La osteonecrosis multifocal se define como la presentación de osteonecrosis en tres o más estructuras óseas. La cabeza femoral es el lugar que en más ocasiones se ve alterado, seguido de la rodilla, el hombro y el tobillo. En la literatura revisada la cadera es la localización inicial que aparece con más frecuencia.
La osteonecrosis es una complicación ortopédica después del trasplante de órganos. La patogenia no está bien establecida aunque se conocen múltiples factores de riesgo. Exponemos el caso de un paciente con trasplante cardíaco que en su evolución presentó osteonecrosis de un hombro y posteriormente de cabeza femoral bilateral. Describimos el diagnóstico, tratamiento, evolución y revisamos la bibliografía de esta patología.
Multifocal osteonecrosis is defined as the presentation of osteonecrosis affecting three or more osseous structures. The femoral head is the most common site altered, followed by the knee, shoulder and ankle. According to bibliography consulted, the hip is the initial site that most frequently appears.
Osteonecrosis is an orthopedic complication after organ transplants. Its pathogeny is not well established although multiple risk factors are known. We present the case of a heart transplanted patient who presented in his evolution osteonecrosis, first in the shoulder and later in the bilateral femoral head. We describe diagnosis, treatment, evolution and review bibliography regarding this disease.
