class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">A 23-year-old man presented with fever, joint pain, and skin lesions. It began 15 days previously with diarrhea and anal pain, and 10 days later associated inflammatory lower back pain, pain and swelling in the left knee (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>A), dactylitis of the left foot (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>B), erythematous patches with a slightly hyperkeratotic border on glans (circinate balanitis) and prepuce (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>C and D) and proctitis (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>E). No conjunctival or other cutaneous or mucosal involvement was found. As antecedent, he reported the practice of repeated receptive anal sex during the previous weeks. With the suspicion of reactive arthritis of sexual (anal) origin, rectal and urethral samples were taken. They corroborated the diagnosis with positivity in the anal polymerase chain reaction (PCR) for <span class="elsevierStyleItalic">Chlamydia trachomatis</span>. Blood tests and X-rays of the feet and knees were normal. Positivity for HLA-B27 was found. Treatment was started with doxycycline 100<span class="elsevierStyleHsp" style=""></span>mg orally 2<span class="elsevierStyleHsp" style=""></span>times/day for 7 days and anti-inflammatories. Three months after treatment, the patient was asymptomatic and a test of cure for <span class="elsevierStyleItalic">Chlamydia</span> was negative.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Given the increased incidence of venereal infections in recent years, their involvement as triggers of reactive arthritis should be considered.
