A hypercapillary focus is seen at the level of the costal lytic lesion in association with elevated somatostatin receptor expression.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><p id="par0005" class="elsevierStylePara elsevierViewall">A 72-year-old woman was referred to the department of Internal Medicine for asthenia, anorexia and weight loss of up to 6 kg in the last two months. Laboratory tests and physical examination showed no relevant alterations. CT scan of the abdomen with intravenous contrast showed an 8 mm lesion in the pancreatic tail-body with peripheral enhancement in arterial phase. This lesion was confirmed in an abdominal MRI in which, despite not showing a typical uptake pattern, the radiological semiology was suggestive of a neuroendocrine tumor.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Given the diagnostic suspicion, somatostatin receptor scintigraphy was performed with [111In] pentetreotide (Octreoscan®). Whole-body planar images were acquired at 4 and 24 hours post-injection, and thoracoabdominal SPECT/CT at 4 hours. The pancreatic lesion showed no uptake of the radiopharmaceutical, although a pathological deposit was observed in the left first rib corresponding to a lytic lesion of approximately 3.8 x 1.9 cm with no associated soft tissue mass (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">Whole-body [18F] FDG PET/CT showed a hypermetabolic focus at the level of the costal lesion. In this case, increased metabolism was also not observed in the pancreatic lesion under study (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0020" class="elsevierStylePara elsevierViewall">A fine needle aspiration biopsy of the lytic lesion was performed, which showed histological findings compatible with plasma cell tumor with features of intermediate maturity, with kappa monoclonality.</p><p id="par0025" class="elsevierStylePara elsevierViewall">Whole-body MRI, 24-hour urine protein measurement, aspiration and bone marrow flow cytometry showed no significant alterations, ruling out the possibility of multiple myeloma. Given these results and the absence of CRAB criteria (hypercalcemia, renal failure, anemia or other bone lesions), the final diagnosis was solitary bone plasmacytoma. The patient was treated with local radiotherapy in the costal lesion and an active surveillance of the pancreatic finding was decided. So far, after 6 months, the patient is asymptomatic and is considered to be in remission.</p><p id="par0030" class="elsevierStylePara elsevierViewall">In our case, whole-body somatostatin receptor scintigraphy with SPECT/CT was key to detect rib infiltration and thus refer to other procedures confirming the presence of a solitary bone plasmacytoma. This imaging test could be a valuable tool to detect disease activity, especially in patients with relapsed myeloma, providing in combination with other techniques (such as [18F] FDG PET/CT) an interesting pathophysiological insight in the management of plasma cell disorders<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>. Therefore, in the evaluation of neuroendocrine tumors with somatostatin receptor imaging, in the presence of exclusively bone involvement, the possibility of plasma cell tumor should be ruled out<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3</span></a>. The Revista Española de Medicina Nuclear e Imagen Molecular (Spanish Journal of Nuclear Medicine and Molecular Imaging), was founded in 1982, and is the official journal of the Spanish Society of Nuclear Medicine and Molecular Imaging, which has more than 700 members. The Journal, which publishes 6 regular issues per year, has the promotion of research and continuing education in all fields of Nuclear Medicine as its main aim. For this, its principal sections are Originals, Clinical Notes, Images of Interest, and Special Collaboration articles. The works may be submitted in Spanish or English and are subjected to a peer review process. In 2009, it became the leading Spanish journal in the field of Medical Imaging on having an Impact Factor , awarded by the Journal Citation Reports.
