Objetivo

conocer los factores que determinan la derivación a centros sociosanitarios o residencias geriátricas en pacientes ancianos hospitalizados.

Pacientes y método

estudio observacional de seguimiento de una cohorte de pacientes ancianos hospitalizados. Se recogió información referente a características sociodemográficas, comorbilidades, síndromes geriátricos, capacidad funcional y capacidad cognitiva, así como el destino al alta.

Resultados

se estudió a 585 pacientes con una edad media de 84 años. Se institucionalizó al 15,4%. No se encontraron diferencias en la edad, el sexo y el índice de comorbilidad de Charlson entre los institucionalizados y los que no. El análisis multivariante señaló como factores de riesgo de institucionalización la pérdida de la capacidad funcional, los antecedentes de caídas y la inmovilidad.

Conclusiones

la información adquirida al ingreso referente a la capacidad funcional y las caídas del paciente puede ser útil para la planificación del alta.

to identify risk factors for admission to nursing homes in elderly patients discharged from acute hospitals.

Patients and method

we performed an observational study in which a cohort of elderly people was followed-up from admission to discharge. Information on sociodemographic characteristics,comorbidities, geriatric syndromes, functional and mental capacity, as well as destination on discharge was registered.

Results

a total 585 patients with a mean age of 84 years were studied. Of these, 15.4% were institutionalised. No differences in age, sex or Charlson score were found between individuals who were institutionalised and those who were not. Multivariate analysis showed loss of functional capacity, immobilisation and accidental falls in the previous year as independent risk factors for admission to nursing homes.

Conclusions

information acquired at admission on functional capacity and falls could be useful for planning discharge.
