Functioning is understood as the operationalization of health and the manifestation of the interaction between a person's health condition and his/her contextual factors.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">2</span></a> As such, the utility of the ICF has increasingly enjoyed recognition in the rehabilitation community.</p><p id="par0010" class="elsevierStylePara elsevierViewall">In a paper introducing the recent changes and future plans of the International Society of Physical and Rehabilitation Medicine (ISPRM), the ICF was highlighted as one of the elements that would support its mission to “optimize functioning and health-related quality of life and minimize disability in persons with disability and/or medical problems throughout the world”.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">3</span></a> Specifically, the implementation of the ICF in rehabilitation practice has been included in ISPRM's work plan. Implementation efforts include the development of appropriate tools for assessing disability and collecting data at the population level as well as applying ICF Core Sets<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">2,4</span></a> as international standards for documentation and reporting in clinical routine along the continuum of care. Essential to the success of such implementation activities is the active support and close collaboration with physical and rehabilitation medicine (PRM) societies all over the world, such as the <span class="elsevierStyleItalic">Sociedad Española de Medicina Fisica y Rehabilitación</span> (SERMEF) and PRM leaders in Latin America.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Cross-border and cross-cultural input has been seen as vital in the development of internationally recognized ICF Core Sets. An <span class="elsevierStyleItalic">ICF Core Set</span> (ICF-CS) is a shortlist of categories from the full ICF classification that are considered essential for describing the functioning of a person with a specific health condition or in a specific healthcare context, irrespective of where the persons lives. Since the number of categories contained in ICF-CSs are considerably less than the 1400+ categories in the full ICF classification, ICF-CSs make the ICF more applicable for everyday use, for example as a minimal international standard for the assessment and reporting of functioning and health in clinical studies and comprehensive single or multi-professional clinical encounters.<a class="elsevierStyleCrossRefs" href="#bib0140"><span class="elsevierStyleSup">4,5</span></a> For each of the 35 ICF-CSs currently available there is at least one comprehensive and one brief version.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">4</span></a> Along with these ICF-CSs, the ICF Generic Set and the ICF Rehabilitation Set are also available.<a class="elsevierStyleCrossRefs" href="#bib0150"><span class="elsevierStyleSup">6,7</span></a> The <span class="elsevierStyleItalic">ICF Generic Set</span>, consisting of 7 ICF categories, can be employed to assess the functioning of the general population as well as patients with various health conditions. With 30 ICF categories including the 7 ICF Generic Set categories, the <span class="elsevierStyleItalic">ICF Rehabilitation Set</span> can be used to assess the clinical population along the continuum of care (acute, early-post acute, long-term, community rehabilitation).<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">7</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">ICF-CSs can serve as the foundation for the ICF-based tools that have been developed for the <span class="elsevierStyleItalic">Rehab-Cycle</span><span class="elsevierStyleSup">®</span>. The Rehab-Cycle is a problem-solving ICF-oriented iterative approach that can be integrated into existing rehabilitation management processes. It includes four key phases: (1) assessment, (2) assignment, (3) intervention and (4) evaluation.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">8–11</span></a> See <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">During the <span class="elsevierStyleItalic">assessment phase</span> the rehabilitation team together with the patient aim to get an overall picture of the patient's functioning at the beginning of rehabilitation, including problems in different areas of functioning and resources that can influence functioning. To achieve this aim, a comprehensive assessment is conducted by gathering functioning information from the patient's case history, various clinical and diagnostic tests, patient-reported outcome measures, patient interviews and observation, etc.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">2</span></a> The functioning information is documented from the perspective of the rehabilitation professional and patient alike. Based on this information, a functioning profile is created, mutually agreed long-term and short-term rehabilitation goals are set, and intervention targets (i.e. areas of functioning represented by ICF categories that are intended to be addressed with an intervention) are identified.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Once the intervention targets are identified, the <span class="elsevierStyleItalic">assignment phase</span> begins. This intervention planning phase involves the coordination of interventions, responsibilities and resources within a multidisciplinary team. During this phase, the interventions that are intended to address the intervention targets are identified and the rehabilitation professional(s) responsible for providing these interventions are assigned. The assignment phase is followed by the <span class="elsevierStyleItalic">intervention phase</span>, during which the defined interventions are conducted and regularly monitored.</p><p id="par0035" class="elsevierStylePara elsevierViewall">The final phase of the Rehab-Cycle is the <span class="elsevierStyleItalic">evaluation phase</span>. Aiming to determine whether the goals set for each intervention target identified in the assessment phase were reached, this phase involves evaluating the patient's functioning status before and after intervention. Note that this “before-after” picture of change does not necessarily signify that the change is due to the intervention itself, but only that there was some sort of change. The outcome of this second assessment of the patient's functioning can facilitate further rehabilitation planning.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Ideally applied in an interdisciplinary rehabilitation setting, the Rehab-Cycle can help the rehabilitation team members organize and coordinate their actions as well as structure documentation.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">8–11</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">For each phase of the Rehab-Cycle, an ICF-based documentation tool has been developed – the ICF Assessment Sheet, ICF Categorical Profile, ICF Intervention Table, and the ICF Evaluation Display. These tools aim to facilitate the documentation and planning of patient-oriented rehabilitation services. In addition, these tools foster transparent documentation and open communication among the rehabilitation team members in every phase of the Rehab-Cycle. The Rehab-Cycle encourages the team members to discuss and document the overall health status of a person at the beginning of rehabilitation (ICF Assessment Sheet), create a functioning profile that contains shared goals (ICF Categorical Profile), determine and document appropriate interventions and the respective rehabilitation team member responsible for each intervention (ICF Intervention Sheet), and discuss and conclude goal achievement (ICF Evaluation Display).<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">8,9,11–18</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">A simplified version of the <span class="elsevierStyleItalic">ICF Categorical Profile</span> was highlighted in a manual “ICF Core Sets – Manual for Clinical Practice”<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">2</span></a> that outlines one approach for applying ICF-CSs in clinical practice. Available in several languages including Spanish, this manual introduces a step-by-step process for creating a <span class="elsevierStyleItalic">functioning profile</span> for a specific patient. While both the functioning profile as described in the manual and the ICF Categorical Profile provide a visual display of the patient's functioning at the time of assessment, the ICF Categorical Profile additionally defines the shared goals set by the rehabilitation team and the patient that should be achieved through rehabilitation interventions.<a class="elsevierStyleCrossRefs" href="#bib0160"><span class="elsevierStyleSup">8,18</span></a> Moreover, while the ICF Categorical is most beneficial in a multidisciplinary rehabilitation setting that has integrated the whole Rehab-Cycle,<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">8</span></a> the functioning profile can be employed in any setting and for any purpose e.g. to determine a person's ability to work,<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">19,20</span></a> and is equally valuable when employed in a single-discipline assessment of functioning.</p><p id="par0055" class="elsevierStylePara elsevierViewall">To create an ICF-CS-based functioning profile the rehabilitation team first has to decide on the appropriate ICF-CS that will be employed as the basis for the assessment of the patient's functioning and the subsequent functioning profile. ICF-CSs can also be combined to assess patients with comorbidities, or selected categories from another ICF-CS that reflect the secondary health condition(s) can be added to the primary ICF-CS.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">2</span></a> Once the respective ICF-CS is selected, the rehabilitation team is required to rate the extent to which a patient is having a problem in each of the ICF-CS categories. To support the rating decision, there are various sources of information that the rehabilitation team members can take into account such as case history, physical examination, patient/caregiver interview, patient observation, diagnostic tests, patient-reported questionnaires, etc. The aforementioned manual provides case examples that illustrate this process of creating a functioning profile along with the corresponding documentation forms and all the ICF-CSs that were available at the time of publication.</p><p id="par0060" class="elsevierStylePara elsevierViewall">To ease the creation of an ICF-CS-based functioning profile for a specific person at a given point in time, a web-based documentation platform is available, <a href="http://www.icf-core-sets.org/">www.icf-core-sets.org</a><span class="elsevierStyleHsp" style=""></span><a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">21</span></a> that allows the user to select an appropriate ICF-CS, combination of ICF-CSs, and add categories from the whole ICF to create an individualized documentation form. This form is then used to rate the person's problem (or lack thereof) in the selected ICF-CS categories. The user is also able to indicate the sources of information considered for the rating as well as enter clarifying text. Once the documentation form is completed a functioning profile can be generated. The web-based documentation platform is also available in various languages including Spanish. See <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">The ICF-based documentation form, regardless of paper or electronic version, offers a standardized and comprehensive tool for information-gathering and for generating a biopsychosocial overview of a person's functioning. This could, in turn, influence the rehabilitation process. A comprehensive and accurate assessment of the patient from the start is essential to a well-planned rehabilitation program. ICF-CSs and ICF-based tools such as the ICF Categorical Profile, the ICF-based documentation form and functioning profile can drive the assessment process, and in conjunction with the aforementioned ICF Intervention Table and ICF Evaluation Display, can guide the subsequent phases of a patient's Rehab-Cycle. That is, these tools can facilitate the definition of rehabilitation goals, identification of intervention targets, intervention planning including allocation of responsibilities of the patient and rehabilitation team, monitoring of interventions, and evaluation of patient progress and goal achievement.<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">2,8,12–18</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">Last but not least, ICF-based tools can facilitate communication not only between members of the rehabilitation team but also between the rehabilitation team and the patient, and perhaps with other stakeholders such as insurers and healthcare providers in the patient's community.<a class="elsevierStyleCrossRefs" href="#bib0170"><span class="elsevierStyleSup">10–13,15,17,18,22</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">Along with the clear benefits of using ICF-CSs in clinical practice, there are also challenges that rehabilitation professionals may face in using ICF-CSs. Among the challenges is the selection of the appropriate ICF-CS for a health condition for which no ICF-CS is available. For this situation, rehabilitation professionals can follow the guiding principles outlined for the joint use of the International Classification of Diseases (ICD) and the ICF.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">23</span></a> If no ICF-CS is available, a related ICF-CS can be used. For example, the ICF-CS for chronic obstructive pulmonary disease (COPD) can be used to assess a patient with tuberculosis, since no ICF-CS is available for tuberculosis. Another option is also to combine ICF-CSs or add categories from related ICF-CSs.
