Resumen
Objetivo

Diagnóstico prenatal de vasa previa en anomalías de inserción de cordón

Material y método

Se examinaron detalladamente 6 casos de anomalías en la inserción de cordón umbilical o de placentación

Resultados

En 3 casos se objetivaron vasos previos a la presentación y en relación con el orificio cervical interno. En dos casos de embarazos gemelares con una inserción velamentosa de cordón en la membrana intergemelar del segundo gemelo, la situación de las inserciones presentaba un vaso previo clínica de segundo gemelo tras el potencial nacimiento del primero. Un último caso correspondió a un tabique vascular interplacentario que terminó por ser previo a la presentación y próximo al orificio cervical interno

Conclusión

La ecografía bidimensional, el estudio Doppler pulsado y color permiten el diagnóstico prenatal de vasa previa en gestantes asintomáticas. Su diagnóstico se basa en su sospecha ante una inserción velamentosa o una placenta bilobulada. En estas situaciones, una evaluación detallada de la relación de los vasos amnióticos con el segmento uterino inferior o con la región previa a la presentación fetal llevará al diagnóstico de vasa previa

Objective

Prenatal diagnosis of vasa previa in cord insertion anomalies

Material and method

We examined six cases of cord insertion anomalies or placentation anomalies

Results

Three cases were diagnosed of vessels previous the presentation and near the internal cervical orifice. Two cases of twin pregnancy with a cord velamentous insertion of the second twin on the partition membrane, the situation of the insertions showed a clinical vasa previa of the second twin after the potential birth of the first twin. The last case showed an interplacental vascular septum, in a bilobed placenta, that was previous to the presentation and near the internal cervical orifice

Conclusion

The bidimensional ultrasounds, the pulsed and color Doppler studies allowed the prenatal diagnosis of the vasa previa in the asymptomatic pregnancies. Its diagnosis was based in its suspect in cases with a velamentous insertion or with a bilobed placenta. In these situations, a detailed evaluation of the relation of the amniotic umbilical vessels with the inferior uterine segment or with the uterine region previous to the fetus will allow the diagnosis of the vasa previa
