is a disease that has become a social hazard in recent years&#44; with a poor clinical prognosis&#46;<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">1</span></a> The spread of the disease in the general population can be attributed to high-risk factors such as drug use&#44; unsafe sexual practices&#44; and iatrogenic infections&#46;<a class="elsevierStyleCrossRefs" href="#bib0300"><span class="elsevierStyleSup">2&#44;3</span></a> HIV damages the CD4&#43; T lymphocytes of the immune system&#44; leading to cellular immune deficiency&#46; Severe lung infections&#44; intracranial infections&#44; and multiple organ failure syndrome are the main causes of death among most infected individuals&#46;<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">4</span></a> In addition to impairing the body&#39;s physiological functions&#44; HIV-positive patients also increases the susceptibility to bacterial&#44; fungal&#44; and viral infections&#46;<a class="elsevierStyleCrossRefs" href="#bib0315"><span class="elsevierStyleSup">5&#44;6</span></a> The impact of HIV on CD4&#43; T lymphocytes results in dysbiosis or reduced immunity&#44; leading to an increasing number of fungal infections in internal organs&#44; skin&#44; and mucous membranes&#46;<a class="elsevierStyleCrossRef" href="#bib0325"><span class="elsevierStyleSup">7</span></a> Recent years have seen a rise in oral candidiasis&#44; a disease of the oral mucosa caused by <span class="elsevierStyleItalic">Candida</span> infections&#44; among the HIV-positive patients&#46; Co-infections in HIV-positive patients are associated with a poorer clinical prognosis and higher mortality rate&#46;<a class="elsevierStyleCrossRef" href="#bib0330"><span class="elsevierStyleSup">8</span></a> Therefore&#44; improving the success rate of clinical treatment is a common concern among healthcare professionals&#46;</p><p id="par0010" class="elsevierStylePara elsevierViewall">There are several types of therapeutic agents used to treat oral candidiasis&#44; including polyenes&#44; azoles&#44; allylamines&#44; and echinocandins&#46;<a class="elsevierStyleCrossRef" href="#bib0335"><span class="elsevierStyleSup">9</span></a> These antifungal drugs have been the focus of research&#46; Polyene antifungal drugs bind to ergosterol on the fungal cell membrane&#44; creating micropores that alter the membrane&#39;s permeability&#46; This leads to an increase in the concentration of small molecules and ions within the cell&#44; as well as the potential leakage of macromolecules through the membrane&#46; Ultimately&#44; this results in the irreversible loss of intracellular components and the death of the fungus&#46;<a class="elsevierStyleCrossRef" href="#bib0340"><span class="elsevierStyleSup">10</span></a> Azoles inhibit the biosynthesis of ergosterol in fungal cell membranes through cytochrome P450-dependent enzymes&#44; which improves mycological cure rates&#46;<a class="elsevierStyleCrossRef" href="#bib0345"><span class="elsevierStyleSup">11</span></a> Clinical studies have also shown that azoles can reduce toxic side effects&#44; however&#44; clinicians are increasingly challenged by patients caused by azole-resistant <span class="elsevierStyleItalic">Candida</span> species&#46;<a class="elsevierStyleCrossRef" href="#bib0350"><span class="elsevierStyleSup">12</span></a> Echinocandins work by inhibiting fungal cell wall glucan synthase&#44; leading to fungal death&#46; Clinical studies have demonstrated the effectiveness of echinocandins in treating invasive candidiasis such as candidaemia and oesophageal candidiasis&#46;<a class="elsevierStyleCrossRef" href="#bib0355"><span class="elsevierStyleSup">13</span></a> However&#44; it remains unclear which antifungal agent is superior for patients with oral candidiasis and HIV infection&#46; Direct comparisons between different antifungal agents are still lacking in current research&#46;</p><p id="par0015" class="elsevierStylePara elsevierViewall">There is a lack of studies comparing different treatment options for oral candidiasis in HIV-positive patients&#44; leading to confusion in clinical decision-making&#46; Treatment options for the same patient vary across hospitals and physicians&#44; making it difficult to determine the most beneficial option&#46; Network meta-analysis &#40;NMA&#41; offers a solution by summarizing relevant literature&#44; excluding irrelevant studies&#44; comparing multiple interventions with similar characteristics&#44; and ranking them based on outcome metrics&#46; This study conducted a network meta-analysis to compare the effects of seven treatments on improving mycological cure rates in oral candidiasis patients with HIV infection&#44; aiming to provide guidance in choosing antifungal drugs&#46;</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0030">Materials and method</span><p id="par0020" class="elsevierStylePara elsevierViewall">This study was registered on PROSPERO with the registration number CRD42024513912&#46; The systematic review and network meta-analysis were carried out in accordance with the Cochrane Handbook for Systematic Reviews of Interventions&#44;<a class="elsevierStyleCrossRef" href="#bib0360"><span class="elsevierStyleSup">14</span></a> and the results are reported following the Preferred Reporting Items for Systematic Reviews and MetaAnalyses &#40;PRISMA&#41; extension statement for NMA&#46;<a class="elsevierStyleCrossRef" href="#bib0365"><span class="elsevierStyleSup">15</span></a></p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0035">Literature screening</span><p id="par0025" class="elsevierStylePara elsevierViewall">Adhering to the PICOS &#40;Participants&#44; Interventions&#44; Comparisons&#44; Outcomes&#44; and Study design&#41; principle&#44; the search terms in the strategy included&#58; &#40;P&#41; Individuals with HIV infection experiencing oral candidiasis&#59; &#40;I&#41; undergoing treatment with various antifungal medications&#59; &#40;C&#47;O&#41; assessing mycological healing rates&#59; &#40;S&#41; conducting a RCT&#46;</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Inclusion and exclusion criteria</span><p id="par0030" class="elsevierStylePara elsevierViewall">The studies included in our network meta-analysis met the following inclusion criteria&#58; &#40;a&#41; The study design was randomized controlled trials &#40;RCTs&#41;&#59; &#40;b&#41; The treatment group received polyenes&#44; azoles&#44; allylamines&#44; or echinocandins&#44; while the control group received other antifungal drugs&#44; placebo&#44; or no treatment&#59; with or without background treatments&#59; &#40;c&#41; The primary outcome measure was mycological cure rates&#59; &#40;d&#41; Only articles written in English were included&#46; The exclusion criteria were&#58; &#40;a&#41; Duplicate published articles or those with the same results&#59; &#40;b&#41; The theoretical research&#44; pathological studies&#44; reviews&#44; guidelines&#44; systematic reviews&#44; meta-analyses&#44; case reports&#44; cohort studies&#44; case&#8211;control studies&#44; nonrandomized studies&#44; or studies without matched control groups&#59; &#40;c&#41; Outcomes irrelevant to our analysis&#46;</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Search strategy</span><p id="par0035" class="elsevierStylePara elsevierViewall">In order to identify RCTs examining the clinical outcomes of various antifungal medications in the treatment of oral candidiasis among individuals with HIV infection&#44; a comprehensive search was conducted&#46; The databases searched included EMBASE&#44; Web of Science&#44; Medline&#44; and Cochrane&#44; encompassing articles published prior to August 2023&#46; Additionally&#44; the reference lists of all included articles were scrutinized to identify any additional relevant studies&#46; The search terms employed were as follows&#58; Acquired Immune Deficiency Syndrome&#59; Immunologic Deficiency Syndrome&#59; Human immunodificiency virus&#59; HIV&#59; Oral Candidiasis&#59; Oral Moniliases&#59; Antifungal drug&#59; Thrush&#59; randomized control study&#59; randomized controlled trial&#59; and RCT&#44; which were combined with &#8220;AND&#8221; or &#8220;OR&#8221;&#46; Two investigators reviewed the literature independently&#44; and a third investigator was involved in case that any disputes occurred&#46; The interexaminer agreement was calculated &#40;Kappa<span class="elsevierStyleHsp" style=""></span>&#61;<span class="elsevierStyleHsp" style=""></span>0&#46;94&#41;&#44; which indicated excellent agreement&#46; The search strategy is shown in detail in <a class="elsevierStyleCrossRef" href="#sec0120">supplementary list 1</a>&#46;</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Study selection and data extraction</span><p id="par0040" class="elsevierStylePara elsevierViewall">For each included study&#44; basic information of patients and primary study outcomes were extracted&#46; Basic information relevant to this meta-analysis was as follows&#58; author names&#44; year of publication&#44; intervention&#44; sample size&#44; and age&#46; Primary clinical outcomes relevant to this analysis were mycological cure rates&#46; The data extraction was performed independently by two investigators&#44; with a third resolving any disagreement when needed&#46;</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Quality analysis</span><p id="par0045" class="elsevierStylePara elsevierViewall">The quality of each included study was evaluated by the Cochrane Handbook using bias risk assessment tools&#46;<a class="elsevierStyleCrossRef" href="#bib0360"><span class="elsevierStyleSup">14</span></a> The certainty of evidence using the Confidence in Network Meta-Analysis framework &#40;CINeMA&#41;&#46;<a class="elsevierStyleCrossRef" href="#bib0370"><span class="elsevierStyleSup">16</span></a> The criteria for quality assessment included the following seven items&#58; random sequences generation &#40;selection bias&#41;&#44; allocation concealment &#40;selection bias&#41;&#44; blinding of participants and personnel &#40;performance bias&#41;&#44; blinding of outcome assessment &#40;detection bias&#41;&#44; incomplete outcome data &#40;attrition bias&#41;&#44; selective reporting &#40;reporting bias&#41;&#44; and other biases&#46; Each item can be rated as &#8220;low risk of bias&#8221;&#44; &#8220;unclear risk of bias&#8221; and &#8220;high risk of bias&#8221;&#46;<a class="elsevierStyleCrossRef" href="#bib0360"><span class="elsevierStyleSup">14</span></a></p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0060">GRADE rating for quality of evidence</span><p id="par0050" class="elsevierStylePara elsevierViewall">The quality of evidence and strength of recommendations were assessed using the GRAD tool designed by the Grading of Recommendations&#44; Assessment&#44; Development and Evaluation &#40;GRADE&#41; Working Group&#44; which evaluates outcome indicators in terms of study limitations&#44; publication bias&#44; indirectness of evidence&#44; inconsistency and imprecision of outcomes&#46;<a class="elsevierStyleCrossRef" href="#bib0375"><span class="elsevierStyleSup">17</span></a></p></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Statistical analysis</span><p id="par0055" class="elsevierStylePara elsevierViewall">R software &#40;version 4&#46;3&#46;0&#41;&#44; Stata14&#46;0&#44; and RevMan 5&#46;3 software &#40;Copenhagen&#58; The Nordic Cochrane Centre&#44; The Cochrane Collaboration&#44; 2014&#41; software were used for all analyses&#46; To conduct pairwise meta-analysis&#44; we evaluated heterogeneity in study findings through Chi-squared and <span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span> examinations&#46; Based on the results&#44; we determined the appropriate analysis models&#44; either fixed-effect or random-effect&#46; High heterogeneity was indicated by a Chi-squared <span class="elsevierStyleItalic">P</span> value of &#8804;0&#46;05 and an <span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span> value exceeding 50&#37;&#46; In such instances&#44; we employed a random-effects model&#46; Conversely&#44; acceptable heterogeneity was indicated by a Chi-squared <span class="elsevierStyleItalic">P</span> value of &#62;0&#46;05 and an <span class="elsevierStyleItalic">I</span><span class="elsevierStyleSup">2</span> value of &#8804;50&#37;&#46; In these cases&#44; a fixed-effects model was utilized&#46;<a class="elsevierStyleCrossRef" href="#bib0380"><span class="elsevierStyleSup">18</span></a> Comparison of continuous variables involved the calculation of odds ratio &#40;OR&#41; and 95&#37; confidence intervals &#40;CI&#41;&#46; Network plot was drawn to describe the interventions&#46; The size of the node reflected the number of patients in each intervention&#44; and the breadth of the edge showed the number of comparisons&#46; We calculated the surface under the cumulative ranking curves &#40;SUCRA&#41; to rank the competing interventions&#46; The larger the SUCRA value&#44; the greater the probability that the intervention will be better&#46; Node-splitting method as a local method and design-by-treatment interaction model as a global method were applied to detect the global and local inconsistencies&#46; Comparison-adjusted funnel plot was used to evaluate publication bias if the number of included studies&#46;</p></span></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Results</span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Literature search</span><p id="par0060" class="elsevierStylePara elsevierViewall">A total of seven hundred and twenty-six articles were acquired through our initial keyword search&#46; Out of these&#44; six hundred and sixty-six articles were excluded after a title-abstract review&#46; The remaining sixty articles underwent a comprehensive full-text assessment&#46; Among them&#44; thirty-four articles were excluded as they did not meet the study inclusion criteria&#46; The reasons for exclusion included&#58; not related to HIV infection&#44; lacking clinical outcomes&#44; and full text not in English &#40;<a class="elsevierStyleCrossRef" href="#sec0120">Supplementary list 2</a>&#41;&#46; Consequently&#44; we identified twenty-six studies that met the inclusion criteria for this meta-analysis&#46;<a class="elsevierStyleCrossRefs" href="#bib0385"><span class="elsevierStyleSup">19&#8211;44</span></a> These studies involved a total of 3145 patients and investigated seven interventions&#44; namely placebo&#44; fluconazole&#44; itraconazole&#44; nystatin&#44; clotrimazole&#44; ketoconazole&#44; and miconazole&#46; The selection process of the studies is illustrated in <a class="elsevierStyleCrossRef" href="#sec0120">supplementary Fig&#46; nystatin was the least effective antifungal drug in increasing mycological cure rates in HIV-positive patients &#40;<a class="elsevierStyleCrossRef" href="#fig0030">Fig&#46; 3</a>&#41;&#46;</p><elsevierMultimedia ident="fig0030"></elsevierMultimedia></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Consistency and publication bias assessment</span><p id="par0085" class="elsevierStylePara elsevierViewall">The consistency between direct and indirect evidence is a key assumption in network meta-analysis&#46; The node-splitting method is utilized to evaluate the extent of consistency between indirect evidence and direct evidence&#46; If the <span class="elsevierStyleItalic">P</span> value exceeded 0&#46;05&#44; the non-significance of the differences between each direct and indirect comparison was deemed&#46; Otherwise&#44; the differences were deemed significant&#46; The results showed for all the outcomes&#44; it was observed that each <span class="elsevierStyleItalic">P</span> value of the node-splitting model exceeded 0&#46;05 &#40;<a class="elsevierStyleCrossRef" href="#fig0035">Fig&#46; 4</a>&#41;&#46; This suggests that there were no considerable disparities observed between the outcomes of direct and indirect comparisons&#46; In <a class="elsevierStyleCrossRef" href="#fig0040">Fig&#46; 5</a>&#44; there was a no symmetry in the log OR funnel plot for bleeding event in these studies&#44; indicating no risk in the publication bias&#46;</p><elsevierMultimedia ident="fig0035"></elsevierMultimedia><elsevierMultimedia ident="fig0040"></elsevierMultimedia></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">GRADE of evidence evaluation</span><p id="par0090" class="elsevierStylePara elsevierViewall">Although the included studies were RCTs with the highest level of evidence&#44; the indirectness results of all the studies were also not serious&#46; However&#44; the inconsistency&#44; imprecision and publication bias&#44; reduced the evidence rating&#44; and the GRADE evidence rating for efficiency was low &#40;<a class="elsevierStyleCrossRef" href="#sec0120">Supplementary table 1</a>&#41;&#46;</p></span></span><span id="sec0080" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Discussion</span><p id="par0095" class="elsevierStylePara elsevierViewall">Our study aimed to provide a guide for selecting appropriate antifungal drugs for the treatment of oral candidiasis in HIV-positive patients&#46; The main findings of our research can be summarized as follows&#58; Firstly&#44; we conducted a synthesis of twenty-six RCTs&#44; and both conventional meta-analyses and network meta-analyses confirmed that fluconazole significantly improved mycological cure rates compared to placebo&#44; clotrimazole&#44; and nystatin&#46; Secondly&#44; among HIV-positive patients&#44; fluconazole ranked first in terms of increasing mycological cure rates&#44; while nystatin was found to be the least effective antifungal drug in increasing mycological cure rates&#46;</p><p id="par0100" class="elsevierStylePara elsevierViewall">In previous meta-analysis&#44; Pienaar et al&#46; developed a pairwise meta-analysis on oral candidiasis in HIV-positive patients&#44; and found there was no different between antifungal drugs and placebo in reducing mycological cure rates in HIV-positive patients&#46;<a class="elsevierStyleCrossRef" href="#bib0515"><span class="elsevierStyleSup">45</span></a> However&#44; this study included less literature and also did not perform an indirect analysis of the effect of different antifungal drugs on oral candidiasis in HIV-positive patients&#46; Fang et al&#46; collected thirty-one trails to analyze the available evidence of antifungal drugs in oral candidiasis&#44; and studied whether a diverse range of antifungal drugs were more or less effective than other antifungal drugs&#44; and found fluconazole ranked the first in reducing mycological cure rates&#46;<a class="elsevierStyleCrossRef" href="#bib0520"><span class="elsevierStyleSup">46</span></a> However&#44; this study didn&#8217;t study whether it was also applicable in HIV-positive patients&#46; Rajadurai et al&#46; preformed a network meta-analysis on oral candidiasis in HIV-positive patients&#44;<a class="elsevierStyleCrossRef" href="#bib0525"><span class="elsevierStyleSup">47</span></a> however&#44; this study only included seven trails and three drugs&#46; Therefore&#44; the conclusions need to be further confirmed&#46;</p><p id="par0105" class="elsevierStylePara elsevierViewall">By destroying T lymphocytes&#44; HIV inhibits both cellular and humoral immunity&#44; causing paralysis of the immune system and facilitating the spread of various diseases in the human body&#46;<a class="elsevierStyleCrossRef" href="#bib0530"><span class="elsevierStyleSup">48</span></a> Moreover&#44; the HIV virus itself cause damage to the body&#39;s chronic immune cells&#44; particularly lymphocytes&#46; This damage can lead to insufficient secretion of antibodies by downstream plasma cells and weakened antigen-presenting cells&#44; affecting overall immunity&#44; metabolism&#44; and organ function&#46;<a class="elsevierStyleCrossRef" href="#bib0535"><span class="elsevierStyleSup">49</span></a> HIV viruses possess RNA genomes&#44; enzymes&#44; and other components derived from host cells&#46; During reverse transcription&#44; these components are expressed and integrated into the genomes of normal somatic cells and poorly differentiated cells&#44; further impairing systemic immune function&#46;<a class="elsevierStyleCrossRef" href="#bib0540"><span class="elsevierStyleSup">50</span></a> Deficiencies in the immune function of CD4&#43; T lymphocytes may contribute to an increased incidence of infections&#44; including fungal and bacterial infections&#46;<a class="elsevierStyleCrossRef" href="#bib0545"><span class="elsevierStyleSup">51</span></a> Co-infection of oral candidiasis in HIV-positive patients further enhances the concentration and amount of structural proteins encoded by the gag gene of the HIV virus&#46; This leads to a significant increase in viral integrative efficiency&#44; tissue damage&#44; and a more pronounced decline in immune barrier function&#46;<a class="elsevierStyleCrossRef" href="#bib0550"><span class="elsevierStyleSup">52</span></a> Studies have shown that HIV-infected patients co-infected with oral <span class="elsevierStyleItalic">Candida</span> have a 15&#37; higher mortality rate over five years&#44; a 5&#37; higher disease progression rate&#44; and a significant increase in severe intracranial or pulmonary infections&#44; resulting in a poor prognosis&#46;<a class="elsevierStyleCrossRef" href="#bib0555"><span class="elsevierStyleSup">53</span></a> Therefore&#44; addressing the issue of oral candidiasis in HIV-positive patients is of utmost importance&#46;</p><p id="par0110" class="elsevierStylePara elsevierViewall">The principle of antifungal therapy for candidiasis is to optimize treatment based on the specific pathogenic fungi&#44; underlying diseases of the patients&#44; and characteristics of antifungal drugs&#46; Treatment strategies include targeted antifungal therapy for proven and probable cases of candidiasis&#46; Empirical treatment is used for cases of suspected invasive candidiasis with unknown pathogens and reductions in neutrophil count&#46;<a class="elsevierStyleCrossRef" href="#bib0560"><span class="elsevierStyleSup">54</span></a> This network meta-analysis found that fluconazole had the highest likelihood of achieving higher mycological cure rates compared to other antifungal drugs in the treatment of oral candidiasis in HIV-positive patients&#46; Large-scale clinical trials have demonstrated the effectiveness of fluconazole in treating oral candidiasis in patients with reduced neutrophil count&#46;</p><p id="par0115" class="elsevierStylePara elsevierViewall">Fluconazole has the characteristics of simple&#44; efficient and safe&#44; and plays an important role in the treatment of candidiasis&#46; Fluconazole is commonly used as a standard treatment for candidaemia in non-neutropenic patients&#46; This is due to its safety profile&#44; which includes a low risk of nephrotoxicity&#46;<a class="elsevierStyleCrossRef" href="#bib0565"><span class="elsevierStyleSup">55</span></a> It is particularly preferred for mild to moderate candidaemia in patients who are haemodynamically stable and have not been previously treated with azoles&#46; Fluconazole is highly effective when taken orally&#44; making it the first choice for treating oropharyngeal and oesophageal candidaemia&#46; It is also the standard of care for oropharyngeal&#44; oesophageal&#44; and vaginal candidiasis&#46; Additionally&#44; fluconazole can be used to treat CNS candidiasis and <span class="elsevierStyleItalic">Candida</span> endophthalmitis because it can easily penetrate the cerebrospinal fluid and ocular vitreous&#46;<a class="elsevierStyleCrossRef" href="#bib0570"><span class="elsevierStyleSup">56</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">The possible mechanisms of fluconazole in the treatment of oral candidiasis are as follows&#46; Fluconazole&#44; a member of the triazole class of compounds&#44; contains a triazole ring structure as its main active ingredient&#46; It has the ability to directly affect the cytochrome P450 and effectively bind to pathogenic bacteria&#46; This binding impedes the production of fungal sterols&#44; preventing the progression of the disease&#46; When fluconazole enters the body&#44; it effectively improves symptoms of oral candidiasis and reduces fungal activity&#46; Prolonged use of the drug also inhibits the fungus in deeper layers of the affected area&#46;<a class="elsevierStyleCrossRef" href="#bib0575"><span class="elsevierStyleSup">57</span></a> Fluconazole offers several advantages&#44; including high bioavailability&#44; long duration of action&#44; and minimal interference from antacids and food&#46; Studies have shown that fluconazole can reach a maximum blood concentration of 8&#46;0<span class="elsevierStyleHsp" style=""></span>mg&#47;L&#46; Additionally&#44; it has a relatively low affinity for plasma proteins&#44; allowing it to be widely distributed in blister fluid&#44; skin&#44; and other tissues&#46; Compared to other antibacterial drugs&#44; this drug not only demonstrates a good therapeutic effect but also presents fewer side effects&#44; making it well tolerated by patients and ensuring the desired treatment outcome&#46;<a class="elsevierStyleCrossRef" href="#bib0580"><span class="elsevierStyleSup">58</span></a> However&#44; prolonged use of fluconazole may still lead to side effects such as dizziness and diarrhea&#46; Therefore&#44; regular monitoring of liver function indexes is recommended when using fluconazole to enhance drug safety and protect patient health&#46; In conclusion&#44; fluconazole is an effective treatment for oral candidiasis with minimal side effects and a low recurrence rate&#44; making it highly recommendable&#46;</p><p id="par0125" class="elsevierStylePara elsevierViewall">This study aimed to collect and analyze clinical controlled studies on the treatment of oral candidiasis in HIV-positive patients&#46; The treatment protocols included in this study were comprehensive and systematic&#46; The results of both direct and indirect comparisons consistently indicated that fluconazole may be the most effective regimen for treating oral candidiasis in HIV-positive patients&#46; However&#44; it is important to note that the conclusions drawn from this study need to be verified by more high-quality controlled clinical studies&#44; as the number of included studies was limited and there was heterogeneity among some of the studies&#46; Furthermore&#44; The relatively early date of the inclusion of the literature resulted in the unclear and high risk of bias within included studies&#44; and this is the limitations in the current meta-analysis&#46; Besides&#44; the findings from the pairwise meta-analysis indicate that certain forest plots incorporate only one study&#44; raising concerns about the reliability of the interpretation&#46; Nevertheless&#44; the findings of this network meta-analysis are expected to assist clinicians and patients in selecting personalized treatment options&#46;</p></span><span id="sec0085" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Conclusion</span><p id="par0130" class="elsevierStylePara elsevierViewall">To summarize&#44; utilizing the existing data&#44; this study presents initial findings regarding the hierarchy of impacts produced by the seven interventions&#46; It&#39;s possible that Fluconazole exhibits the highest efficacy in boosting mycological cure rates among individuals with HIV infection&#46; Nevertheless&#44; owing to the constraints of the original investigation&#44; the substantiation backing this conclusion necessitates further validation through additional randomized controlled trials of superior quality in the future&#46;         "resumen" => "<span id="abst0005" class="elsevierStyleSection elsevierViewall"><p id="spar0005" class="elsevierStyleSimplePara elsevierViewall">Oral candidiasis infection is particularly prevalent among individuals in HIV-positive patients&#46; Antifungal drugs have shown promising therapeutic effects in treating oral candidiasis in HIV-positive patients&#46; However&#44; the selection of specific antifungal drugs for the treatment of oral candidiasis in HIV-positive patients lacks evidence-based guidelines&#46; This study aims to address this gap by conducting a comprehensive review of relevant randomized controlled trials &#40;RCTs&#41; and performing a network meta-analysis to assess the efficacy of different antifungal drugs in treating oral candidiasis in HIV-positive patients&#46; A systematic search was conducted in databases including EMBASE&#44; Web of Science&#44; Medline&#44; and Cochrane databases to identify relevant articles&#46; Additionally&#44; key pertinent sources in the literatures were also reviewed&#46; All studies published prior to August 2023 were eligible for inclusion&#46; Two researchers independently conducted the screening of literature&#44; extraction of data&#44; and evaluation of quality&#46; Pairwise and network meta-analysis were then performed to assess the primary outcomes of the randomized controlled trials &#40;RCTs&#41; included&#46; The protocol was registered on the PROSPERO database &#40;CRD42024513912&#41;&#46; Twenty-six RCTs were included in this meta-analysis&#44; involving a total of 3145 patients and evaluating seven interventions &#40;placebo&#44; fluconazole&#44; itraconazole&#44; nystatin&#44; clotrimazole&#44; ketoconazole&#44; miconazole&#41;&#46; Pairwise meta-analysis and network meta-analysis showed fluconazole was significantly efficacy in increasing mycological cure rates when compared with placebo&#44; clotrimazole&#44; and nystatin&#46; Ketoconazole and miconazole were significantly efficacy in increasing mycological cure rates when compared with nystatin&#46; Network meta-analysis also suggested the efficacy of the seven interventions in increasing mycological cure rates was ranked as follows&#58; placebo &#40;35&#46;3&#37;&#41;&#44; fluconazole &#40;95&#46;2&#37;&#41;&#44; itraconazole &#40;61&#46;6&#37;&#41;&#44; nystatin &#40;17&#46;0&#37;&#41;&#44; clotrimazole &#40;52&#46;7&#37;&#41;&#44; ketoconazole &#40;69&#46;2&#37;&#41;&#44; miconazole &#40;69&#46;1&#37;&#41;&#46; The available evidence indicates that fluconazole had the greatest possibility to increase mycological cure rates in HIV-positive patients&#44; while&#44; nystatin was the least effective antifungal drug in increasing mycological cure rates in HIV-positive patients&#46;</p></span>"
