La rotura del tendón del tibial posterior (TTP) como complicación de las fracturas cerradas de tobillo sigue siendo una entidad inusual. Ésta se ve más frecuentemente relacionada con enfermedades sistémicas como la artritis reumatoide. El mecanismo lesional más frecuentemente relacionado es el de pronación-rotación externa. Existen indicios tanto preoperatorios como intraoperatorios para pensar en dicha entidad. Es importante no subestimar la rotura para poder evitar problemas posteriores en la biomecánica del pie.

En este artículo presentamos un caso clínico de rotura parcial de TTP en una fractura cerrada de maléolo interna, junto a una revisión bibliográfica al respecto.

The posterior tibial tendon rupture (PTT) as complication of closed fractures of the ankle remains as a rare entity. This is most commontly associated with systemic diseases such as rheumatoid arthritis. The injury mechanism is most likely related to pronation and external rotation. Preoperative and intraoperative evidences are presented and help us to think about it. It is important not to underestimate the rupture to avoid problems in the foot's biomechanics.

In this article we present a case of posterior tibial tendon partial rupture in a closed medial malleolar fracture, along with a literature review about it.
