Resumen

Los ultrasonidos han sido ampliamente usados en numerosas patologías: contracturas, tendinitis, bursitis, etc. El objetivo de este estudio ha sido revisar las indicaciones y eficacia del tratamiento con ultrasonidos en la patología osteomuscular.

Material y métod

Se ha basado en artículos recogidos de las bases de datos Medline, Pubmed y de la página web BMJ (clinical evidence) desde el año 1995 hasta el 2001. Se localizaron 648 trabajos de los cuales incluimos 47: de ellos, 12 eran revisiones de ensayos randomizados y controlados, un meta-análisis, 18 eran ensayos randomizados y controlados, tres artículos de casos aislados y 12 estudios experimentales.

Resultados

Hombro-siete estudios (cinco ensayos randomizados y controlados); ligamentos-nueve estudios (siete ensayos randomizados y controlados); úlceras venosas-dos revisiones de ensayos randomizados y controlados; síndrome de túnel carpiano-tres estudios (dos ensayos randomizados y controlados); tejido muscular (experimentales); tejido óseo-17 estudios (nueve in vitro/ animales y ocho en humanos); dolor lumbar-cinco estudios (tres ensayos randomizados y controlados y un meta-análisis); patología inflamatoria muscular-un ensayo randomizado y controlado; dolor miofacialtres ensayos randomizados y controlados.

Conclusiones

En general, no se observa gran eficacia con esta técnica. Las patologías en las que se encuentra mayor probabilidad de eficacia son: calcificaciones tendinosas del hombro, síndrome del túnel carpiano, osteoradionecrosis, fracturas recientes, pseudofracturas y retraso de consolidación. Se observa menor probabilidad en: patología del hombro, de ligamentos (en la epicondilitis puede haber efectos beneficiosos sobre la mejoría del dolor), dolor lumbar, patología inflamatoria muscular y dolor miofascial.

Summary

Ultrasounds have been widely used in many diseases: contractures, tendinitis, bursitis, etcétera. The objective of our study has been to review the indications and efficacy of treatment with ultra-sounds in osteomuscular disease.

Material and methods

We have based this on articles gathered from the Medline, Pubmed and web page BMJ (clinical evidence) data bases from 1995 to 2001. 648 papers were located, 46 of which we include: 12 reviews of randomized and controlled trials, one meta-analysis, 17 randomized and controlled trials, three articles on isolated cases and 12 experimental studies.

Results

Shoulder-seven studies (five randomized and controlled trials); ligaments-nine studies (seven randomized and controlled trials); venous ulcers-two reviews of randomized and controlled trials; carpal tunnel syndrome – three studies (two randomized and controlled trials); muscle tissue (experimental); bone tissue - 17 studies (nine in vitro/ animals and eight in humans); low back pain – five studies (three randomized and controlled trials and one meta-analysis); muscular inflammatory disease – one randomized and controlled trial; myofascial pain – three randomized and controlled trials.

Conclusions

In general, much efficacy is not observed. The diseases in which the most probability of efficacy are found would be: tendinous calcifications of the shoulder, carpal tunnel syndrome, osteoradionecrosis, recent fractures, pseudofractures and consolidation delay. Less probability is observed in: shoulder pathology, ligament pathology (in epicondyiltis, there can be beneficial effect on the improvement of pain), low back pain, muscular inflammatory disease and myofascial pain.
