La triple artrodesis del tarso (astrágalo-calcánea, astrágaloescafoidea y calcáneo-cuboidea) es una intervención quirúrgica utilizada frecuentemente para corregir deformidades del retropié –como pie plano, pie cavo o pie equino–, y para el tratamiento de la artrosis a este nivel. Clásicamente, esta intervención se ha realizado mediante un abordaje lateral para el pie cavo y un doble abordaje medial y lateral para el pie plano.

La artrodesis subastragalina y astrágalo-calcánea a través de un único abordaje medial en pacientes con pie plano valgo severo permite una óptima corrección de la deformidad del retropié, reduciendo la complejidad quirúrgica y el riesgo de complicaciones cutáneas, y respetando la articulación calcáneo-cuboidea.

En este trabajo se exponen las indicaciones y contraindicaciones de esta intervención, sus ventajas e inconvenientes, y se realiza una descripción detallada de la técnica quirúrgica.

Triple hindfoot arthrodesis (subtalar, talo-navicular and calcaneo-cuboid) is a surgical procedure commonly used to correct hindfoot deformities, as flat foot, cavus foot or equinnus foot, and to treat osteoarthritis of the hindfoot. Classically, this operation has been performed through a lateral approach for cavus foot and through a double medial and lateral approach for flat foot.

Subtalar and talonavicular arthrodesis through a single medial approach in patients with severe planovalgus deformity provides good correction of the hindfoot deformity, decreasing surgical complexity and the risk of skin problems, and preserving calcaneocuboid joint.

In this paper we present the indications and contraindications of this procedure, its advantages and disadvantages and we describe in detail the surgical technique.
