This affects patients’ lives, and they experience what is known as the “treatment burden”.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">The translation of The Exercise Therapy Burden Questionnaire<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">4</span></a> (ETBQ) into Spanish, originally created and validated in French, would allow the creation of multidisciplinary treatments that take into account the physical and psychological burdens that occur when working with therapeutic exercise in Spanish-speaking patients.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The aim of this study was to translate into Spanish, and validate the Exercise Therapy Burden Questionnaire (ETBQ) to assess the barriers facing patients with chronic ailments to doing the exercise prescribed by the doctor or physiotherapist.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Method</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Participants</span><p id="par0020" class="elsevierStylePara elsevierViewall">A transversal study was made with participants with the following criteria for inclusion: they had been prescribed exercise as part of their treatment, were aged over 18, had been diagnosed with chronic illness, spoke and understood Spanish, and had signed the informed consent form. The exclusion criteria were: cognitive problems that would hinder their understanding of the questionnaire.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Design</span><p id="par0025" class="elsevierStylePara elsevierViewall">The data were collected between April and December 2016, in a voluntary not probabilistic manner,in the Huercal healthcare centre in Almeria, the municipal sports centre and in the senior citizens’ day centre in Aguilas (Murcia). The data were collected informally, in three ways: on paper, by phone, or via a Google email platform.</p><p id="par0030" class="elsevierStylePara elsevierViewall">A sample of 57 patients was used for the retest, who repeated the same questionnaire between one and two weeks after first having done so.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The study was previously approved by the ethics committee of the province of Almería.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Description of the instrument</span><p id="par0040" class="elsevierStylePara elsevierViewall">The Exercise Therapy Burden Questionnaire<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">4</span></a> has 10 items to assess the barriers perceived by patients to doing therapeutic exercise; pain, tiredness, boredom, difficulty, waste of time, self-perception of the disease during exercise, lack of support, lack of motivation, ill adaption to the exercise, and self-perception of the effectiveness of the exercise.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Calculating the sample size</span><p id="par0045" class="elsevierStylePara elsevierViewall">A sample of between 150 and 300 patients was sufficient for a 10 item questionnaire, to obtain a Cronbach's alpha between 0.7 and 0.9, and a factorial analysis of components with less than three factors.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">5</span></a></p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Procedures</span><p id="par0050" class="elsevierStylePara elsevierViewall">Medical, socio-demographic data related to the exercise modality and related to the treatment with physiotherapy were collected. In addition to the ETBQ questionnaire translated into Spanish, self-efficacy for the exercise,<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">6</span></a> TBQ (the Treatment Burden Questionnaire),<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">7</span></a> and the EuroQol five dimensions questionnaire<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">8</span></a> (EQ-5D) in order to validate the divergence.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Translation</span><p id="par0055" class="elsevierStylePara elsevierViewall">The translation was done using the back-translation method according to the following steps<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">9</span></a>: The questionnaire obtained in French was subsequently submitted by committee to the authors of the original ETBQ to determine the equivalence between them.</p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Statistical analysis</span><p id="par0060" class="elsevierStylePara elsevierViewall">A factor components analysis was done to determine the number of dimensions comprising the questionnaire in Spanish. All the factors above 1 were retained (Kaiser criterion). The internal consistency of the ETBQ in Spanish was measured with Cronbach's alpha, and a coefficient of >0.70 up to 0.95 was considered acceptable.</p><p id="par0065" class="elsevierStylePara elsevierViewall">The reliability of the test–retest was measured through the intraclass correlation coefficient (ICC) and the Bland–Altman plot. In the ICC, a coefficient of ≥0.75 was considered an excellent reliability.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">10</span></a> In the Bland–Altman plot, 95% of the confidence limits were defined as the difference between the two means and the mean<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>1.96 of the two measures. To study the divergence of the total ETBQ with the rest of the questionnaires, Spearman's nonparametric test was used for the questionnaires on self-efficacy for exercise, the TBQ (The Treatment Burden Questionnaire), item 6 of EQ-5D, and the sum of the scores of the five remaining items of the EQ-5D. The software used for the statistics was R ×64 3.3.2 and SPSS version 22.0.</p></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Results</span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Sample</span><p id="par0070" class="elsevierStylePara elsevierViewall">One hundred and sixty-seven patients were recruited, 55.93% men and 44.07% women, with an average age of 51.03<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>14.91, of which 47.46% were of rheumatology, 11.3% diabetology, 1.69% neurology 1.69% pneumology, 26.55% systemic, 0.56% nephrology, 8.47% cardiovascular, and 2.26% oncology (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Factor analysis</span><p id="par0075" class="elsevierStylePara elsevierViewall">The result of the factor analysis was 3 components over 1 with self-scores of 4.724, 1.335 and 1.148 respectively, explaining a total variance of 72.062%.</p></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Cronbach's alpha analysis</span><p id="par0080" class="elsevierStylePara elsevierViewall">Cronbach's alpha was good for the first component of the ETBQ<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.899, questionable for the second component<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.695, and acceptable for the third component<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.746. Cronbach's alpha for the questionnaire as a whole was good<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.8715.</p></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Retest and divergence</span><p id="par0085" class="elsevierStylePara elsevierViewall">The retest showed good reliability with an ICC of 0.745 and a confidence interval of 95% between 0.604 and 0.841 (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). The divergence was done both for the total score of the ETBQ and for each of its components, resulting in a moderate to very low relation with the other questionnaires that were given to the patients (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Final score for the ETBQ in Spanish</span><p id="par0090" class="elsevierStylePara elsevierViewall">The global score for the ETBQ in Spanish ranges from 0 to 100 and is obtained from the total sum of the scores for the items. It was considered that the best approach was to add the scores of the items for each dimension and subsequently divide them by the total number of items in each dimension, which would thus score from 0 to 10.</p></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Other results</span><p id="par0095" class="elsevierStylePara elsevierViewall">Significant differences (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.021) were found for sex in the third dimension in the ETBQ questionnaire and also between the type of physical activity and the score for the second component of the ETBQ (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05).</p></span></span><span id="sec0085" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0140">Discussion</span><p id="par0100" class="elsevierStylePara elsevierViewall">The Exercise Therapy Burden Questionnaire<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">4</span></a> (ETBQ) is the first to be translated into Spanish that measures the burdens<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">3</span></a> facing chronic patients<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">1</span></a> when doing the physical exercise as well as take advantage of its benefit in the treatment of the chronic sickness.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">2</span></a> The ETBQ questionnaire in Spanish showed some differences with the original in French.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">4</span></a></p><p id="par0105" class="elsevierStylePara elsevierViewall">The ETBQ questionnaire in Spanish showed some differences with the original in French.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">4</span></a> In the test–retest process, with a result of ICC<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.93 for the original ETBQ compared to ICC<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.745 for the ETBQ in Spanish.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">10</span></a> This can be explained by the fact that the original questionnaire was based on a sample of 24 patients as opposed to 57. However, the internal consistency was excellent in both cases.</p><p id="par0110" class="elsevierStylePara elsevierViewall">This new questionnaire translated into Spanish, at a clinical level, will involve a better understanding of what the burden the type of physical exercise prescribed by the health professional supposes for the patient, and it will allow the better adaptation of the treatment to improve adherence thereto.</p><p id="par0115" class="elsevierStylePara elsevierViewall">This study presented the following limitations, the number of patients in the group for each ailment was insufficient to determine the prevalence of the limitations for exercise in each one, nor were found significant differences in the comparison of the ETBQ scoring between the, sociodemographic, medical, characteristics, the characteristics of the modality of the exercise type and of the physiotherapy treatment, except in relation to the sex and the type of activity. The number of patients who refused to complete the questionnaire was not recorded, nor was taken into account the manner in which the questionnaire was filled in (on paper, by telephone or by internet) to determine whether this influenced the results.</p></span><span id="sec0090" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0145">Conclusion</span><p id="par0120" class="elsevierStylePara elsevierViewall">We have obtained the validated version in Spanish of the ETBQ questionnaire, this questionnaire has been (2) Physical limitations for doing physical exercise. (3) Limitations caused by the patients’ predisposition to their exercises. The reliability of the test–retest was measured through the intraclass correlation coefficient (ICC) and the Bland–Altman plot. Cronbach's alpha was 0.8715 for the total ETBQ.
