CVE: cerebrovascular events.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Giant cell arteritis (GCA) is a large vessel vasculitis affecting mostly those arteries branching from the aorta, particularly the superficial temporal artery.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">1</span></a> It affects individuals over 50 years of age with a prevalence of approximately 10 in 10,000 patients.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">2</span></a> GCA tends to affect large and medium arteries containing elastic tissue within their walls. Involvement of the intracranial arteries is rare<a class="elsevierStyleCrossRefs" href="#bib0135"><span class="elsevierStyleSup">3–5</span></a> as they contain little or no elastic tissue, and because they lack vasa vasorum, through which inflammatory cells enter in the vessel wall.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">6</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">GCA complications can be serious, including blindness, stroke, and aortic dissection. Cerebrovascular events (CVE), including stroke and transient ischemic attack (TIA), are severe manifestations of GCA. They occur as early manifestations or concomitantly with GCA up to 4 weeks after the onset of glucocorticoid therapy in 1.5–7% of patients<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">7</span></a> and are not always identified as the first manifestation of the disease. In previous studies, the prevalence of CVE in GCA measured 2.8% (of 278 patients, 8 suffered a stroke).<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">8</span></a> The same rate was also found in a series of 180 biopsy-proven GCA patients from northern Italy.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">9</span></a> CVE most frequently affect elderly people. Moreover, due to the low prevalence of GCA, it can be misdiagnosed as a CVE of atherosclerotic cause or other common etiologies in this population if clinicians are not sufficiently vigilant to recognize CVE as a manifestation of GCA. While CVE are not the most frequent manifestation in GCA, and although GCA patients represent only 0.15% of all CVE cases,<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">10</span></a> CVE are the leading cause of mortality in GCA,<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">6,11,12</span></a> thus highlighting the importance of early diagnosis. In this context, our first objective was to determine the prevalence of CVEs in GCA and to alert clinicians to the importance of detecting CVE early in this disease.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0015" class="elsevierStylePara elsevierViewall">This is a retrospective observational study involving a cohort of 123 consecutive patients diagnosed with GCA at a single hospital from 2005 to 2016. Enrolled patients were those with new GCA diagnoses who fulfilled the American College of Rheumatology (ACR) 1990 classification criteria. All patients had a positive ultrasound test for the diagnosis of temporal arteritis in agreement with the EULAR recommendations on the use of imaging in large vessel vasculitis.<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">13</span></a> The acting clinician based his/her diagnosis on the patient's clinical history, blood tests and imaging results. Patients had at least an 18-month observation follow-up period to confirm the diagnosis.</p><p id="par0020" class="elsevierStylePara elsevierViewall">This study had the approval of the ethics committee and the clinical investigation commission at our center (HULP PI 2676).</p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Database procedures</span><p id="par0025" class="elsevierStylePara elsevierViewall">We reviewed the past neurological and general medical histories to obtain the signs, symptoms and ultrasound parameters of the first medical evaluation. All data were anonymized to preserve the privacy of the patients.</p><p id="par0030" class="elsevierStylePara elsevierViewall">We collected data regarding age, sex, past medical history of hypertension, diabetes, dyslipidemia, anterior ischemic optic neuropathy (AION), headache, visual disturbances, polymyalgia rheumatica (PMR), jaw claudication, general symptoms (fever, asthenia and weight loss), GCA-related CVE, at disease presentation and disease flares (after exclusion of other causes by a neurologist), and the following laboratory tests (before initiation glucocorticoid treatment): erythrocyte sedimentation rate (ESR), C reactive protein (CRP) and hemoglobin (Hb). Temporal artery biopsy (TAB) was performed according to clinical decision. We divided the sample into three groups: patients who suffered a CVE and cranial nerve palsies related to GCA, patients with an AION, and the rest of the study population. We defined neurologic ischemic event related to GCA, when it occurred during the first 4 weeks following diagnosis or relapse of GCA. In addition, CT scans were run on all patients and the information of all MRI studies available was recorded. Finally, the neurologist had to rule out other etiologies such as atherosclerosis, arrhythmia, bleeding and others causes.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Statistical analysis</span><p id="par0035" class="elsevierStylePara elsevierViewall">In the descriptive statistics, the frequencies for qualitative variables and the median and standard deviations for quantitative variables were used. The Fisher's exact test was used to compare qualitative results, the Mann–Whitney <span class="elsevierStyleItalic">U</span> test to analyze the quantitative nonparametric values, and ANOVA for differences between groups. SPSS program version 20.0 was used, and a value of <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.05 was considered statistically significant.</p></span></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0040" class="elsevierStylePara elsevierViewall">Of our 123-patient cohort, 37 (30.08%) suffered relevant ischemic events of the cranial arteries; of these, 25 patients (20.32%) had AION, and 12 suffered from neurologic symptoms other than AION, of whom 9 (7.3%) experienced ischemic events related to GCA, and 3 (2.44%), according to the diagnosis of the neurologist, presented a CVE due to other common causes most likely not related to GCA: two cases of atrial fibrillations and one case of large artery atherosclerosis, all three of which were recorded during disease follow-up (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">Of the 9 patients with CVE caused by GCA, 5 were diagnosed with transient ischemic attacks (TIAs), 2 with ischemic strokes and 3 with cranial nerve palsy, one of them also presenting a TIA. None of the patients in this group developed concomitant AION or presented lacunar infarct.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Of our 123 patients cohort, 32 (26.01%) patients underwent TAB, of these group, 13 (40.62%) patients were positive (evidence of multinucleated giant cells in the biopsy) and 19 (59.37%) patients negative. 50 (40.65%) patients were diagnosed of GCA without TAB and there is no data available regarding the TAB in 41 (33.33%) patients. In the CVE group, TAB was performed in 3 (33.3%) patients, where 1 (33.3%) patient was positive and 2 (66.6%) patients were negative.</p><p id="par0055" class="elsevierStylePara elsevierViewall">The analysis of the cohort characteristics as a whole are reported in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>, as are the characteristics for the three groups (CVE, AION and the rest of GCA patients). Although statistically significant differences are only evident in PMR (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.01), which had a lower prevalence in patients with stroke or TIA, <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a> also reveals clear differential trends between some characteristic features of the disease in patients with a TIA or stroke. It is interesting to note that the mortality rate decreased from 33% in the CVE group (3/9 patients: 1 patient died at the time of diagnosis due to cerebral vasculitis and 2 patients of natural causes) to 16% in the AION ischemic group and then further to 7.9% in the non-ischemic population.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">Among ischemia patients whose CVE was associated with GCA (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>), posterior circulation was affected in 4/9 patients; 4/7 if cranial nerve palsy was excluded. Interestingly, stenosis or occlusion of one (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2) or both (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2) vertebral arteries was found in the CT angiograms of these patients. In addition, in this group, 5 patients underwent MRI, showing one case of cerebral vasculitis.</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0065" class="elsevierStylePara elsevierViewall">GCA-related CVE are most likely underdiagnosed. The literature reports the incidence rate for events of GCA etiology to be 0.15% for general cerebrovascular ischemic events,<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">10</span></a> which is equivalent to one case in every 633 patients with a stroke or TIA. This low frequency in elderly patients, in whom arteriosclerosis is common, often causes delayed diagnosis of this pathology, with a high risk of recurrence and potentially avoidable death. In other words, the literature shows that stroke and TIA are serious complications of GCA, occurring in 1.5–7% of patients.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">7</span></a> GCA was found to be a rare form of ischemic stroke presentation of unusual cause, presented only in 5.7% of the cases (4 of 70 patients) in a hospital clinical series of 2000 consecutive stroke patients.<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">14</span></a> This discrepancy in frequency is probably due to differences in clinical suspicions and the origins of the patients; i.e., the frequency is probably higher in neurology departments, than in general rheumatology. Our study included patients from the rheumatology, ophthalmology, internal medicine, neurology and geriatric departments, as well as from the emergency room. Thus, it comprised a global sample of all the cases seen in our hospital and probably faithfully represented the broad spectrum of the disease, showing a prevalence of 7.3%, in the upper range of those frequencies reported.<a class="elsevierStyleCrossRefs" href="#bib0175"><span class="elsevierStyleSup">11,15</span></a> Two studies suggest that coexistent PMR can serve to identify a subset of GCA patients with a low risk of cranial ischemic symptoms.<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">16,17</span></a> However, three other studies demonstrated no such effect.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">8,15,18</span></a> It has also been identified that depending the chronologic occurrence of PMR during the GCA course, PMR may have been associated with a protective role or with an increased risk for developing GCA-related ischemic events.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">19</span></a> The protective effect of PMR is related to the hypothesis concerning two different patterns in GCA symptoms: one associated with PMR and one with an ischemic presentation.<a class="elsevierStyleCrossRefs" href="#bib0195"><span class="elsevierStyleSup">15,20</span></a> Our study supports the concept that PMR plays a protective role vis-à-vis cranial ischemia. However, this association must be weighed with cautious due to the retrospective nature of the study and the small sample size.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Few cases of CVE in patients with GCA have been reported in the literature. While our study confirms a tendency for the involvement of posterior circulation in stroke or TIA, other cerebral arteries were also affected in nearly half of the patients. We observed trends toward higher male affectation, in patients with CVE. The latter is probably related to the higher severity of arteriosclerosis in males, which facilitates ischemic events in cases involving vascular wall edemas. It has been described that vasculitis can cause lacunar infarcts by different mechanisms, like endothelial damage or most likely in GCA, related to hemodynamic alterations (stenosis of a penetrating artery), which may cause a status of distal low perfusion, normally presenting as TIA.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">21</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">Limitations of these results include the retrospective nature of this study and the fact that the number of patients is relatively small to draw conclusions. The prevalence reported of CVE includes 2 cases of cranial nerve palsy, although this can be caused not only by a stroke of TIA, but also by an ischemic affectation of the extracranial nerve or tissues such as ocular muscles. In our case, the prevalence of CVE was slightly lower (5.7%). Another limitation is that the frequencies presented would be altered if the proportion of patients originating from the neurology and other departments changed or if cases of CVE were underdiagnosed. Although not all patients presented a positive biopsy, all did have a positive ultrasound image. Ultrasound has recently been incorporated into the EULAR recommendations for the use of imaging in large vessel vasculitis in clinical practice,<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">22</span></a> and our group has extensive experience in this field.<a class="elsevierStyleCrossRefs" href="#bib0235"><span class="elsevierStyleSup">23,24</span></a> All of our patients fulfilled the GCA ACR classification criteria, and their diagnoses were supported by their clinicians after a long follow-up period.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conclusions</span><p id="par0080" class="elsevierStylePara elsevierViewall">In summary, in our study neither stroke nor TIA was found to be a rare form of GCA presentation (7.3%). We believe clinicians should be vigilant with all CVE that are associated with high acute-phase reactants and perhaps other ischemic non-polymyalgic symptoms of GCA in elderly patients. All patients fulfilled the American College of Rheumatology (ACR) 1990 GCA classification criteria and had a positive ultrasound test for GCA in agreement with the EULAR recommendations. Demographic and clinical parameters were recorded with special attention paid to ischemic cranial events.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">We studied 123 consecutive GCA patients, 74 (60.2%) women with a mean age of 79 years. Twelve patients (9.75%) suffered from neurologic symptoms other than AION, of whom 9 (7.3%) experienced ischemic events related to GCA and 3 (2.44%) likely experienced CVE due to other common causes. Of the 9 patients with CVE caused by GCA, 5 were diagnosed with transient ischemic attacks (TIAs), 2 with ischemic stroke, and 2 were cases involving cranial nerve palsies. High rates of mortality were found in patients with a TIA or stroke, while polymyalgia rheumatica (PMR) appeared to confer some protection against ischemic pathologies in GCA patients.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusions</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">Stroke and TIA are common presentation patterns associated with GCA and should be suspected in all CVE-related cases with high acute-phase reactants commonly present in the elderly.
