, which account for 50% of our series, we only found one possible event, and that was the occurrence of uncomplicated pericardial effusion after closure of the PFO. The rest of events occurred in the group of adult patients, especially in those over 65 years of age: 7 events in 40 patients between the ages of 14 and 64 and 10 events in 7 patients over 65.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Finally, based on the time elapsed after percutaneous closure, we found 3 events during the first 30 days (all of them supraventricular arrhythmias) and one supraventricular arrhythmia between 30 days and 3 years; the rest of the events occurred 3 years after the procedure.</p><p id="par0055" class="elsevierStylePara elsevierViewall">Percutaneous closure of ASD is safe, with a low rate of long-term complications, most of them benign (supraventricular arrhythmias), with the age group above 65 years being the most at risk, especially after 3 years of follow-up.</p></span>" "pdfFichero" => "main.pdf" "tienePdf" => true "NotaPie" => array:1 [ 0 => array:2 [ "etiqueta" => "☆" "nota" => "<p class="elsevierStyleNotepara" id="npar0005">Please cite this article as: Lapeña-Reguero M, Rueda-Nuñez F, Álvarez-García N.
