The structural studies (CT and MRI) described a left parasternal mass showing infiltrative characteristics along with ipsilateral pleural effusion. A biopsy of the mass was performed revealing a T-predominant mixed lymphoid component, identifying clear clusters of pleomorphic large cells with a CD30 + / ALK- phenotype, with high Ki-67. The genetic study was indicative of polyclonality for TCRG and TCRB. These results were congruent with the diagnosis of breast implant-associated anaplastic large cell lymphoma (BIA-ALCL).</p><p id="par0010" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleSup">18</span>F-FDG PET/CT was requested for staging (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). The metabolic images showed an heterogeneous uptake of the tracer in the peripheral effusion of the left breast seroma, infiltrating the chest wall and expressing a high rate of cell proliferation with a SUVmaximum up to 13.18 in its parasternal portion. Also, multiple pathological hypermetabolic adenopathies were identified in the bilateral axillary (predominantly left), left internal mammary, left prevascular, left hilar, subcarinal and left diaphragmatic (blue arrow) regions; reaching sizes up to 24 mm and a SUV maximum of 22.13 in the left retropectoral region. These findings are suggestive of malignancy. The left pleural effusion presented an increased FDG uptake (maximum SUV 2.78), making imperative to discard its malignancy.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">After 6 cycles of chemotherapy, a partial metabolic response of lymphadenopathy and peripheral soft tissues involvement was shown (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>). In addition, a complete resolution of the pleural effusion was observed.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">The BIA-ALCL is a rare and under-recognized subtype of T-cell lymphoma whose population incidence is increasing in the last years<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>. The main cause and principal pathogenesis mechanism have still not been identified<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a>. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
