He came to the emergency department with sepsis of probable abdominal origin. An abdomino-pelvic computed tomography (CT) scan without intravenous contrast (IVC) was performed in which the origin of the infectious focus could not be located; however, metastatic lesions were evidenced in both hepatic lobes (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). Given the impossibility of locating the infectious focus, technetium 99 metastable-hexamethylpropyleneamine (<span class="elsevierStyleSup">99m</span>Tc-HMPAO) labeled autologous leukocytes scintigraphy was requested, without evidence of active infectious process, however, a heterogeneous uptake of the radiotracer was observed in the hepatic parenchyma, in relation to bilobar hepatic metastatic disease (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). Shortly thereafter, the patient died of multiorgan failure.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleSup">99m</span>Tc-HMPAO labeled autologous leukocytes scintigraphy shows physiological uptake of the radiopharmaceutical in the liver because the main cells of the endoplasmic reticulum system (ERS) called Kupffer cells (KC) are responsible for phagocytosing leukocytes.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Pathophysiological studies have shown that, in the presence of liver damage, KC decrease significantly causing an alteration in the reticuloendothelial and phagocytic function of the liver. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
