Presentamos el caso de una paciente de 78 años de edad con un he-matoma epidural espinal espontáneo que debutó con dolor interescapu-lar brusco junto con hemiparesia izquierda y mejoría significativa a los 15 minutos. Inicialmente diagnosticada de angor, la persistencia de dolor en raquis cervicodorsal aconseja la realización de una resonancia magnética (RM). Esta muestra una lesión en el conducto raquídeo, de localización epidural, desde C3 hasta D2, de señal heterogénea, con áreas hiperintensas en secuencias T1 y áreas hipointensas en eco de gradiente y sin signos de mielopatía compresiva. Diagnosticada de he-matoma epidural, debido a la evolución favorable de la clínica se optó por un tratamiento conservador. En la segunda RM se observó reabsor-ción completa del hematoma epidural

We present the case of a 78-year-old woman with a spontaneous spinal epidural hematoma that presented with sudden interscapular pain accompanied by left hemiparesis and a significant improvement 15 minutes later. Initially diagnosed as angina, the persistence of pain in dorsal cervical spine suggested the need to perform magnetic resonance imaging (MRI). The images demonstrated a lesion in the epidural spinal canal at level C3-D2 that presented a heterogeneous signal intensity, with hyperintense areas in T1-weighted sequences and hypointense areas in gradient-echo sequences, with no sign of compression myelopathy. A diagnosis of epidural hematoma was established and, given the favorable clinical course, conservative treatment was indicated. The second MRI study showed the complete resorption of the epidural hematoma.
