El objetivo de este editorial es proporcionar información sobre cómo actúan estos nuevos fármacos para establecer cuál es el seguimiento clínico adecuado a lo largo de uso. Todo ello, permitirá identificar de forma precoz los posibles efectos adversos derivados y el correcto manejo de los mismos<a class="elsevierStyleCrossRefs" href="#bib0105"><span class="elsevierStyleSup">1–4</span></a>.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Ahora sabemos que el sistema inmune desempeña un papel fundamental no solo en el control de enfermedades infecciosas bacterianas, víricas o parasitarias, sino que se comporta como un actor principal en la evolución y el control de la enfermedad tumoral. Además, conocemos que su papel es importante en todas las fases de la enfermedad, tanto en su parte inicial como en los estadios más avanzados. El sistema inmune es capaz de reconocer las células tumorales en su desarrollo e impedir la proliferación y supervivencia de las mismas. Desgraciadamente, este mecanismo no siempre funciona a la perfección, por fallos en la presentación antigénica o por el desarrollo de mecanismos de evasión inmune tumoral. Estos mecanismos permiten a las células neoplásicas escapar del control del sistema inmune y, por tanto, proliferar y desarrollarse.</p><p id="par0015" class="elsevierStylePara elsevierViewall">La activación linfocitaria es un proceso extremadamente complejo que necesita una perfecta orquestación del sistema inmune. Así pues, para la activación del linfocito T CD8+, el efector final de la actividad citotóxica, se requieren una serie de mecanismos. Primero la presentación del antígeno por parte de la célula presentadora de antígeno hacia el receptor del linfocito T. Esta sinapsis linfocitaria es necesaria, pero no es suficiente para la activación linfocitaria final. Es necesaria una coestimulación entre el CD28 del linfocito T y el B7/B7.1 de la célula presentadora de antígeno<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">5</span></a>. En este momento, el linfocito T citotóxico es activado y puede ejercer su función, reconociendo el antígeno contra el que ha sido activado y, por tanto, actuar de forma consecuente contra su entorno. Pero el sistema inmune ha generado mecanismos para modular la respuesta efectora de los linfocitos T citotóxicos para evitar su sobreactivación con posibles efectos de autoinmunidad deletéreos. En ese sentido el propio linfocito T CD8+, al ser activado, comienza a expresar en su superficie proteínas inhibitorias como CTLA-4. Esta última tiene como misión competir con CD28 en su unión con B7 de cara a inactivar al linfocito T, modulando así su actividad citotóxica y actuando por tanto como un autorregulador de la capacidad citotóxica del sistema inmune<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">6</span></a>. Es por ello por lo que el primer fármaco inmunoterápico de la era moderna fue desarrollado para bloquear CTLA-4 impidiendo su competencia con B7, dificultando así la inactivación del linfocito T citotóxico. Este fármaco es ipilimumab, que demostró eficacia inicialmente en melanoma y más adelante en otros tumores sólidos<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">1</span></a>.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Posteriormente hemos identificado otros puntos de control que regulan la activación/inactivación de dichos linfocitos T CD8+. Entre ellos destacan inicialmente los inhibidores de PD-1 y posteriormente PD-L1. Al igual que ocurre con CTLA-4, el linfocito T activado coexpresa en su superficie PD-1 fundamentalmente. Su ligando PD-L1 se expresa de forma mayoritaria en células propias de sistema inmune pero también en células tumorales. La unión de PD-1 y PD-L1 desencadena una respuesta inhibitoria en el linfocito, conduciendo a su inactivación. Este mecanismo de expresión de PD-L1 por parte del tumor obedece a una forma de enmascaramiento y por tanto de evasión tumoral de la respuesta y control inmune<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">7</span></a>.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Estos fármacos contra PD-1 (nivolumab o pembrolizumab) y PD-L1 (durvalumab, atezolizumab, avelumab entre otros) han demostrado incluso mayor eficacia que los inhibidores de CTLA-4 con un perfil de toxicidad más manejable. En el momento actual la combinación de ambos mecanismos de acción contra PD-1 y CTLA-4 ya ha demostrado mayor eficacia que los tratamientos estándar de varios tumores metastásicos entre los que destacan el melanoma y el cáncer renal<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">8,9</span></a>.</p><p id="par0030" class="elsevierStylePara elsevierViewall">La intervención farmacológica sobre el sistema inmune no es algo nuevo en oncología. Hace varias décadas se utilizaba, por ejemplo, la interleucina 2 como activador del sistema inmune con casos publicados de respuestas completas en pacientes con cáncer renal avanzado. También se han utilizado otras interleucinas con el interferón en melanoma. Ambas estrategias han caído en desuso en la actualidad por 2 motivos principales. Por un lado, su perfil de toxicidad, de difícil manejo para los resultados demostrados. Por otro lado, la llegada de inhibidores de tirosina cinasa y posteriormente los nuevos fármacos inmunoterápicos, más efectivos y con mejor perfil de tolerancia.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Actualmente tenemos aprobados para su uso en nuestro país inhibidores de CTLA-4 y fármacos contra el eje PD-1/PD-L1. Pero el desarrollo de la inmunoterapia, lejos de estancarse, continúa a día de hoy. Conocemos que hay más puntos de control en la activación o desactivación del linfocito citotóxico, por lo que se están desarrollando fármacos encaminados a mantener la activación del linfocito actuando contra proteínas como LAG-3, TIM-3 o incluso OX-40 entre otros<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">10</span></a>.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Por tanto, con la inmunoterapia hemos logrado ser capaces de revertir esa capacidad de generar tolerancia inmune en los linfocitos por parte del tumor, no permitiendo la actuación de los mecanismos de inactivación linfocitaria.</p><p id="par0045" class="elsevierStylePara elsevierViewall">Y aunque estas estrategias son tremendamente ingeniosas y efectivas, la visión simplista del sistema inmune con un único representante fundamental en el linfocito T, no es sino una quimera. El sistema inmune se compone de una gran cantidad de elementos intensamente relacionados para poder modular y dar respuesta a cualquier contingencia que ocurra en el ser humano. Es más, el microambiente tumoral, constituido por células del sistema inmune innato, es clave para la repuesta del adaptativo, ya que es capaz de modularla. Es por ello por lo que no todos los pacientes responden al tratamiento con inmunoterapia, debido a la enorme complejidad de la relación de todo el sistema inmune con su microambiente y con la enfermedad tumoral<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">11</span></a>.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Todos los pacientes en tratamiento inmunoterápico, independientemente de cual sea, deben ser informados de los posibles efectos adversos derivados del fármaco en cuestión, realizando una vigilancia activa de los mismos por parte médica. Esta estrategia de información a los pacientes se recomienda sea por escrito, explicando, punto por punto, las posibles toxicidades y la manera de obrar ante cada una de ellas. La implicación del paciente en su propia vigilancia clínica puede ayudar a mejorar la adherencia al tratamiento, permitiendo la identificación precoz de los efectos adversos. Dichos efectos adversos inmunomediados (EAI) derivan de la sobreactivación del linfocito T que ejerce su efecto citotóxico sobre las células tumorales, pero también sobre los tejidos sanos, pudiendo provocar inflamación sistémica y, por tanto, de cualquier órgano.</p><p id="par0055" class="elsevierStylePara elsevierViewall">No todos los fármacos inmunoterápicos poseen la misma frecuencia en la aparición de EAI. La frecuencia de aparición es mayor con los inhibidores de CTLA-4 que en los fármacos contra PD-1/PD-L1 de forma general. Además, las combinaciones de ambos también suelen presentar mayor frecuencia de EAI que el uso en monoterapia de inhibidores de CTLA-4 o PD-1/PD-L1<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">12</span></a>.</p><p id="par0060" class="elsevierStylePara elsevierViewall">Los EAI se clasifican según su gravedad, según los criterios «<span class="elsevierStyleItalic">Common Terminology Criteria for Adverse Events</span> (CTCAE)»’ en 5 categorías, siendo 1 la sintomatología más leve y 5 la muerte secundaria a dicho efecto adverso. Como ejemplo, los efectos adversos de grado 3-4 (considerados más graves y que requieren la hospitalización del paciente) se presentan en un 59% en los pacientes tratados con la combinación de inhibidores de CTLA-4 y PD-1, en un 28% con inhibidores CTLA4 y en un 21% con fármacos anti PD-1<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">13</span></a>.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Conocemos también que no todos los EAI tienen la misma incidencia, siendo los más frecuentes las alteraciones cutáneas en forma de dermatitis e incluso las endocrinopatías en forma de tiroiditis. Ambas, además de ser las más frecuentes suelen ser las que antes aparecen, en las primeras 12 semanas de tratamiento y suelen ser leves, de grados 1 y 2 de fácil manejo con tratamiento tópico o reposición tiroidea con levotiroxina. Se recomienda vigilar al menos una vez al mes la función tiroidea de los pacientes en tratamiento.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Otra de las toxicidades relativamente frecuente y de pronta aparición suele ser la digestiva, habitualmente en forma de colitis. En este caso y dependiendo de la afectación del paciente puede ser necesario el uso de glucocorticoides de forma precoz por la posible gravedad en caso de colitis. Menos frecuentes y más tardíos en su aparición suelen ser la afectación hepática, pulmonar, renal, cardíaca, neurológica, oftalmológica, o endocrinopatías menos frecuentes como insuficiencia suprarrenal o hipofisitis. Todas ellas, y dado que los sistemas que afectan suelen ser de mayor gravedad, requieren dosis elevadas de glucocorticoides y la hospitalización y monitorización del paciente<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">14,15</span></a>.</p><p id="par0075" class="elsevierStylePara elsevierViewall">De forma general los EAI menos graves de grados 1-2 no suelen interferir en la vida de los pacientes y con tratamiento de soporte suelen ser controlados. Destacar que aquellas toxicidades moderadamente sintomáticas, de grado 2, mantenidas en el tiempo, pueden requerir tratamiento con glucocorticoides, por ejemplo, metilprednisolona 0,5-1<span class="elsevierStyleHsp" style=""></span>mg/kg/día por vía oral para su resolución. Estas toxicidades mantenidas o aquellas que sean más graves, grado 3-4 (más de 6 deposiciones diarias, elevación de GOT/GPT por encima de 5 veces el límite superior de la normalidad, sintomatología respiratoria, deterioro de la función renal o alteraciones neurológicas, entre otras) requieren la hospitalización del paciente o al menos la valoración de los especialistas implicados, desde oncología médica hasta la propia de la afección principal que presente el paciente antes de decidir los siguientes pasos. En estos casos está indicada la utilización de glucocorticoides a altas dosis por vía intravenosa 1-2<span class="elsevierStyleHsp" style=""></span>mg/kg/día. Si los síntomas son refractarios más de 48<span class="elsevierStyleHsp" style=""></span>h o no existe mejoría a pesar del uso de glucocorticoides, en casos seleccionados y siempre de forma multidisciplinar, puede estar indicado el uso de otros inmunosupresores como en micofenolato, infliximab e incluso inmunoglobulinas en algunos EAI de diversa índole. Como norma general debemos plantearnos la suspensión del tratamiento con inmunoterapia ante toxicidades grado 3 o superior. En algunos casos, como en las hepatitis o tiroiditis, podemos plantear la reintroducción de la inmunoterapia tras la resolución de EAI a grado <span class="elsevierStyleSmallCaps">i</span> o inferior. En otras ocasiones como la neumonitis no está indicada su reintroducción. Debemos individualizar, en función del paciente y tipo de EAI, la suspensión definitiva o la reintroducción de la inmunoterapia<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">14–18</span></a>.</p><p id="par0080" class="elsevierStylePara elsevierViewall">Cada vez disponemos de mayor experiencia de uso y mayor seguimiento de pacientes tratados con inmunoterapia y el empleo de glucocorticoides de forma puntual no parece impactar en un detrimento en la supervivencia de estos pacientes por su potencial efecto inmunosupresor. Lo que debemos tener en cuenta es que el descenso de la dosis de los glucocorticoides tras un EAI debe ser gradual en la mayoría de los casos para evitar efectos rebote. El uso de esteroides durante más de 6 semanas se ha relacionado con enfermedades oportunistas, así como mayor incidencia de osteoporosis. Es por ello por lo que en su uso prolongado debemos plantear la profilaxis contra patógenos oportunistas como <span class="elsevierStyleItalic">Pneumocystis jirovecii</span> con trimetroprim/sulfametoxazol o la vitamina D y calcio para la prevención de la osteoporosis inducida. No se ha demostrado que el uso de antibioterapia interfiera en la eficacia de la inmunoterapia<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">19,20</span></a>.</p><p id="par0085" class="elsevierStylePara elsevierViewall">Según las guías europeas y americanas, todos los pacientes en tratamiento con inmunoterapia deben seguir controles estrictos teniendo en cuenta los EAI repasados previamente. Estos regímenes de tratamiento suelen seguir esquemas de 2/3 semanas habitualmente. En las visitas previas al tratamiento debemos realizar anamnesis dirigida por órganos y sistemas, preguntando por las toxicidades más frecuentes, pero también indagando por las posibles toxicidades más sutiles. Como apoyo, es necesaria la realización de analítica previa a cada consulta con bioquímica que incluya función renal, iones, perfil hepático, perfil tiroideo y hemograma. En caso de sospecha clínica, se solicitarán pruebas destinadas al diagnóstico dirigido y se recomienda la derivación precoz al especialista implicado y el manejo multidisciplinar<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">15–17</span></a>.</p><p id="par0090" class="elsevierStylePara elsevierViewall">La inmunoterapia ha cambiado radicalmente el tratamiento de diferentes enfermedades oncológicas metastásicas con datos de supervivencia a largo plazo nunca vistos previamente en la oncología. Desgraciadamente no es un tratamiento exento de efectos adversos, siendo nuevos y diferentes a los conocidos clásicamente en la oncología, relacionados con la quimioterapia o los inhibidores de tirosina cinasa. Estos efectos adversos inmunomediados están provocados por una reacción «autoinmune» al hiperactivar a los linfocitos T CD8+. Debemos sospechar estos EAI utilizando herramientas básicas. La anamnesis por aparatos y sistemas apoyada en la exploración física y en los datos analíticos son la clave en diagnóstico.
